Diagnosis of cutaneous T-cell lymphoma by insurance type before and after the Affordable Care Act: a national database study
The Affordable Care Act (ACT) was implemented to increase health care access and reduce the uninsured in the age group between pediatric and Medicare populations (18-64). The association of the ACA with insurance type upon diagnosis (uninsured, Medicaid, non-Medicaid) has been investigated for otolaryngologic, gynecologic, and the top five non-skin malignancies. Such studies for cutaneous malignancies are lacking. We conducted a retrospective analysis of the prospective National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) cancer database to assess the impact of the ACA on new diagnoses of cutaneous T-cell lymphoma (CTCL) by insurance type. Unlike prior studies of other malignancies, we did not observe significant differences between rate of diagnosis of CTCL by insurance type before and after full implementation of the ACA in all states, expansion states, and non-expansion states. Skin cancers do not have screening guidelines and CTCL is an uncommon malignancy, both of which may contribute to these findings. However, Medicaid-expansion states were much closer to reducing the percentage of newly diagnosed uninsured patients with CTCL than non-expansion states. As such, it may be prudent to investigate intrinsic socioeconomic barriers to care in Medicaid patients to improve their access to care to decrease the uninsured population and improve outcomes
The anti-cancer effect of retinoic acid signaling in CRC occurs via decreased growth of ALDH+ colon cancer stem cells and increased differentiation of stem cells
Background: Tumorigenesis is driven by stem cell (SC) overpopulation. BecauseALDH is both a marker for SCs in many tissues and a key enzyme in retinoid acid (RA)signaling, we studied RA signaling in normal and malignant colonic SCs.Hypothesis: RA signaling regulates growth and differentiation of ALDH+ colonicSCs dysregulation of RA signaling contributes to SC overpopulation and colorectalcancer (CRC) development. Methods: We analyzed normal and malignant colonic tissues and CRC cell linesto see if retinoid receptors (RXR &RAR) are exclusively expressed in ALDH+ SCs,and if RA signaling changes during CRC development. We determined whether RAsignaling regulates cancer SC (CSC) proliferation, differentiation, sphere formation, and population size. Results: RXR &RAR were expressed in ALDH+ colonic SCs, but not in MCM2+proliferative cells. Western blotting/immunostaining of CRCs revealed that RAsignaling components become overexpressed in parallel with ALDH overexpression, which coincides with the known overpopulation of ALDH+ SCs that occurs during, and drives, CRC development. Treatment of SCs with all-trans retinoic acid (ATRA)decreased proliferation, sphere formation and ALDH+ SC population size, and induceddifferentiation along the neuroendocrine cell (NEC) lineage. Conclusions: Retinoid signaling, by regulating ALDH+ colonic CSCs, decreases SCproliferation, sphere formation, and population size, and increases SC differentiation toNECs. Dysregulation of RA signaling in colonic SCs likely contributes to overpopulationof ALDH+ SCs and CRC growth.Implications: That retinoid receptors RXR and RAR are selectively expressed inALDH+ SCs indicates RA signaling mainly occurs via ALDH+ SCs, which provides amechanism to selectively target CSCs. © 2018 Impact Journals LLC. Thermal modeling of lithium ion batteries for temperature rise predictions in hybrid vehicle application
In order to develop a hybrid vehicle with lithium ion battery packs, it is necessary to understand the thermal behaviour of the lithium ion batteries used. This paper focuses on predicting the temperature rise of lithium ion batteries during a drive cycle in hybrid two wheeler applications. To predict the rise in temperature, a model is developed in Simulink, parameterized using the empirical parameters. The model is based on the Joule heating effect and heat capacity equation while considering the variation of internal resistance with respect to ambient temperature of operation, state of charge and C rate of operation. The internal resistance is measured by parameter evaluation testing through the pulse power characterisation method. To validate the Simulink model, the lithium ion batteries are tested on standard drive cycles and constant current discharges, and the rise in temperature is measured. The accuracy of the Simulink model was found to be ± 2.2°C, which is acceptable for this study and comparable to the other available models in the literature
Avoiding Drunken Driving Road Accidents by using Alcoholic Sensors
