The long-term impact of the MEMA kwa Vijana adolescent sexual and reproductive health intervention: effect of dose and time since intervention exposure.
BACKGROUND: Despite recent decreases in HIV incidence in many sub-Saharan African countries, there is little evidence that specific behavioural interventions have led to a reduction in HIV among young people. Further and wider-scale decreases in HIV require better understanding of when behaviour change occurs and why. The MEMA kwa Vijana adolescent sexual and reproductive health intervention has been implemented in rural Mwanza, Tanzania since 1999. A long-term evaluation in 2007/8 found that the intervention improved knowledge, attitudes to sex and some reported risk behaviours, but not HIV or HSV2 prevalence. The aim of this paper was to assess the differential impact of the intervention according to gender, age, marital status, number of years of exposure and time since last exposure to the intervention. METHODS: In 2007, a cross-sectional survey was conducted in the 20 trial communities among 13,814 young people (15-30 yrs) who had attended intervention or comparison schools between 1999 and 2002. Outcomes for which the intervention had an impact in 2001 or 2007 were included in this subgroup analysis. Data were analysed using cluster-level methods for stratified cluster-randomised trials, using interaction tests to determine if intervention impact differed by subgroup. RESULTS: Taking into account multiplicity of testing, concurrence with a priori hypotheses and consistency within the results no strong effect-modifiers emerged. Impact on pregnancy knowledge and reported attitudes to sex increased with years of exposure to high-quality intervention. CONCLUSIONS: The desirable long-term impact of the MEMA kwa Vijana intervention did not vary greatly according to the subgroups examined. This suggests that the intervention can have an impact on a broad cross-section of young people in rural Mwanza. TRIAL REGISTRATION: ClinicalTrials.gov NCT00248469
The development of a new measure of quality of life in the management of gastro-oesophageal reflux disease: the Reflux Questionnaire.
INTRODUCTION
This paper reports on the development of a new measure of health-related quality of life for use among patients with gastro-oesophageal reflux disease (GORD), funded as part of the REFLUX trial. This is a large UK multi centre trial that aims to compare the clinical and cost effectiveness of minimal access surgery with best medical treatment for patients with GORD within the NHS.
Method Potential items were identified via a series of interviews and focus groups carried out with patients who were receiving/had received medical or surgical treatment for GORD. The final measure consisted of 31 items covering 7 categories (Heartburn; Acid reflux; Wind; Eating and swallowing; Bowel movements; Sleep; Work, physical and social activities). The measure produced two outputs: a quality of life score (RQLS) and five Reflux symptom scores. Reliability (internal consistency), criterion validity with the SF-36 and, sensitivity to change in terms of relationship with reported change in prescribed medication were assessed amongst a sample of 794 patients recruited into the trial.
RESULTS
The measure was shown to be internally consistent, to show criterion validity with the SF-36 and sensitive to changes in patients use of prescribed medication at baseline and 3 month follow-up.
DISCUSSION
The Reflux questionnaire is a new self-administered questionnaire for use amongst patients with GORD. Initial findings suggest that the new measure is valid, reliable, acceptable to respondents and simple to administer in both a clinical and research context
Measuring geometric phases of scattering states in nanoscale electronic devices
We show how a new quantum property, a geometric phase, associated with
scattering states can be exhibited in nanoscale electronic devices. We propose
an experiment to use interference to directly measure the effect of the new
geometric phase. The setup involves a double path interferometer, adapted from
that used to measure the phase evolution of electrons as they traverse a
quantum dot (QD). Gate voltages on the QD could be varied cyclically and
adiabatically, in a manner similar to that used to observe quantum adiabatic
charge pumping. The interference due to the geometric phase results in
oscillations in the current collected in the drain when a small bias across the
device is applied. We illustrate the effect with examples of geometric phases
resulting from both Abelian and non-Abelian gauge potentials.Comment: Six pages two figure
What are communities of practice? A comparative review of four seminal works
This paper is a comparative review of four seminal works on communities of practice. It is argued that the ambiguities of the terms community and practice are a source of the concept's reusability allowing it to be reappropriated for different purposes, academic and practical. However, it is potentially confusing that the works differ so markedly in their conceptualizations of community, learning, power and change, diversity and informality. The three earlier works are underpinned by a common epistemological view, but Lave and Wenger's 1991 short monograph is often read as primarily about the socialization of newcomers into knowledge by a form of apprenticeship, while the focus in Brown and Duguid's article of the same year is, in contrast, on improvising new knowledge in an interstitial group that forms in resistance to management. Wenger's 1998 book treats communities of practice as the informal relations and understandings that develop in mutual engagement on an appropriated joint enterprise, but his focus is the impact on individual identity. The applicability of the concept to the heavily individualized and tightly managed work of the twenty-first century is questionable. The most recent work by Wenger – this time with McDermott and Snyder as coauthors – marks a distinct shift towards a managerialist stance. The proposition that managers should foster informal horizontal groups across organizational boundaries is in fact a fundamental redefinition of the concept. However it does identify a plausible, if limited, knowledge management (KM) tool. The first GCT camera for the Cherenkov Telescope Array
The Gamma Cherenkov Telescope (GCT) is proposed to be part of the Small Size
