    Les cabinets ministériels fédéraux : effectif, recrutement et profil des « chefs »

    Cet article présente d’abord un portrait statistique des membres des cabinets ministériels fédéraux. Il s’intéresse ensuite au mode de recrutement des premiers responsables des cabinets ministériels, les « chefs de cabinet ». L’article trace également le profil socio-professionnel de ces chefs. L’examen des caractéristiques sociologiques et professionnelles montre que les chefs sont issus de milieux diversifiés, qu’ils sont assez bien formés et plutôt jeunes. De plus, cet examen contredit la prévision selon laquelle les chefs proviennent des milieux privés d’affaires plus sympathiques aux idées néo-conservatrices. De fait, le recrutement des chefs se fait dans plusieurs milieux dont au premier chef la filière partisane. De ce point de vue, il n’y a pas un moule et le groupe des chefs de cabinet ne forme ni une dynastie, ni une chapelle ou un club fermé qui ne serait accessible qu’à un petit nombre.This article first presents a statistical portrait of members of Federal minister's offices. The article also examines the recruitment of the first responsible of a minister's office, the chief of staff and also shows their socio-professional profile. It is shown that the chiefs of staff come from diversified social backgrounds, are well educated and rather young. More, this exam contradicts our hypothesis wanting that federal chiefs of staff are rather issued from groups sympathizing more with neo-conservative ideas. In fact, chiefs of staff come from many groups and in first, from the partisan network. Nevertheless, there is no mold and the groups of chiefs of staff do not form a dynasty nor a chapel or a closed club accessible for the less number

    La politique d’internationalisation de Montréal : une politique en devenir

    Cet article porte sur la question de l’« internationalité » des grandes villes et plus spécifiquement de l’« internationalité » de Montréal. Partant d’une démarche qui repose à la fois sur l’étude des déclarations publiques et des stratégies mises de l’avant par les acteurs institutionnels, l’article veut faire état des réalisations, des contraintes et des perspectives d’avenir que suscite cette question. Montréal compte plusieurs avantages favorisant son internationalisation, bien que plusieurs facteurs nuisent à ce développement, notamment cette difficulté qu’ont les acteurs à se concerter sur le devenir de cette métropole. Les données recueillies lors de cette étude montrent toutefois qu’une dynamique nouvelle émerge entre les acteurs à l’effet de remplacer l’attentisme du passé face aux gouvernements supérieurs par une prise en charge qui devrait permettre de développer une vision à long terme. En revanche, nous ne pouvons pas encore parler d’une véritable politique d’internationalisation de la ville de Montréal mais d’une politique d’internationalisation encore en devenir

    Bimodal release ondansetron for acute gastroenteritis among adolescents and adults: A randomized clinical trial

