Drogues, sexe et risques dans la communauté gaie montréalaise : 1997-2003
Cet article décrit l’évolution de la consommation de drogues entre 1997 et 2003 chez des hommes gais et bisexuels séronégatifs de la grande région montréalaise, selon l’âge et selon les comportements sexuels à risque pour le VIH. Les données proviennent d’Oméga, une étude longitudinale sur l’incidence et les déterminants psychosociaux de l’infection au VIH chez ces hommes. Les participants ont rempli un questionnaire tous les six mois, et les données sont celles de leur première visite de suivi. Des analyses de tendance par période de calendrier ont été réalisées et un modèle de régression logistique utilisant une estimation par équations généralisées a été généré pour chaque type de drogues. Les résultats révèlent une augmentation de la consommation de cocaïne, de l’ecstasy, des hallucinogènes, du speed et du GHB entre 1997 et 2003. En revanche, la consommation de marijuana, de poppers et d’héroïne ne semble pas avoir changé de façon significative. Les moins de 30 ans et ceux qui ont eu des relations anales à risque semblent avoir été plus enclins à consommer certaines drogues pendant cette même période. Indices de transformations possibles dans les modes de vie de ces hommes, ces résultats soulèvent de nouveaux défis pour la prévention du VIH dans la communauté gaie.This article describes the evolution of drug use between 1997 and 2003 in HIV-negative men who have sex with men living in the Montreal region, according to age and HIV risk-taking behaviours. Data come from Omega, a longitudinal study on the incidence and on the psychosocial determinants of HIV infection among these men. Participants completed a questionnaire every six months and data are from the first follow-up visit. Trends analyses by calendar period were done and a logistic regression model using generalised equations for parameter estimation was generated for each type of drug. Results reveal an increase in cocaine, ecstasy, hallucinogen, speed and GHB use between 1997 and 2003. On the other hand, marijuana, poppers and heroin use do not seem to have change significantly. Men who are younger than 30 years old and who had risky anal intercourse seem to be more likely to have used specific drugs in the same period. This indicates possible transformations in the way of life of these men and these results suggest new challenges for HIV prevention in the gay community.Este artículo describe la evolución del consumo de drogas entre 1997 y 2003 entre los hombres gay y bisexuales seronegativos de la gran región montrealesa, según la edad y los comportamientos sexuales de riesgo con respecto al VIH. Los participantes respondieron un cuestionario cada seis meses y los datos son los de su primera visita de seguimiento. Se realizaron análisis de tendencia por período de calendario y se generó un modelo de regresión logística, utilizando una estimación por ecuaciones generalizadas para cada tipo de droga. Los resultados revelan un aumento del consumo de cocaína, éxtasis, alucinógenos, speed y GHB entre 1997 y 2003. Por el contrario, el consumo de marihuana, poppers y heroína no parece haber cambiado de manera significativa. Los menores de 30 años y quienes tienen relaciones sexuales anales a riesgo parecen haber tenido una mayor tendencia a consumir ciertas drogas durante este mismo período. Disparities and risks of sexually transmissible infections among men who have sex with men in China: a meta-analysis and data synthesis.
BACKGROUND: Sexually transmitted infections (STIs), including Hepatitis B and C virus, are emerging public health risks in China, especially among men who have sex with men (MSM). This study aims to assess the magnitude and risks of STIs among Chinese MSM. METHODS: Chinese and English peer-reviewed articles were searched in five electronic databases from January 2000 to February 2013. Pooled prevalence estimates for each STI infection were calculated using meta-analysis. Infection risks of STIs in MSM, HIV-positive MSM and male sex workers (MSW) were obtained. This review followed the PRISMA guidelines and was registered in PROSPERO. RESULTS: Eighty-eight articles (11 in English and 77 in Chinese) investigating 35,203 MSM in 28 provinces were included in this review. The prevalence levels of STIs among MSM were 6.3% (95% CI: 3.5-11.0%) for chlamydia, 1.5% (0.7-2.9%) for genital wart, 1.9% (1.3-2.7%) for gonorrhoea, 8.9% (7.8-10.2%) for hepatitis B (HBV), 1.2% (1.0-1.6%) for hepatitis C (HCV), 66.3% (57.4-74.1%) for human papillomavirus (HPV), 10.6% (6.2-17.6%) for herpes simplex virus (HSV-2) and 4.3% (3.2-5.8%) for Ureaplasma urealyticum. HIV-positive MSM have consistently higher odds of all these infections than the broader MSM population. As a subgroup of MSM, MSW were 2.5 (1.4-4.7), 5.7 (2.7-12.3), and 2.2 (1.4-3.7) times more likely to be infected with chlamydia, gonorrhoea and HCV than the broader MSM population, respectively. CONCLUSION: Prevalence levels of STIs among MSW were significantly higher than the broader MSM population. Co-infection of HIV and STIs were prevalent among Chinese MSM. Integration of HIV and STIs healthcare and surveillance systems is essential in providing effective HIV/STIs preventive measures and treatments. TRIAL REGISTRATION: PROSPERO NO: CRD42013003721