Telescope (SST) array of the Cherenkov Telescope Array (CTA). The GCT
dual-mirror optical design allows the use of a compact camera of diameter
roughly 0.4 m. The curved focal plane is equipped with 2048 pixels of
~0.2{\deg} angular size, resulting in a field of view of ~9{\deg}. The GCT
camera is designed to record the flashes of Cherenkov light from
electromagnetic cascades, which last only a few tens of nanoseconds. Modules
based on custom ASICs provide the required fast electronics, facilitating
sampling and digitisation as well as first level of triggering. The first GCT
camera prototype is currently being commissioned in the UK. On-telescope tests
are planned later this year. Here we give a detailed description of the camera

Suzaku and SWIFT-BAT observations of a newly discovered Compton-thick AGN
Obscured AGN are fundamental to understand the history of Super Massive Black
Hole growth and their influence on galaxy formation. However, the Compton-thick
AGN (NH>1e24 cm^-2) population is basically unconstrained, with less than few
dozen confirmed Compton-thick AGN found and studied so far. A way to select
heavily obscured AGN is to compare the X-ray emission below 10 keV (which is
strongly depressed) with the emission from other bands less affected by the
absorption, i.e. the IR band. To this end, we have cross-correlated the 2XMM
catalogue with the IRAS Point Source catalogue and, by using the X-ray to
infrared flux ratio and X-ray colors, we selected a well defined sample of
Compton-thick AGN candidates at z<0.1. The aim of this work is to confirm the
nature and to study one of these local Compton-thick AGN candidates, the nearby
(z=0.029) Seyfert 2 galaxy IRAS 04507+0358, by constraining the amount of
intrinsic absorption (NH) and thus the intrinsic luminosity. To this end we
obtained deep (100 ks) Suzaku observations (AO4 call) and performed a joint fit
with SWIFT-BAT data. We analyzed XMM-Newton, Suzaku and SWIFT-BAT data and we
present here the results of this broad-band (0.4-100 keV) spectral analysis. We
found that the broad-band X-ray emission of IRAS 04507+0358 requires a large
amount of absorption (larger than 1e24 cm^-2) to be well reproduced, thus
confirming the Compton-thick nature of this source. In particular, the most
probable scenario is that of a mildly (NH (1.3-1.5)x1e24 cm^-2, L(2-10 keV)
(5-7)x1e43 erg s^-1) Compton-thick AGN.Comment: Accepted for publication in A&A; 8 pages, 7 figure
Family composition and age at menarche: findings from the international Health Behaviour in School-Aged Children Study
This research was funded by The University of St Andrews and NHS Health Scotland.Background Early menarche has been associated with father absence, stepfather presence and adverse health consequences in later life. This article assesses the association of different family compositions with the age at menarche. Pathways are explored which may explain any association between family characteristics and pubertal timing. Methods Cross-sectional, international data on the age at menarche, family structure and covariates (age, psychosomatic complaints, media consumption, physical activity) were collected from the 2009–2010 Health Behaviour in School-aged Children (HBSC) survey. The sample focuses on 15-year old girls comprising 36,175 individuals across 40 countries in Europe and North America (N = 21,075 for age at menarche). The study examined the association of different family characteristics with age at menarche. Regression and path analyses were applied incorporating multilevel techniques to adjust for the nested nature of data within countries. Results Living with mother (Cohen’s d = .12), father (d = .08), brothers (d = .04) and sisters (d = .06) are independently associated with later age at menarche. Living in a foster home (d = −.16), with ‘someone else’ (d = −.11), stepmother (d = −.10) or stepfather (d = −.06) was associated with earlier menarche. Path models show that up to 89% of these effects can be explained through lifestyle and psychological variables. Conclusions Earlier menarche is reported amongst those with living conditions other than a family consisting of two biological parents. This can partly be explained by girls’ higher Body Mass Index in these families which is a biological determinant of early menarche. Lower physical activity and elevated psychosomatic complaints were also more often found in girls in these family environments.Publisher PDFPeer reviewe
Heatwave planning: the role of the community in co-producing resilience
Drawing on a thematic analysis of relevant policy documents, the aim of this paper is to comment on an apparent disconnect between two associated contemporary UK policy areas: planning for heatwaves and community resilience. Regional and national policy documents that plan for heatwaves in the UK tend to focus on institutional emergency responses and infrastructure development. In these documents, although communities are mentioned, they are understood as passive recipients of resilience that is provided by active institutions. Meanwhile, contemporary discussion about community resilience highlights the potential for involving communities in planning for and responding to emergencies (although the concept is also the subject of critique). Within this context, the paper proposes that – through engagement with the ‘community resilience’ policy agenda and its critique – effort should be made to articulate and realise greater participation by individuals, and voluntary and community sector groups in heatwave preparation, planning and response
Supermassive Black Holes in Galactic Nuclei: Past, Present and Future Research
This review discusses the current status of supermassive black hole research,
as seen from a purely observational standpoint. Since the early '90s, rapid
technological advances, most notably the launch of the Hubble Space Telescope,
the commissioning of the VLBA and improvements in near-infrared speckle imaging
techniques, have not only given us incontrovertible proof of the existence of
supermassive black holes, but have unveiled fundamental connections between the
mass of the central singularity and the global properties of the host galaxy.
It is thanks to these observations that we are now, for the first time, in a
position to understand the origin, evolution and cosmic relevance of these
