    In her analysis of pejoratives, Eva Picardi rejects a too sharp separation between descriptive and expressive content. I reconstruct some of her arguments, endorsing Eva’s criticism of Williamson’s analysis of Dummett and developing a suggestion by Manuel Garcia Carpintero on a speech act analysis of pejoratives. Eva’s main concern is accounting for our instinctive refusal to endorse an assertion containing pejoratives because it suggests a picture of reality we do not share. Her stance might be further developed claiming that uses of pejoratives not only suggest, but also promote a wrong picture of reality. Our refusal to endorse implies rejecting not only a wrong picture of reality but also a call for participation to what that picture promotes

    Considering the impact of situation-specific motivations and constraints in the design of naturally ventilated and hybrid buildings

    A simple logical model of the interaction between a building and its occupants is presented based on the principle that if free to do so, people will adjust their posture, clothing or available building controls (windows, blinds, doors, fans, and thermostats) with the aim of achieving or restoring comfort and reducing discomfort. These adjustments are related to building design in two ways: first the freedom to adjust depends on the availability and ease-of-use of control options; second the use of controls affects building comfort and energy performance. Hence it is essential that these interactions are considered in the design process. The model captures occupant use of controls in response to thermal stimuli (too warm, too cold etc.) and non-thermal stimuli (e.g. desire for fresh air). The situation-specific motivations and constraints on control use are represented through trigger temperatures at which control actions occur, motivations are included as negative constraints and incorporated into a single constraint value describing the specifics of each situation. The values of constraints are quantified for a range of existing buildings in Europe and Pakistan. The integration of the model within a design flow is proposed and the impact of different levels of constraints demonstrated. It is proposed that to minimise energy use and maximise comfort in naturally ventilated and hybrid buildings the designer should take the following steps: 1. Provide unconstrained low energy adaptive control options where possible, 2. Avoid problems with indoor air quality which provide motivations for excessive ventilation rates, 3. Incorporate situation-specific adaptive behaviour of occupants in design simulations, 4. Analyse the robustness of designs against variations in patterns of use and climate, and 5. Incorporate appropriate comfort standards into the operational building controls (e.g. BEMS)

    Clinical profile and drug utilization pattern in an intensive care unit of a teaching hospital in western Nepal

    To analyze the clinical profile of patients admitted to the intensive care unit (ICU) of Manipal Teaching Hospital (MTH) at Pokhara, Nepal, identify the commonly prescribed drugs, drug categories, dosage forms, antimicrobials, sensitivity pattern of antimicrobials and the treatment outcomes. A cross sectional, descriptive study in which he case records of all the patients admitted in the ICU during 1st August to 30th September, 2007 were collected and the details were entered in the patient profile form. The filled patient profile forms were retrospectively analyzed as per the study objectives. Altogether, 201 patients [males 101 (50.25%)] were admitted. Most common diagnosis was 'Myocardial Infarction /Ischemic heart disease' [13.96 % (n=62)]. The median (interquartile range) of the ICU stay was 3 (2-4) days. Cardiovascular drugs [31.7% (n=761) were the most commonly prescribed. Among the antimicrobials, metronidazole was most commonly prescribed followed by ceftriaxone. The morality rate in the ICU was 17.41 % and the major causes of mortality were cardiovascular and respiratory diseases. Antimicrobials was the most common drug category used in the ICU and 'pantoprazole' was the most commonly prescribed individual drug.     Drug-Resistant Tuberculosis among HIV-Infected Patients Starting Antiretroviral Therapy in Durban, South Africa

    Objective: To estimate the prevalence of drug-resistant tuberculosis (TB) and describe the resistance patterns in patients commencing antiretroviral therapy (ART) in an HIV clinic in Durban, South Africa. Design Cross-sectional cohort study. Methods Consecutive HIV-infected adults (≥18y/o) initiating HIV care were enrolled from May 2007–May 2008, regardless of signs or symptoms of active TB. Prior TB history and current TB treatment status were self-reported. Subjects expectorated sputum for culture (MGIT liquid and 7H11 solid medium). Positive cultures were tested for susceptibility to first- and second-line anti-tuberculous drugs. The prevalence of drug-resistant TB, stratified by prior TB history and current TB treatment status, was assessed. Results: 1,035 subjects had complete culture results. Median CD4 count was 92/µl (IQR 42–150/µl). 267 subjects (26%) reported a prior history of TB and 210 (20%) were receiving TB treatment at enrollment; 191 (18%) subjects had positive sputum cultures, among whom the estimated prevalence of resistance to any antituberculous drug was 7.4% (95% CI 4.0–12.4). Among those with prior TB, the prevalence of resistance was 15.4% (95% CI 5.9–30.5) compared to 5.2% (95% CI 2.1–8.9) among those with no prior TB. 5.1% (95% CI 2.4–9.5) had rifampin or rifampin plus INH resistance. Conclusions: The prevalence of TB resistance to at least one drug was 7.4% among adults with positive TB cultures initiating ART in Durban, South Africa, with 5.1% having rifampin or rifampin plus INH resistance. Improved tools for diagnosing TB and drug resistance are urgently needed in areas of high HIV/TB prevalence

