Legionellapreventie: betere naleving Waterleidingwet nodig
Ongeveer 20 procent van de zorginstellingen en zwembaden heeft in 2005 te maken gehad met een legionellabesmetting in de leidingwaterinstallatie. Dit gold ook voor 5 procent van de hotels en campings. Vooral in zorginstellingen kan Legionella in de waterleidingen gevaarlijk zijn, omdat de mensen hier vaak een zwakkere gezondheid hebben. Circa 90% van de instellingen leeft niet alle wettelijk voorgeschreven preventiemaatregelen volledig na. Bij hercontrole is echter een groot deel van de problemen opgelost. Dit toont de effectiviteit en het belang van de controles: eenmaal op de hoogte van de risico’s zijn de eigenaren meestal bereid de nodige aanpassingen te doen. Het aantal collectieve leidingwaterinstallaties met Legionella nam tussen 2000 en 2006 niet duidelijk af. Een betere naleving van de legionellapreventie regelgeving en installatievoorschriften leidt naar verwachting wél tot minder Legionella in leidingwaterinstallaties. Tussen 2000 en 2006 steeg het aantal gemelde patiënten met legionellose. Forces between clustered stereocilia minimize friction in the ear on a subnanometre scale
The detection of sound begins when energy derived from acoustic stimuli
deflects the hair bundles atop hair cells. As hair bundles move, the viscous
friction between stereocilia and the surrounding liquid poses a fundamental
challenge to the ear's high sensitivity and sharp frequency selectivity. Part
of the solution to this problem lies in the active process that uses energy for
frequency-selective sound amplification. Here we demonstrate that a
complementary part involves the fluid-structure interaction between the liquid
within the hair bundle and the stereocilia. Using force measurement on a
dynamically scaled model, finite-element analysis, analytical estimation of
hydrodynamic forces, stochastic simulation and high-resolution interferometric
measurement of hair bundles, we characterize the origin and magnitude of the
forces between individual stereocilia during small hair-bundle deflections. We
find that the close apposition of stereocilia effectively immobilizes the
liquid between them, which reduces the drag and suppresses the relative
squeezing but not the sliding mode of stereociliary motion. The obliquely
oriented tip links couple the mechanotransduction channels to this least
dissipative coherent mode, whereas the elastic horizontal top connectors
stabilize the structure, further reducing the drag. As measured from the
distortion products associated with channel gating at physiological stimulation
amplitudes of tens of nanometres, the balance of forces in a hair bundle
permits a relative mode of motion between adjacent stereocilia that encompasses
only a fraction of a nanometre. A combination of high-resolution experiments
and detailed numerical modelling of fluid-structure interactions reveals the
physical principles behind the basic structural features of hair bundles and
shows quantitatively how these organelles are adapted to the needs of sensitive

Time to tweak the TTO: results from a comparison of alternative specifications of the TTO
Abstract This article examines the effect that different
specifications of the time trade-off (TTO) valuation task
may have on values for EQ-5D-5L health states. The new
variants of the TTO, namely lead-time TTO and lag-time
TTO, along with the classic approach to TTO were compared
using two durations for the health states (15 and
20 years). The study tested whether these methods yield
comparable health-state values. TTO tasks were administered
online. It was found that lag-time TTO produced
lower values than lead-time TTO and that the difference
was larger in the longer time frame. Classic TTO values
most resembled those of the lag-time TTO in a 20-year
time frame in terms of mean absolute difference. The relative
importance of different domains of health was systematically
affected by the duration of the health state. In
the tasks with a 10-year health-state duration, anxiety/
depression had the largest negative impact on health-state
values; in the tasks with a 5-year duration, the pain/discomfort
domain had the largest negative impact
Salvage surgery for local failures after stereotactic ablative radiotherapy for early stage non-small cell lung cancer
markdownabstract__Introduction:__ The literature on surgical salvage, i.e. lung resections in patients who develop a local recurrence following stereotactic ablative radiotherapy (SABR), is limited. We describe our experience with salvage surgery in nine patients who developed a local recurrence following SABR for early stage non-small cell lung cancer (NSCLC).
__Methods:__ Patients who underwent surgical salvage for a local recurrence following SABR for NSCLC were identified from two Dutch institutional databases. Complications were scored using the Dindo-Clavien-classification.