Segmented RF shield design to minimize eddy currents for low-field Halbach MRI systems
MRI systems have a thin conducting layer placed between the gradient and RF coils, this acts as a shield at the RF-frequency, minimizing noise coupled into the experiment, and decreasing the coupling between the RF and gradient coils. Ideally, this layer should be transparent to the gradient fields to reduce eddy currents. In this work the design of such a shield, specifically for low-field point-of-care Halbach based MRI devices, is discussed. A segmented double layer shield is designed and constructed based on eddy current simulations. Subsequently, the performance of the improved shield is compared to a reference shield by measuring the eddy current decay times as well as using noise measurements. A maximum reduction factor of 2.9 in the eddy current decay time is observed. The segmented shield couples in an equivalent amount of noise when compared to the unsegmented reference shield. Turbo spin echo images of a phantom and the brain of a healthy volunteer show improvements in terms of blurring using the segmented shield.</p
HIV infection and sexual risk among men who have sex with men and women (MSMW): A systematic review and meta-analysis
Objectives: To estimate the number of men who have sex with men and women who are HIV-positive in the United States, and to compare HIV prevalence rates between men who have sex with men and women, men who have sex with men only, and men who have sex with women exclusively. Methods: Following PRISMA guidelines, we conducted a systematic review and meta-analysis of reports referencing HIV prevalence and men who have sex with men and women. We searched PubMed and Ovid PsycINFO for peer-reviewed, U.S.-based articles reporting on HIV prevalence among men who have sex with men and women. We conducted event rate, effect size, moderation and sensitivity analyses. Results: We estimate that 1.0% of U.S. males are bisexually-behaving, and that 121,800 bisexually-behaving men are HIV-positive. Men who have sex with men and women are less than half as likely to be HIV-positive as men who have sex with men only (16.9% vs. 33.3%; OR = 0.41, 95% CI: 0.31, 0.54), but more than five times as likely to be HIV-positive as men who have sex with women exclusively (18.3% vs. 3.5%; OR = 5.71, 95% CI: 3.47, 9.39). They are less likely to engage in unprotected receptive anal intercourse than men who have sex with men only (15.9% vs. 35.0%; OR = 0.36, 95% CI: 0.28, 0.46). Men who have sex with men and women in samples with high racial/ethnic minority proportions had significantly higher HIV prevalence than their counterparts in low racial/ethnic minority samples. Conclusions: This represents the first meta-analysis of HIV prevalence in the U.S. between men who have sex with men and women and men who have sex with men only. Data collection, research, and HIV prevention and care delivery specifically tailored to men who have sex with men and women are necessary to better quantify and ameliorate this population's HIV burden. © 2014 Friedman et al
Electrical properties tomography: a methodological review
Electrical properties tomography (EPT) is an imaging method that uses a magnetic resonance (MR) system to non-invasively determine the spatial distribution of the conductivity and permittivity of the imaged object. This manuscript starts by providing clear definitions about the data required for, and acquired in, EPT, followed by comprehensively formulating the physical equations underlying a large number of analytical EPT techniques. This thorough mathematical overview of EPT harmonizes several EPT techniques in a single type of formulation and gives insight into how they act on the data and what their data requirements are. Furthermore, the review describes machine learning-based algorithms. Iterative Solution Methods of the Maxwell Equations Using Staggered Grid Spatial Discretization
The lethal cargo of Myxococcus xanthus outer membrane vesicles
Myxococcus xanthus is a bacterial micro-predator known for hunting other microbes in a wolf pack-like manner. Outer membrane vesicles (OMVs) are produced in large quantities by M. xanthus and have a highly organized structure in the extracellular milieu, sometimes occurring in chains that link neighboring cells within a biofilm. OMVs may be a vehicle for mediating wolf pack activity by delivering hydrolytic enzymes and antibiotics aimed at killing prey microbes. Here, both the protein and small molecule cargo of the OMV and membrane fractions of M. xanthus were characterized and compared. Our analysis indicates a number of proteins that are OMV-specific or OMV-enriched, including several with putative hydrolytic function. Secondary metabolite profiling of OMVs identifies 16 molecules, many associated with antibiotic activities. Several hydrolytic enzyme homologs were identified, including the protein encoded by MXAN_3564 (mepA), an M36 protease homolog. Genetic disruption of mepA leads to a significant reduction in extracellular protease activity suggesting MepA is part of the long-predicted (yet to date undetermined) extracellular protease suite of M. xanthus
