    Wear and corrosion interactions on titanium in oral environment : literature review

    The oral cavity is a complex environment where corrosive substances from dietary, human saliva, and oral biofilms may accumulate in retentive areas of dental implant systems and prostheses promoting corrosion at their surfaces. Additionally, during mastication, micromovements may occur between prosthetic joints causing a relative motion between contacting surfaces, leading to wear. Both processes (wear and corrosion) result in a bio-tribocorrosion system once that occurs in contact with biological tissues and fluids. This review paper is focused on the aspects related to the corrosion and wear behavior of titanium-based structures in the oral environment. Furthermore, the clinical relevance of the oral environment is focused on the harmful effect that acidic substances and biofilms, formed in human saliva, may have on titanium surfaces. In fact, a progressive degradation of titanium by wear and corrosion (tribocorrosion) mechanisms can take place affecting the performance of titanium-based implant and prostheses. Also, the formation of wear debris and metallic ions due to the tribocorrosion phenomena can become toxic for human tissues. This review gathers knowledge from areas like materials sciences, microbiology, and dentistry contributing to a better understanding of bio-tribocorrosion processes in the oral environment.(undefined

    Balancing repair and tolerance of DNA damage caused by alkylating agents

    Alkylating agents constitute a major class of frontline chemotherapeutic drugs that inflict cytotoxic DNA damage as their main mode of action, in addition to collateral mutagenic damage. Numerous cellular pathways, including direct DNA damage reversal, base excision repair (BER) and mismatch repair (MMR), respond to alkylation damage to defend against alkylation-induced cell death or mutation. However, maintaining a proper balance of activity both within and between these pathways is crucial for a favourable response of an organism to alkylating agents. Furthermore, the response of an individual to alkylating agents can vary considerably from tissue to tissue and from person to person, pointing to genetic and epigenetic mechanisms that modulate alkylating agent toxicity

    Endoglin controls blood vessel diameter through endothelial cell shape changes in response to haemodynamic cues

    The hierarchical organization of properly sized blood vessels ensures the correct distribution of blood to all organs of the body, and is controlled via haemodynamic cues. In current concepts, an endothelium-dependent shear stress set point causes blood vessel enlargement in response to higher flow rates, while lower flow would lead to blood vessel narrowing, thereby establishing homeostasis. We show that during zebrafish embryonic development increases in flow, after an initial expansion of blood vessel diameters, eventually lead to vessel contraction. This is mediated via endothelial cell shape changes. We identify the transforming growth factor beta co-receptor endoglin as an important player in this process. Endoglin mutant cells and blood vessels continue to enlarge in response to flow increases, thus exacerbating pre-existing embryonic arterial-venous shunts. Together, our data suggest that cell shape changes in response to biophysical cues act as an underlying principle allowing for the ordered patterning of tubular organs.Drug Delivery Technolog

    Eight million Ukrainians have taken refuge in the European Union. Many have asthma and/or allergic rhinitis and/or urticaria, and around 100,000 may have a severe disease. Cultural and language barriers are a major obstacle to appropriate management. Two widely available mHealth apps, MASK-air® (Mobile Airways Sentinel NetworK) for the management of rhinitis and asthma and CRUSE® (Chronic Urticaria Self Evaluation) for patients with chronic spontaneous urticaria, were updated to include Ukrainian versions that make the documented information available to treating physicians in their own language. The Ukrainian patients fill in the questionnaires and daily symptom-medication scores for asthma, rhinitis (MASK-air) or urticaria (CRUSE) in Ukrainian. Then, following the GDPR, patients grant their physician access to the app by scanning a QR code displayed on the physician's computer enabling the physician to read the app contents in his/her own language. This service is available freely. It takes less than a minute to show patient data to the physician in the physician's web browser.     Concepts for the development of person-centred, digitally-enabled, Artificial Intelligence-assisted ARIA care pathways (ARIA 2024)

    The traditional healthcare model is focused on diseases (medicine and natural science) and does not acknowledge patients' resources and abilities to be experts in their own life based on their lived experiences. Improving healthcare safety, quality and coordination, as well as quality of life, are important aims in the care of patients with chronic conditions. Person-centred care needs to ensure that people's values and preferences guide clinical decisions. This paper reviews current knowledge to develop (i) digital care pathways for rhinitis and asthma multimorbidity and (ii) digitally-enabled person-centred care (1). It combines all relevant research evidence, including the so-called real-world evidence, with the ultimate goal to develop digitally-enabled, patient-centred care. The paper includes (i) Allergic Rhinitis and its Impact on Asthma (ARIA), a two-decade journey, (ii) Grading of Recommendations, Assessment, Development and Evaluation (GRADE), the evidence-based model of guidelines in airway diseases, (iii) mHealth impact on airway diseases, (iv) from guidelines to digital care pathways, (v) embedding Planetary Health, (vi) novel classification of rhinitis and asthma, (vi) embedding real-life data with population-based studies, (vii) the ARIA-EAACI strategy for the management of airway diseases using digital biomarkers, (viii) Artificial Intelligence, (ix) the development of digitally-enabled ARIA Person-Centred Care and (x) the political agenda.     CRUSE®-An innovative mobile application for patient monitoring and management in chronic spontaneous urticaria