    The Gemini Planet Imager Exoplanet Survey: Giant Planet and Brown Dwarf Demographics From 10-100 AU

    We present a statistical analysis of the first 300 stars observed by the Gemini Planet Imager Exoplanet Survey (GPIES). This subsample includes six detected planets and three brown dwarfs; from these detections and our contrast curves we infer the underlying distributions of substellar companions with respect to their mass, semi-major axis, and host stellar mass. We uncover a strong correlation between planet occurrence rate and host star mass, with stars M >> 1.5 MM_\odot more likely to host planets with masses between 2-13 MJup_{\rm Jup} and semi-major axes of 3-100 au at 99.92% confidence. We fit a double power-law model in planet mass (m) and semi-major axis (a) for planet populations around high-mass stars (M >> 1.5M_\odot) of the form d2Ndmdamαaβ\frac{d^2 N}{dm da} \propto m^\alpha a^\beta, finding α\alpha = -2.4 ±\pm 0.8 and β\beta = -2.0 ±\pm 0.5, and an integrated occurrence rate of 94+59^{+5}_{-4}% between 5-13 MJup_{\rm Jup} and 10-100 au. A significantly lower occurrence rate is obtained for brown dwarfs around all stars, with 0.80.5+0.8^{+0.8}_{-0.5}% of stars hosting a brown dwarf companion between 13-80 MJup_{\rm Jup} and 10-100 au. Brown dwarfs also appear to be distributed differently in mass and semi-major axis compared to giant planets; whereas giant planets follow a bottom-heavy mass distribution and favor smaller semi-major axes, brown dwarfs exhibit just the opposite behaviors. Comparing to studies of short-period giant planets from the RV method, our results are consistent with a peak in occurrence of giant planets between ~1-10 au. We discuss how these trends, including the preference of giant planets for high-mass host stars, point to formation of giant planets by core/pebble accretion, and formation of brown dwarfs by gravitational instability.Comment: 52 pages, 18 figures.     Where is the jet quenching in Pb+Pb collisions at 158 AGeV?

    Because of the rapidly falling particle spectrum at large pTp_T from jet fragmentation at the CERN SPS energy, the high-pTp_T hadron distribution should be highly sensitive to parton energy loss inside a dense medium as predicted by recent perturbative QCD (pQCD) studies. A careful analysis of recent data from CERN SPS experiments via pQCD calculation shows little evidence of energy loss. This implies that either the life-time of the dense partonic matter is very short or one has to re-think about the problem of parton energy loss in dense matter. The hadronic matter does not seem to cause jet quenching in Pb+PbPb+Pb collisions at the CERN SPS. High-pTp_T two particle correlation in the azimuthal angle is proposed to further clarify this issue.Comment: 4 pages with 2 ps figures. Minors changes are made in the text with updated references.     Systematic Study of High p_T Hadron Spectra in pp, pA and AA Collisions from SPS to RHIC Energies

    High-pTp_T particle spectra in p+pp+p (pˉ+p\bar p + p), p+Ap+A and A+BA+B collisions are calculated within a QCD parton model in which intrinsic transverse momentum, its broadening due to initial multiple parton scattering, and jet quenching due to parton energy loss inside a dense medium are included phenomenologically. The intrinsic kTk_T and its broadening in p+Ap+A and A+BA+B collisions due to initial multiple parton scattering are found to be very important at low energies (s<50\sqrt{s}<50 GeV). Comparisons with S+SS+S, S+AuS+Au and Pb+PbPb+Pb data with different centrality cuts show that the differential cross sections of large transverse momentum pion production (pT>1p_T>1 GeV/cc) in A+BA+B collisions scale very well with the number of binary nucleon-nucleon collisions (modulo effects of multiple initial scattering). This indicates that semi-hard parton scattering is the dominant particle production mechanism underlying the hadron spectra at moderate pT>1p_T \stackrel{>}{\sim} 1 GeV/cc. However, there is no evidence of jet quenching or parton energy loss. Within the parton model, one can exclude an effective parton energy loss dEq/dx>0.01dE_q/dx>0.01 GeV/fm and a mean free path λq<7\lambda_q< 7 fm from the experimental data of A+BA+B collisions at the SPS energies. Predictions for high pTp_T particle spectra in p+Ap+A and A+AA+A collisions with and without jet quenching at the RHIC energy are also given.
