Family support and cardiac rehabilitation: A comparative study of the experiences of South Asian and White-European patients and their carer's living in the United Kingdom
Background: Effective lifestyle modification facilitated by cardiac rehabilitation is known to reduce the occurrence of adverse coronary events and mortality. South Asians have poorer outcomes after a myocardial infarction than the general UK population, but little is known about their experiences of family support, cardiac rehabilitation and lifestyle change. Aims: To explore the nature of family support available to a sample of South Asian and White-European cardiac patients and to highlight similarities and differences between these groups with regard to cardiac rehabilitation and lifestyle modification. Methods: Using a qualitative approach, semi-structured interviews (in 1 of 6 languages) were conducted by researchers with; 45 South Asian patients and 37 carers and 20 White-European patients and 17 carers. Interviews were conducted in a home setting, up to eighteen months after discharge from hospital following myocardial infarction, coronary artery bypass surgery or unstable angina. Results: The main themes that emerged related to the provision of advice and information, family support and burden, dietary change and exercise regimes. Conclusions: Several cultural and ethnic differences were identified between patients and their families alongside similarities, irrespective of ethnicity. These may represent generic characteristics of recovery after a cardiac event. Health professionals should develop a cultural repertoire to engage with diversity and difference. Not every difficulty a person encounters as they try to access appropriate service delivery can be attributed to ethnic background. By improving services generally, support for South Asian populations can be improved. The challenge is to know when ethnicity makes a difference and mediates a person's relationship with service support and when it does not. (C) 2007 European Society of Cardiology. Interactions of Cathinone NPS with Human Transporters and Receptors in Transfected Cells
Pharmacological assays carried out in transfected cells have been very useful for describing the mechanism of action of cathinone new psychoactive substances (NPS). These in vitro characterizations provide fast and reliable information on psychoactive substances soon after they emerge for recreational use. Well-investigated comparator compounds, such as methamphetamine, 3,4-methylenedioxymethamphetamine, cocaine, and lysergic acid diethylamide, should always be included in the characterization to enhance the translation of the in vitro data into clinically useful information. We classified cathinone NPS according to their pharmacology at monoamine transporters and receptors. Cathinone NPS are monoamine uptake inhibitors and most induce transporter-mediated monoamine efflux with weak to no activity at pre- or postsynaptic receptors. Cathinones with a nitrogen-containing pyrrolidine ring emerged as NPS that are extremely potent transporter inhibitors but not monoamine releasers. Cathinones exhibit clinically relevant differences in relative potencies at serotonin vs. dopamine transporters. Additionally, cathinone NPS have more dopaminergic vs. serotonergic properties compared with their non-β-keto amphetamine analogs, suggesting more stimulant and reinforcing properties. In conclusion, in vitro pharmacological assays in heterologous expression systems help to predict the psychoactive and toxicological effects of NPS
Phenotypic diversity within Ugandan yam (Dioscorea species) germplasm collection
Open Access JournalA proper understanding of the diversity of the available germplasm is an initial step for the genetic improvement of a crop through breeding. However, there is limited information on the diversity of Uganda’s yam germplasm. The study sought to characterize the diversity of yam germplasm utilized for decades in Uganda together with germplasm recently introduced from West Africa using phenotypic traits. A germplasm collection of 291 genotypes was characterized using 28 phenotypic traits. Data were subjected to multivariate analysis using principal component analysis and cluster analysis. The traits assessed were informative and discriminating, with 62% of the total variation explained among the first six principal components. Results showed that the important phenotypic traits contributing to most of the variability among the genotypes were leaves, flowering, and tuber traits. Ugandan genotypes were identified with amorphous tuber shapes compared to West African genotypes. The study has shown that there is ample phenotypic variability within the major yam genotypes in Uganda yam germplasm that can be used for genetic improvement. More in-depth molecular and biochemical studies to further understand the diversity are recommended. Refractory depression – Cost-Effectiveness of Radically Open Dialectical Behaviour Therapy (RefraMED):findings of trial-based economic evaluation
