    Influence of oxygen tension on myocardial performance. Evaluation by tissue Doppler imaging

    BACKGROUND: Low O(2 )tension dilates coronary arteries and high O(2 )tension is a coronary vasoconstrictor but reports on O(2)-dependent effects on ventricular performance diverge. Yet oxygen supplementation remains first line treatment in cardiovascular disease. We hypothesized that hypoxia improves and hyperoxia worsens myocardial performance. METHODS: Seven male volunteers (mean age 38 ± 3 years) were examined with echocardiography at respiratory equilibrium during: 1) normoxia (≈21% O(2), 79% N(2)), 2) while inhaling a hypoxic gas mixture (≈11% O(2), 89% N(2)), and 3) while inhaling 100% O(2). Tissue Doppler recordings were acquired in the apical 4-chamber, 2-chamber, and long-axis views. Strain rate and tissue tracking displacement analyses were carried out in each segment of the 16-segment left ventricular model and in the basal, middle and apical portions of the right ventricle. RESULTS: Heart rate increased with hypoxia (68 ± 4 bpm at normoxia vs. 79 ± 5 bpm, P < 0.001) and decreased with hyperoxia (59 ± 5 bpm, P < 0.001 vs. normoxia). Hypoxia increased strain rate in four left ventricular segments and global systolic contraction amplitude was increased (normoxia: 9.76 ± 0.41 vs hypoxia: 10.87 ± 0.42, P < 0.001). Tissue tracking displacement was reduced in the right ventricular segments and tricuspid regurgitation increased with hypoxia (7.5 ± 1.9 mmHg vs. 33.5 ± 1.8 mmHg, P < 0.001). The TEI index and E/E' did not change with hypoxia. Hyperoxia reduced strain rate in 10 left ventricular segments, global systolic contraction amplitude was decreased (8.83 ± 0.38, P < 0.001 vs. normoxia) while right ventricular function was unchanged. The spectral and tissue Doppler TEI indexes were significantly increased but E/E' did not change with hyperoxia. CONCLUSION: Hypoxia improves and hyperoxia worsens systolic myocardial performance in healthy male volunteers. Tissue Doppler measures of diastolic function are unaffected by hypoxia/hyperoxia which support that the changes in myocardial performance are secondary to changes in vascular tone. It remains to be settled whether oxygen therapy to patients with heart disease is a consistent rational treatment

    A prospective, double-blind, randomized, controlled clinical trial comparing standard wound care with adjunctive hyperbaric oxygen therapy (HBOT) to standard wound care only for the treatment of chronic, non-healing ulcers of the lower limb in patients with diabetes mellitus: a study protocol

    <p>Abstract</p> <p>Background</p> <p>It has been suggested that the use of adjunctive hyperbaric oxygen therapy improves the healing of diabetic foot ulcers, and decreases the risk of lower extremity amputations. A limited number of studies have used a double blind approach to evaluate the efficacy of hyperbaric oxygen therapy in the treatment of diabetic ulcers. The primary aim of this study is to assess the efficacy of hyperbaric oxygen therapy plus standard wound care compared with standard wound care alone in preventing the need for major amputation in patients with diabetes mellitus and chronic ulcers of the lower limb.</p> <p>Methods/Design</p> <p>One hundred and eighteen (59 patients per arm) patients with non-healing diabetic ulcers of the lower limb, referred to the Judy Dan Research and Treatment Centre are being recruited if they are at least 18 years of age, have either Type 1 or 2 diabetes with a Wagner grading of foot lesions 2, 3 or 4 on lower limb not healing for at least 4 weeks. Patients receive hyperbaric oxygen therapy every day for 6 weeks during the treatment phase and are provided ongoing wound care and weekly assessments. Patients are required to return to the study centre every week for an additional 6 weeks of follow-up for wound evaluation and management. The primary outcome is freedom from having, or meeting the criteria for, a major amputation (below knee amputation, or metatarsal level) up to 12 weeks after randomization. The decision to amputate is made by a vascular surgeon. Other outcomes include wound healing, effectiveness, safety, healthcare resource utilization, quality of life, and cost-effectiveness. The study will run for a total of about 3 years.</p> <p>Discussion</p> <p>The results of this study will provide detailed information on the efficacy of hyperbaric oxygen therapy for the treatment of non-healing ulcers of the lower limb. This will be the first double-blind randomized controlled trial for this health technology which evaluates the efficacy of hyperbaric oxygen therapy in prevention of amputations in diabetic patients.</p> <p>Trial registration</p> <p>ClinicalTrials.gov Identifier: <a href="http://www.clinicaltrials.gov/ct2/show/NCT00621608">NCT00621608</a></p

    Resource Selection and Its Implications for Wide-Ranging Mammals of the Brazilian Cerrado