__Results:__ Nine patients who underwent surgery for a local recurrence were identified. Median time to local recurrence was 22 months. Recurrences were diagnosed with CT- and/or 18FDG-PET-imaging, with four patients also having a pre-surgical pathological diagnosis. Extensive adhesions were observed during two resections, requiring conversion from a thoracoscopic procedure to thoracotomy during one of these procedures. Three patients experienced complications post-surgery; grade 2 (N = 2) and grade 3a (N = 1), respectively. All resection specimens showed viable tumor cells. Median length of hospital stay was 8 days (range 5-15 days) and 30-day mortality was 0 %. Lymph node dissection revealed mediastinal metastases in 3 patients, all of whom received adjuvant therapy.
__Conclusions:__ Our experience with nine surgical procedures for local recurrences post-SABR revealed two grade IIIa complications, and a 30-day mortality of 0 %, suggesting that salvage surgery can be safely performed after SABR
Introducing the composite time trade-off: a test of feasibility and face validity
__Abstract__
__Introduction__ This study was designed to test the feasibility and face validity of the composite time trade-off (composite TTO), a new approach to TTO allowing for a more consistent elicitation of negative health state values.
__Methods__ The new instrument combines a conventional TTO to elicit values for states regarded better than dead and a lead-time TTO for states worse than dead.
__Results__ A total of 121 participants completed the composite TTO for ten EQ-5D-5L health states. Mean values ranged from −0.104 for health state 53555 to 0.946 for 21111. The instructions were clear to 98 % of the respondents, and 95 % found the task easy to understand, indicating feasibility. Further, the average number of steps taken in the iteration procedure to achieve the point of indifference in the TTO and the average duration of each task were indicative of a deliberate cognitive process.
__Conclusion__ Face validity was confirmed by the high mean values for the mild health states (>0.90) and low mean values for the severe states (<0.42). In conclusion, this study demonstrates the feasibility and face validity of the composite TTO in a face-to-face standardized computer-assisted interview setting
The relationships between golf and health:A scoping review
OBJECTIVE: To assess the relationships between golf and health. DESIGN: Scoping review. DATA SOURCES: Published and unpublished reports of any age or language, identified by searching electronic databases, platforms, reference lists, websites and from consulting experts. REVIEW METHODS: A 3-step search strategy identified relevant published primary and secondary studies as well as grey literature. Identified studies were screened for final inclusion. Data were extracted using a standardised tool, to form (1) a descriptive analysis and (2) a thematic summary. RESULTS AND DISCUSSION: 4944 records were identified with an initial search. 301 studies met criteria for the scoping review. Golf can provide moderate intensity physical activity and is associated with physical health benefits that include improved cardiovascular, respiratory and metabolic profiles, and improved wellness. There is limited evidence related to golf and mental health. The incidence of golfing injury is moderate, with back injuries the most frequent. Accidental head injuries are rare, but can have serious consequences. CONCLUSIONS: Practitioners and policymakers can be encouraged to support more people to play golf, due to associated improved physical health and mental well-being, and a potential contribution to increased life expectancy. Injuries and illnesses associated with golf have been identified, and risk reduction strategies are warranted. Further research priorities include systematic reviews to further explore the cause and effect nature of the relationships described. Research characterising golf's contribution to muscular strengthening, balance and falls prevention as well as further assessing the associations and effects between golf and mental health are also indicated
Experience-based utility and own health state valuation for a health state classification system: why do it and how to do it
In the estimation of population value sets for health state classification systems such as the EQ-5D,
there is increasing interest in asking respondents to value their own health state, sometimes referred to as "experienced-based utility values" or more correctly ownrather than hypothetical health states. Own health
state values differ to hypothetical health state values, and this may be attributed to many reasons. This paper
critically examines: whose values matter; why there is a difference between own and hypothetical values; how
to measure own health state values; and why to use own health state values. Finally, the paper also examines