    Background: Chronic spontaneous urticaria (CSU) is unpredictable and can severely impair patients' quality of life. Patients with CSU need a convenient, user-friendly platform to complete patient-reported outcome measures (PROMs) on their mobile devices. CRUSE®, the Chronic Urticaria Self Evaluation app, aims to address this unmet need. Methods: CRUSE® was developed by an international steering committee of urticaria specialists. Priorities for the app based on recent findings in CSU were defined to allow patients to track and record their symptoms and medication use over time and send photographs. The CRUSE® app collects patient data such as age, sex, disease onset, triggers, medication, and CSU characteristics that can be sent securely to physicians, providing real-time insights. Additionally, CRUSE® contains PROMs to assess disease activity and control, which are individualised to patient profiles and clinical manifestations. Results: CRUSE® was launched in Germany in March 2022 and is now available for free in 17 countries. It is adapted to the local language and displays a country-specific list of available urticaria medications. English and Ukrainian versions are available worldwide. From July 2022 to June 2023, 25,710 observations were documented by 2540 users; 72.7% were females, with a mean age of 39.6 years. At baseline, 93.7% and 51.3% of users had wheals and angioedema, respectively. Second-generation antihistamines were used in 74.0% of days. Conclusions: The initial data from CRUSE® show the wide use and utility of effectively tracking patients' disease activity and control, paving the way for personalised CSU management

    Geoecological problems of decentralization (on Ternopol region materials)

    The objective of the article is to study the essential geoecological problems of Ukrainian associated territorial communities. The necessity of implementation of effective environmental management, through introduction of an official responsible for improvement, ecology and sustainable development of the associated territorial communities in the administrative apparatus of communities is substantiated.The decentralization reform created a considerable potential for development of the local communities. The transmission of significant financial resources and powers provided the preconditions for the formation of competent associated communities. It was discovered during the study that despite of significant advantages of new administrative units, managerial approaches to the use and preservation of natural resources had not changed. Administration of forest, water, mineral and land resources is further carried out from the center. Practically no one cares about the main ecological problems, communities are drowning in wastes, using water polluted municipal waste with agrochemicals, and the last hectares of forests are being cut down and used as energy resources. In context of sustainable development, the priority issues to be solved for 38% of the communities in Ternopil region are the issues of an ecological nature the issues of an economic nature are priority for 34% of communities and the issue of the social nature – of 28% of comminities. In Ternopil region, as in one of the leaders in decentralization reform, 78% of community leaders consider high-quality water supply to be much more important than availability of cooperative or lighting of the streets. The most actual in regional communities are issues concerning sorting and utilization of wastes, lack of treatment facilities and an outdated drainage system.The article proposes amendments to the Article 19 of the Law of Ukraine «On Environmental Protection», regarding the extension of powers of village, settlement, city councils as well as associated territorial communities in the field of Environmental protection. The necessity of introduction the position of ecologist responsible for improvement, ecology and sustainable development of the community is substantiated. His duties are the following: monitoring of components of the environment; struggle with the illegal landfills, regulation of processes of economical nature management; optimization of land use; control of rational water use, preparation of a submission for the creation of new protected objects of local importance; creation of recreational and green zones.</jats:p

    Basin systems of small rivers of Western Podillya: state, change tendencies, perspectives of nature management and nature protection optimization

    The level of anthropogenization of the natural processes and the geocomponents of the basin geosystems of small rivers in Western Podillya has been estimated and rated as ecologically dangerous from the viewpoint of sustainable and conflict free functioning. The scales of the transformation of the components of natural environment by economic activities since 1774 were revealed using the method of comparative-geographic analysis of cartographic sources. The scales of deforestation were determined, as well as the scales of the influences of drainage meliorations on wetlands, river floodplains and riverbed complexes. It has been established that such transformations of the state of the components of landscape systems have caused the manifestation of a set of unfavorable processes and phenomena (lowering of ground water level, promoting desiccation, soil erosion and deflation, soil dehumification, decreasing landscape and biological diversity, etc.). Calculated indices of the anthropogenic modification of natural components testify that the strongest adversary impacts on river systems and basin landscapes are caused by agriculture, deforestation, and drainage meliorations. Our analysis of the current state of reclaimed lands in the basins of the rivers Dzhuryn and Nichlava confirmed the conclusions of B. I Kozlovsky on the effects of drainage reclamation on groundwater in drained lands and of the formation within them and around drainage systems of negative hydrogeological zones of different widths. In the absence of precipitation for 30-45 consecutive days in summer there is a sharp decrease in groundwater levels, and overdrying of soils, which causes the manifestation and intensification of deflation, shallowing and even drying of the upper reaches of rivers and streams. At the final stage of the study, a system of measures aimed at ensuring the sustainability of river basin geosystems was substantiated. The introduction of an optimization model of land use in the basin geosystem is one of the priority tasks in the context of negative changes in the water regime of watercourses and the water balance of river basin systems. Optimization measures provide for the transformation of the part of degraded and unproductive lands towards the grasslands and the planting of gardens (slopes up to 7°) and afforestation (surface steepness over 7°) to improve the quality of environment and to form the environmentally secure land use system. Regional indices of anthropogenic transformation for the existing and proposed land structure as a normative regional indices of nature utilization optimality are calculated. Substantiation of schemes of basin nature protection networks was based on taking into account the role of protected areas in maintaining certain functional features at the sources, in the middle and lower parts of river basins. Based on the results of field surveys, it is proposed to create nine protected areas within the Dzhuryn Basin and eight protected areas within the Nichlava river basin, which will increase the share of protected areas of the Dzhuryn basin to 8% (compared to present 4.8%) and Nichlava to 19%. At the same time, it is proposed to change the structure of the nature reserve fund of the Nichlava river basin, taking into account the existing high share (77%) of general zoological reserves, inefficient from the standpoint of conservation of natural complexes, instead creating six landscape reserves on an area of about 800 hectares. The paper considers the possibility of further development of the tourist and recreational sphere in the near-Dnister sections of the river basins of Dzhuryn and Nichlava, and proposes the creation of Borshchiv Regional landscape park in the picturesque valley of the Nichlava River.</jats:p