    Conserving animals beyond protected areas is critical because even the largest reserves may be too small to maintain viable populations for many wide-ranging species. Identification of landscape features that will promote persistence of a diverse array of species is a high priority, particularly, for protected areas that reside in regions of otherwise extensive habitat loss. This is the case for Emas National Park, a small but important protected area located in the Brazilian Cerrado, the world's most biologically diverse savanna. Emas Park is a large-mammal global conservation priority area but is too small to protect wide-ranging mammals for the long-term and conserving these populations will depend on the landscape surrounding the park. We employed novel, noninvasive methods to determine the relative importance of resources found within the park, as well as identify landscape features that promote persistence of wide-ranging mammals outside reserve borders. We used scat detection dogs to survey for five large mammals of conservation concern: giant armadillo (Priodontes maximus), giant anteater (Myrmecophaga tridactyla), maned wolf (Chrysocyon brachyurus), jaguar (Panthera onca), and puma (Puma concolor). We estimated resource selection probability functions for each species from 1,572 scat locations and 434 giant armadillo burrow locations. Results indicate that giant armadillos and jaguars are highly selective of natural habitats, which makes both species sensitive to landscape change from agricultural development. Due to the high amount of such development outside of the Emas Park boundary, the park provides rare resource conditions that are particularly important for these two species. We also reveal that both woodland and forest vegetation remnants enable use of the agricultural landscape as a whole for maned wolves, pumas, and giant anteaters. We identify those features and their landscape compositions that should be prioritized for conservation, arguing that a multi-faceted approach is required to protect these species

    Search for Neutral Supersymmetric Higgs Bosons in ppˉp\bar{p} Collisions at s=1.8\sqrt{s}=1.8 TeV

    We present the results of a search for neutral Higgs bosons produced in association with bb quarks in ppˉbbˉϕbbˉbbˉp\bar{p}\to b\bar{b} \phi\to b\bar{b}b\bar{b} final states with 91±791 \pm 7 pb1^{-1} of ppˉp\bar{p} collisions at s=1.8\sqrt{s}=1.8 TeV recorded by the Collider Detector at Fermilab. We find no evidence of such a signal and the data is interpreted in the context of the neutral Higgs sector of the Minimal Supersymmetric extension of the Standard Model. With basic parameter choices for the supersymmetric scale and the stop quark mixing, we derive 95% C.L. lower mass limits for neutral Higgs bosons for \tb values in excess of 35.Comment: 2 tex files 3 figure

    Measurement of dsigma/DM Forward-Backward Charge Asymmetry for High Mass Drell-Yan e+e- Pairs from p-pbar Collisions at sqrt(s)=1.8 TeV

    We report on a measurement of the mass dependence of the forward-backward charge asymmetry, A_FB, and production cross section dsigma/dM for e+e- pairs with mass M_ee>40 GeV/c2. The data sample consists of 108 pb-1 of p-pbar collisions at sqrt(s)=1.8 TeV taken by the Collider Detector at Fermilab during 1992-1995. The measured asymmetry and dsigma/dM are compared with the predictions of the Standard Model and a model with an extra Z' gauge boson.Comment: 7 pages submitted to Phys. Rev. Lett. 1 figure, 2 Table

    Diffractive Dijet Production at s = 630 and 1800 GeV at the Fermilab Tevatron

    We report a measurement of the diffractive structure function F-jj(D) of the antiproton obtained from a study of dijet events produced in association with a leading antiproton in (p) over barp collisions at roots = 630 GeV at the Fermilab Tevatron. The ratio of F-jj(D) at roots = 630 GeV to F-jj(D) obtained from a similar measurement at roots = 1800 GeV is compared with expectations from QCD factorization and other theoretical predictions. We also report a measurement of the xi (x-Pomeron) and beta (x of parton in Pomeron) dependence of F-jj(D) at roots = 1800 GeV . In the region 0.035 < &xi; < 0.095 , \t\ < 1 GeV2 , and &beta; < 0.5 , F-jj(D)(beta, xi) is found to be of the form beta(-1.0+/-0.1) xi(-0.9+/-0. 1) , which obeys beta-xi factorization

    Lawson criterion for ignition exceeded in an inertial fusion experiment

    For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion

    Measurement of the average time-integrated mixing probability of b-flavored hadrons produced at the Fermilab Tevatron

    We have measured the number of like-sign (LS) and opposite-sign (OS) lepton pairs arising from double semileptonic decays of b and (b) over bar hadrons, pair produced at the Fermilab Tevatron collider. The data samples were collected with the Collider Detector at Fermilab during the 1992-1995 collider run by triggering on the existence of mumu or emu candidates in an event. The observed ratio of LS to OS dileptons leads to a measurement of the average time-integrated mixing probability of all produced b-flavored hadrons which decay weakly, (χ) over bar =0.152+/-0.007 (stat)+/-0.011 (syst), that is significantly larger than the world average (χ) over bar =0.118+/-0.005

    Measurement of the Ratio of b Quark Production Cross Sections in Antiproton-Proton Collisions at 630 GeV and 1800 GeV

    We report a measurement of the ratio of the bottom quark production cross section in antiproton-proton collisions at 630 GeV to 1800 GeV using bottom quarks with transverse momenta greater than 10.75 GeV identified through their semileptonic decays and long lifetimes. The measured ratio sigma(630)/sigma(1800) = 0.171 +/- .024 +/- .012 is in good agreement with next-to-leading order (NLO) quantum chromodynamics (QCD)

    Search for lepton flavor violating decays of a heavy neutral particle in p(p)over-bar collisions at root s=1.8 TeV

    We report on a search for a high mass, narrow width particle that decays directly to emu, etau, or mutau. We use approximately 110 pb(-1) of data collected with the Collider Detector at Fermilab from 1992 to 1995. No evidence of lepton flavor violating decays is found. Limits are set on the production and decay of sneutrinos with R-parity violating interactions