BackgroundRefractory depression is a major contributor to the economic burden of depression. Radically Open Dialectical Behaviour Therapy (RO DBT) is an unevaluated new treatment targeting over-controlled personality, common in refractory depression, but it is not yet known whether the additional expense of RO DBT is good value for money. AimTo estimate the cost-effectiveness of RO DBT plus treatment as usual (TAU) compared with TAU alone in patients with refractory depression. MethodsWe undertook a cost-effectiveness analysis alongside a randomised trial evaluating RO DBT plus TAU versus TAU alone for refractory depression in three UK secondary care centres. Our economic evaluation, 12 months after randomisation, adopted the perspective of the UK National Health Service (NHS) and personal social services. It evaluated cost-effectiveness by comparing the net cost of RO DBT with the net gain in quality-adjusted life years, estimated using the EQ-5D-3L measure of health-related quality of life. ResultsThe additional cost of RO DBT plus TAU compared to TAU alone was £7,048, which was associated with a difference of 0.032 QALYs, yielding an ICER of £220,250 per QALY. This ICER was well above the NICE upper threshold of £30,000 per QALY. A cost-effectiveness acceptability curve indicated that RO DBT had a zero probability of being cost-effective compared to TAU at the NICE £30,000 threshold.ConclusionIn its current resource-intensive form, RO DBT was not a cost-effective use of resources in the UK NHS.<br/

ChemProt: a disease chemical biology database
Systems pharmacology is an emergent area that studies drug action across multiple scales of complexity, from molecular and cellular to tissue and organism levels. There is a critical need to develop network-based approaches to integrate the growing body of chemical biology knowledge with network biology. Here, we report ChemProt, a disease chemical biology database, which is based on a compilation of multiple chemical–protein annotation resources, as well as disease-associated protein–protein interactions (PPIs). We assembled more than 700 000 unique chemicals with biological annotation for 30 578 proteins. We gathered over 2-million chemical–protein interactions, which were integrated in a quality scored human PPI network of 428 429 interactions. The PPI network layer allows for studying disease and tissue specificity through each protein complex. ChemProt can assist in the in silico evaluation of environmental chemicals, natural products and approved drugs, as well as the selection of new compounds based on their activity profile against most known biological targets, including those related to adverse drug events. Results from the disease chemical biology database associate citalopram, an antidepressant, with osteogenesis imperfect and leukemia and bisphenol A, an endocrine disruptor, with certain types of cancer, respectively. Extragalactic Star Cluster Science with the Nancy Grace Roman Space Telescope's High Latitude Wide Area Survey and the Vera C. Rubin Observatory
The Nancy Grace Roman Telescope's High Latitude Wide Area Survey will have a
number of synergies with the Vera Rubin Observatory's Legacy Survey of Space
and Time (LSST), particularly for extragalactic star clusters. Understanding
the nature of star clusters and star cluster systems are key topics in many
areas of astronomy, chief among them stellar evolution, high energy
astrophysics, galaxy assembly/dark matter, the extragalactic distance scale,
and cosmology. One of the challenges will be disentangling the age/metallicity
degeneracy because young (Myr) metal-rich clusters have similar SEDs to
old (Gyr) metal-poor clusters. Rubin will provide homogeneous,
photometric coverage, and measurements in the red Roman filters will help break
the age-metallicity and age-extinction degeneracies, providing the first
globular cluster samples that cover wide areas while essentially free of
contamination from Milky Way stars. Roman's excellent spatial resolution will
also allow measurements of cluster sizes. We advocate for observations of a
large sample of galaxies with a range of properties and morphologies in the
Rubin/LSST footprint matching the depth of the LSST Wide-Fast-Deep field
band limit (26.3 mag), and recommend adding the F213 filter to the survey.Comment: white paper submitted for Roman CCS inpu
