Bullous lesions as a manifestation of systemic lupus erythematosus in two Mexican teenagers
Rarely, systemic lupus erythematosus (SLE) presents with bullous lesions due to severe edema and hydropic degeneration of the basal layer, or as a subepidermal blistering disease. Here, we describe two Mexican teenagers, one with SLE with blisters and another with bullous SLE. Substance abuse and intimate partner violence: treatment considerations
Given the increased use of marital- and family-based treatments as part of treatment for alcoholism and other drug disorders, providers are increasingly faced with the challenge of addressing intimate partner violence among their patients and their intimate partners. Yet, effective options for clinicians who confront this issue are extremely limited. While the typical response of providers is to refer these cases to some form of batterers' treatment, three fundamental concerns make this strategy problematic: (1) most of the agencies that provide batterers' treatment only accept individuals who are legally mandated to complete their programs; (2) among programs that do accept nonmandated patients, most substance-abusing patients do not accept such referrals or drop out early in the treatment process; and (3) available evidence suggests these programs may not be effective in reducing intimate partner violence. Given these very significant concerns with the current referral approach, coupled with the high incidence of IPV among individuals entering substance abuse treatment, providers need to develop strategies for addressing IPV that can be incorporated and integrated into their base intervention packages
Lúpus eritematoso sistêmico bolhoso: diagnóstico diferencial com dermatite herpetiforme
O lúpus eritematoso sistêmico bolhoso é um subtipo raro do lúpus eritematoso sistêmico, que ocorre ainda de forma mais incomum nos pacientes pediátricos. Relatamos o caso de uma adolescente de 12 anos, apresentando lesões vésico-bolhosas em face, pescoço, tronco, mucosas oral e genital, anemia, leucocitúria estéril, FAN: 1/1280 padrão nuclear pontilhado grosso, Anti-Sm e Anti-RNP positivos. O estudo anatomopatológico sugere dermatite herpetiforme e a imunofluorescência direta revela IgG, IgA e fibrina ao longo da zona de membrana basal. Apresentamos um caso típico de lúpus eritematoso sistêmico bolhoso e enfatizamos a importância do diagnóstico diferencial com a dermatite herpetiforme<br>Bullous systemic lupus erythematosus is a rare subset of systemic lupus erythematosus that is even rarer in pediatric patients. We report a case of a 12-year-old girl who presented with a vesiculobullous eruption on her face, neck, trunk and genital and oral mucosa, as well as anemia, sterile pyuria, ANA (1:1280, speckled pattern) and positive anti-Sm and anti-RNP. Pathological examination suggested dermatitis herpetiformis, and direct immunofluorescence revealed IgG, IgA and fibrin in the epithelial basement membrane zone. Ice nucleation and overseeding of ice in volcanic clouds
Water is the dominant component of volcanic gas emissions, and water phase transformations, including the formation of ice, can be significant in the dynamics of volcanic clouds. The effectiveness of volcanic ash particles as ice-forming nuclei (IN) is poorly understood and the sparse data that exist for volcanic ash IN have been interpreted in the context of meteorological, rather than volcanic clouds. In this study, single-particle freezing experiments were carried out to investigate the effect of ash particle composition and surface area on water drop freezing temperature. Measured freezing temperatures show only weak correlations with ash IN composition and surface area. Our measurements, together with a review of previous volcanic ash IN measurements, suggest that fine-ash particles (equivalent diameters between approximately 1 and 1000 mm) from the majority of volcanoes will exhibit an onset of freezing between similar to 250-260 K. In the context of explosive eruptions where super-micron particles are plentiful, this result implies that volcanic clouds are IN-rich relative to meteorological clouds, which typically are IN-limited, and therefore should exhibit distinct microphysics. We can expect that such "overseeded" volcanic clouds will exhibit enhanced ice crystal concentrations and smaller average ice crystal size, relative to dynamically similar meteorological clouds, and that glaciation will tend to occur over a relatively narrow altitude range
