Sublingual allergen immunotherapy with a liquid birch pollen product in patients with seasonal allergic rhinoconjunctivitis with or without asthma
Background: Sublingual allergen immunotherapy (SLIT) has been demonstrated to be both clinically efficacious and safe. However, in line with the current regulatory guidance from the European Medicines Agency, allergen immunotherapy (AIT) products must demonstrate their efficacy and safety in pivotal phase III trials for registration.
Objective: We sought to investigate the efficacy and safety of sublingual high-dose liquid birch pollen extract (40,000 allergy units native [AUN]/mL) in adults with birch pollen allergy.
Methods: A randomized, double-blind, placebo-controlled, parallel-group multicenter trial was conducted in 406 adult patients with moderate-to-severe birch pollen-induced allergic rhinoconjunctivitis with or without mild-to-moderate controlled asthma. Treatment was started 3 to 6 months before the birch pollen season and continued during the season in 40 clinical study centers in 5 European countries. For primary end point assessment, the recommended combined symptom and medication score of the European Academy of Allergy and Clinical Immunology was used. Secondary end points included quality-of-life assessments, immunologic parameters, and safety.
Results: Primary efficacy results demonstrated a significant (P < .0001) and clinically relevant (32%) reduction in the combined symptom and medication score compared with placebo after 3 to 6 months of SLIT. Significantly better rhinoconjunctivitis quality-of-life scores (P < .0001) and the patient's own overall assessment of his or her health status, including the visual analog scale score (Euro Quality of Life Visual Analogue Scale; P = .0025), were also demonstrated. In total, a good safety profile of SLIT was observed.
Conclusion: This study confirmed both the clinical efficacy and safety of a sublingual liquid birch pollen extract in adults with birch pollen allergy in a pivotal phase III trial (EudraCT: 2013-005550-30; ClinicalTrials. gov: NCT02231307)

Overcoming High Energy Backgrounds at Pulsed Spallation Sources
Instrument backgrounds at neutron scattering facilities directly affect the
quality and the efficiency of the scientific measurements that users perform.
Part of the background at pulsed spallation neutron sources is caused by, and
time-correlated with, the emission of high energy particles when the proton
beam strikes the spallation target. This prompt pulse ultimately produces a
signal, which can be highly problematic for a subset of instruments and
measurements due to the time-correlated properties, and different to that from
reactor sources. Measurements of this background have been made at both SNS
(ORNL, Oak Ridge, TN, USA) and SINQ (PSI, Villigen, Switzerland). The
background levels were generally found to be low compared to natural
background. However, very low intensities of high-energy particles have been
found to be detrimental to instrument performance in some conditions. Given
that instrument performance is typically characterised by S/N, improvements in
backgrounds can both improve instrument performance whilst at the same time
delivering significant cost savings. A systematic holistic approach is
suggested in this contribution to increase the effectiveness of this.

A systematic review of the epidemiology of hepatitis E virus in Africa
BACKGROUND: Hepatitis E Virus (HEV) infection is a newly recognized serious threat to global public health and Africa is suspected to be among the most severely affected regions in the world. Understanding HEV epidemiology in Africa will expedite the implementation of evidence-based control policies aimed at preventing the spread of HEV including policies for the use of available resources such as HEV vaccines. METHODS: Here we present a comprehensive review of HEV epidemiology in Africa based on published data. We searched for articles on HEV epidemiology in Africa from online databases such as PubMed, Scopus, and ISI Web of Science and critically reviewed appropriate publications to extract consistent findings, identify knowledge gaps, and suggest future studies. RESULTS: Taking a particularly high toll in pregnant women and their fetuses, HEV has infected human populations in 28 of 56 African countries. Since 1979, 17 HEV outbreaks have been reported about once every other year from Africa causing a reported 35,300 cases with 650 deaths. CONCLUSIONS: In Africa, HEV infection is not new, is widespread, and the number of reported outbreaks are likely a significant underestimate. The authors suggest that this is a continent-wide public health problem that deserves the attention of local, regional and international agencies to implement control policies that can save numerous lives, especially those of pregnant women and their fetuses
Synthesis, Characterization, and Optical Properties of a Cyano-Functionalized 4,5,9,10-tetraaryl-l,6-dioxapyrene
5,10-Di(4-cyanophenyl)-4,9-di(4-methylphenyl)-1,6-dioxapyrene ( CN-diox), a symmetrically substituted 4,5,9,10-tetraaryldioxapyrene, was synthesized in seven steps from 1,4-dihydroxynaphthalene. The synthetic methodology incorporated a base-catalyzed ring-closure process followed by dehydration to introduce the first tetraaryl- 1,6-dioxapyrene. Crystal structure and electrochemical analysis were performed to directly compare the properties of CN-diox to previously reported dioxapyrene derivatives, specifically 1,6-dioxapyrene (Diox) and 4,9-diethyl-2,7-dimethyl- 1,6-dioxapyrene (Alkyl-diox). Optical spectroscopy studies were performed to evaluate the potential of the 1,6-dioxapyrenes as fluorescent probes. CN-diox revealed a broad absorption centered near 450 nm (epsilon = 31,900/M/cm) in THF with a corresponding fluorescence at 619 nm (Phi(sub f) = 0.011). This was in sharp contrast to both Diox and Alkyl-diox which displayed broad absorption bands near 400 nm (epsilon approx. 5,000-10,000/M/cm) in THF with corresponding fluorescence near 500 nm (Phi(sub f) = 0.059 and 0.082 for Diox and Alkyl-diox, respectively). The luminescence of CN-diox was found to be solvatochromic (lambda(sub max) = 619 nm-644 nm) with single exponential lifetimes of less than 1.3 ns. Neither Diox nor Alkyl-diox showed solvatochromic properties