The purpose of this work is to avoid drunken driving accident by using alcohol detecting sensor. The uncontrolled many accidents were happened because of influence of alcohol during driving. To avoid such accidents, we have proposed alcoholic sensor in the vehicle ignition system. This system is integrating the alcohol sensor with microcontroller, relay switch and battery. The alcohol sensor used in this project is MQ-3 which detects the presence of alcohol content in human breath. Vehicle ignition system gets power from battery through relay switch. The ignition system will operate based on the level of Blood Alcohol Content (BAC) in human breath detected by the alcohol sensor and the signal will send to microcontroller, which will supply power to spark plug
Welding
Friction welding method is one of the most simple, economical and highly productive methods in joining similar and dissimilar metals. It is widely used in the automotive, aircraft and aerospace industrial applications. For many applications it is often necessary to join aluminium (6061) to make finished part. In this project the main aim is to weld the small thickness of aluminum (6061) plates for that friction welding machine used is of higher cost. Here the aim is reduce the cost of friction welding machine with simple parts like three phase A.C induction motor, bush, frame stand, friction tool, universal vice, vertical moving bed, horizontal moving bed, etc. The result expected would be of same strength as that of old friction welding machine. It is very easy and at same time production time is very much reduced. This machine is best suitable for mass production
Bis(phosphinoselenoic amides) as versatile chelating ligands for alkaline earth metal (Mg, Ca, Sr and Ba) complexes: syntheses, structure and ε-caprolactone polymerisation
We report here a series of heavier alkaline earth metal complexes with N,N′-(ethane-1,2-diyl)bis(P,P-diphenylphosphinoselenoic amide) using two synthetic routes. In the first route, the heavier alkaline earth metal bis(trimethylsilyl)amides [M{N(SiMe3)2}2(THF)n] (M = Ca, Sr, Ba), when treated with phosphinoselenoic amine [Ph2P(Se)NHCH2CH2NHPPh2(Se)] (1), afforded the corresponding alkaline earth metal complexes of the composition [(THF)3M{Ph2P(Se)NCH2CH2NPPh2(Se)}] [M = Ca (2), Sr (3), Ba (4)]. The metal complexes 2–4 were also obtained from a one-pot reaction, where potassium phosphinoselenoic amide was generated in situ by the reaction of compound 1 and [KN(SiMe3)2], followed by the addition of the respective metal diiodides in THF at room temperature. The magnesium complex [(THF)3Mg{Ph2P(Se)NCH2CH2NPPh2(Se)}] (5) was also prepared. The solid-state structures of alkaline earth metal complexes 2–5 were established by single crystal X-ray diffraction analysis. In the solid state, all the metal complexes are monomeric but in complexes 2–4, ligand 1 is chelated in a tetra-dentate fashion to each metal ion but in complex 5, ligand 1 behaves as a bidentate ligand. Complexes 2–4 were tested as catalysts for the ring-opening polymerisation of ε-caprolactone and a high level of activity for the barium complex 4 was observed, with narrow polydispersity. We also report the synthesis and structure of the bis(amidophosphino borane) ligand [Ph2P(BH3)NHCH2CH2NHPPh2(BH3)] (6) and the corresponding barium complex [(THF)2Ba{Ph2P(BH3)NCH2CH2NPPh2(BH3)}]2 (7)

Exploration of PET and MRI radiomic features for decoding breast cancer phenotypes and prognosis.
Radiomics is an emerging technology for imaging biomarker discovery and disease-specific personalized treatment management. This paper aims to determine the benefit of using multi-modality radiomics data from PET and MR images in the characterization breast cancer phenotype and prognosis. Eighty-four features were extracted from PET and MR images of 113 breast cancer patients. Unsupervised clustering based on PET and MRI radiomic features created three subgroups. These derived subgroups were statistically significantly associated with tumor grade (p = 2.0 × 10-6), tumor overall stage (p = 0.037), breast cancer subtypes (p = 0.0085), and disease recurrence status (p = 0.0053). The PET-derived first-order statistics and gray level co-occurrence matrix (GLCM) textural features were discriminative of breast cancer tumor grade, which was confirmed by the results of L2-regularization logistic regression (with repeated nested cross-validation) with an estimated area under the receiver operating characteristic curve (AUC) of 0.76 (95% confidence interval (CI) = [0.62, 0.83]). The results of ElasticNet logistic regression indicated that PET and MR radiomics distinguished recurrence-free survival, with a mean AUC of 0.75 (95% CI = [0.62, 0.88]) and 0.68 (95% CI = [0.58, 0.81]) for 1 and 2 years, respectively. The MRI-derived GLCM inverse difference moment normalized (IDMN) and the PET-derived GLCM cluster prominence were among the key features in the predictive models for recurrence-free survival. In conclusion, radiomic features from PET and MR images could be helpful in deciphering breast cancer phenotypes and may have potential as imaging biomarkers for prediction of breast cancer recurrence-free survival