    Importance: Vomiting resulting from acute gastroenteritis is commonly treated with intravenous antiemetics in acute care settings. If oral treatment were beneficial, patients might not need intravenous administered hydration or medication. Furthermore, a long-acting treatment could provide sustained relief from nausea and vomiting. Objective: To determine whether an experimental long-acting bimodal release ondansetron tablet decreases gastroenteritis-related vomiting and eliminates the need for intravenous therapy for 24 hours after administration. Design, Setting, and Participants: This placebo-controlled, double-blind, randomized clinical trial included patients from 19 emergency departments and 2 urgent care centers in the United States from December 8, 2014, to February 17, 2017. Patients 12 years and older with at least 2 vomiting episodes from presumed gastroenteritis in the previous 4 hours and symptoms with less than 36 hours\u27 duration were randomized using a 3:2 active to placebo ratio. Analyses were performed on an intent-to-treat basis and conducted from June 1, 2017, to November 1, 2017. Intervention: Bimodal release ondansetron tablet containing 6 mg of immediate release ondansetron and 18 mg of a 24-hour release matrix for a total of 24 mg of ondansetron. Main Outcomes and Measures: Treatment success was defined as no further vomiting, no need for rescue medication, and no intravenous hydration for 24 hours after bimodal release ondansetron administration. Results: Analysis included 321 patients (mean [SD] age, 29.0 [11.1] years; 195 [60.7%] women), with 192 patients in the bimodal release ondansetron group and 129 patients in the placebo group. Treatment successes were observed in 126 patients in the bimodal release ondansetron group (65.6%) compared with 70 patients in the placebo group (54.3%), with an 11.4% (95% CI, 0.3%-22.4%) absolute probability difference. The proportion of treatment success was 21% higher among patients who received bimodal release ondansetron compared with those who received a placebo (relative risk, 1.21; 95% CI, 1.00-1.46; P = .04). In an analysis including only patients with a discharge diagnosis of acute gastroenteritis and no major protocol violations, there were 123 treatment successes (69.5%) in the bimodal release ondansetron group compared with 67 treatment successes (54.9%) in the placebo group (relative risk, 1.27; 95% CI, 1.05-1.53; P = .01). Adverse effects were infrequent and similar to the known safety profile of ondansetron. Conclusions and Relevance: This randomized clinical trial found that a long-acting bimodal release oral ondansetron tablet was an effective antiemetic among adolescents and adults with moderate to severe vomiting from acute gastroenteritis. The drug benefits extended to 24 hours after administration. Bimodal release ondansetron may decrease the need for intravenous access and emergency department care to manage acute gastroenteritis.     Towards a more prosperous Springfield, MA: what jobs exist for people without a college education?

    This paper analyzes projections of Massachusetts employment opportunities by occupation to address concerns about a shortage of jobs for those who lack a college education. While occupations requiring a college degree will grow more rapidly over the period 2006-2016 than occupations that do not require college, replacement needs will ensure large numbers of job openings that do not require college. Wage levels in jobs that do not require college are generally low, however. The exceptions usually require meaningful training of another sort, such as long-term on-the-job training or courses in postsecondary schools or community college. Additionally, some individuals who demonstrate the necessary qualities achieve higher wages through promotion. The distribution of occupations in the Springfield metropolitan area is sufficiently similar to that in Massachusetts that inferences from the Massachusetts projections should be relevant to Springfield.Cities and towns - Massachusetts ; Labor market - Massachusetts

    Greater Springfield employment challenges: findings of employer survey and interviews

    This paper presents the findings from in-depth interviews and a survey of employers in the Greater Springfield area regarding (a) employment opportunities for entry-level workers with limited skills and (b) barriers that residents of Springfield's low-income neighborhoods face in accessing these jobs. The survey and interviews indicate that entry-level jobs that do not require college are available in the Springfield area. However, even entry-level jobs require the ability to perform a variety of tasks, and many applicants lack the skills needed to perform these jobs or have work readiness problems. Lack of prior experience and the absence of referral networks limit access to these jobs for residents of Springfield's low-income neighborhoods.Cities and towns - Massachusetts ; Labor market - Massachusetts

    Reinvigorating Springfield's economy: lessons from resurgent cities

    As part of the Federal Reserve Bank of Boston's commitment to supporting efforts to revitalize the economy of Springfield, Massachusetts, this paper analyzes the economic development approaches of other mid-sized manufacturing-oriented cities during the past half century. From among a comparison group of 25 municipalities that were similar to Springfield in 1960, the study identifies 10 "resurgent cities" that have made substantial progress in improving living standards for their residents, and that are recognized as vital communities in a broader sense by experts on urban economic development and policy. These case studies suggest that industry mix, demographic composition, and geographic position are not the key factors distinguishing the resurgent cities from Springfield. Instead, the most important lessons from the resurgent cities concern leadership and collaboration. Initial leadership in these cities came from a variety of key institutions and individuals. In some cases, the turnaround started with efforts on the part of the public sector, while in other cases nongovernmental institutions or private developers were at the forefront. Regardless of who initiated the turnaround, economic redevelopment efforts spanned decades and involved collaborations among numerous organizations and sectors.Cities and towns ; Cities and towns - Massachusetts ; Economic policy - Massachusetts