other ways that own health state values can be taken into account, such as including the use of informed general
population preferences that may better take into account experience-based values
Three-weekly doses of azithromycin for Indigenous infants hospitalized with bronchiolitis: a multicentre, randomized, placebo-controlled trial
Background: Bronchiolitis is a major health burden in infants globally, particularly among Indigenous populations. It is unknown if 3 weeks of azithromycin improve clinical outcomes beyond the hospitalization period. In an international, double-blind randomized controlled trial, we determined if 3 weeks of azithromycin improved clinical outcomes in Indigenous infants hospitalized with bronchiolitis.Methods: Infants aged ≤24 months were enrolled from three centers and randomized to receive three once-weekly doses of either azithromycin (30 mg/kg) or placebo. Nasopharyngeal swabs were collected at baseline and 48 h later. Primary endpoints were hospital length of stay (LOS) and duration of oxygen supplementation monitored every 12 h until judged ready for discharge. Secondary outcomes were: day-21 symptom/signs, respiratory rehospitalizations within 6 months post-discharge and impact upon nasopharyngeal bacteria and virus shedding at 48 h.Results: Two hundred nineteen infants were randomized (n = 106 azithromycin, n = 113 placebo). No significant between-group differences were found for LOS (median 54 h for each group, difference = 0 h, 95% CI: −6, 8; p = 0.8), time receiving oxygen (azithromycin = 40 h, placebo = 35 h, group difference = 5 h, 95% CI: −8, 11; p = 0.7), day-21 symptom/signs, or rehospitalization within 6 months (azithromycin n = 31, placebo n = 25 infants, p = 0.2). Azithromycin reduced nasopharyngeal bacterial carriage (between-group difference 0.4 bacteria/child, 95% CI: 0.2, 0.6; p < 0.001), but had no significant effect upon virus detection rates.Conclusion: Despite reducing nasopharyngeal bacterial carriage, three large once-weekly doses of azithromycin did not confer any benefit over placebo during the bronchiolitis illness or 6 months post hospitalization. Azithromycin should not be used routinely to treat infants hospitalized with bronchiolitis.Clinical trial registration: The trial was registered with the Australian and New Zealand Clinical Trials Register: Clinical trials number: ACTRN1261000036099

Biomineralisation by earthworms: an investigation into the stability and distribution of amorphous calcium carbonate
Background
Many biominerals form from amorphous calcium carbonate (ACC), but this phase is highly unstable when synthesised in its pure form inorganically. Several species of earthworm secrete calcium carbonate granules which contain highly stable ACC. We analysed the milky fluid from which granules form and solid granules for amino acid (by liquid chromatography) and functional group (by Fourier transform infrared (FTIR) spectroscopy) compositions. Granule elemental composition was determined using inductively coupled plasma-optical emission spectroscopy (ICP-OES) and electron microprobe analysis (EMPA). Mass of ACC present in solid granules was quantified using FTIR and compared to granule elemental and amino acid compositions. Bulk analysis of granules was of powdered bulk material. Spatially resolved analysis was of thin sections of granules using synchrotron-based μ-FTIR and EMPA electron microprobe analysis.
Results
The milky fluid from which granules form is amino acid-rich (≤ 136 ± 3 nmol mg−1 (n = 3; ± std dev) per individual amino acid); the CaCO3 phase present is ACC. Even four years after production, granules contain ACC. No correlation exists between mass of ACC present and granule elemental composition. Granule amino acid concentrations correlate well with ACC content (r ≥ 0.7, p ≤ 0.05) consistent with a role for amino acids (or the proteins they make up) in ACC stabilisation. Intra-granule variation in ACC (RSD = 16%) and amino acid concentration (RSD = 22–35%) was high for granules produced by the same earthworm. Maps of ACC distribution produced using synchrotron-based μ-FTIR mapping of granule thin sections and the relative intensity of the ν2: ν4 peak ratio, cluster analysis and component regression using ACC and calcite standards showed similar spatial distributions of likely ACC-rich and calcite-rich areas. We could not identify organic peaks in the μ-FTIR spectra and thus could not determine whether ACC-rich domains also had relatively high amino acid concentrations. No correlation exists between ACC distribution and elemental concentrations determined by EMPA.
Conclusions
ACC present in earthworm CaCO3 granules is highly stable. Our results suggest a role for amino acids (or proteins) in this stability. We see no evidence for stabilisation of ACC by incorporation of inorganic components
