An extracellular steric seeding mechanism for Eph-ephrin signaling platform assembly
Erythropoetin-producing hepatoma (Eph) receptors are cell-surface protein tyrosine kinases mediating cell-cell communication. Upon activation, they form signaling clusters. We report crystal structures of the full ectodomain of human EphA2 (eEphA2) both alone and in complex with the receptor-binding domain of the ligand ephrinA5 (ephrinA5 RBD). Unliganded eEphA2 forms linear arrays of staggered parallel receptors involving two patches of residues conserved across A-class Ephs. eEphA2-ephrinA5 RBD forms a more elaborate assembly, whose interfaces include the same conserved regions on eEphA2, but rearranged to accommodate ephrinA5 RBD. Cell-surface expression of mutant EphA2s showed that these interfaces are critical for localization at cell-cell contacts and activation-dependent degradation. Our results suggest a 'nucleation' mechanism whereby a limited number of ligand-receptor interactions 'seed' an arrangement of receptors which can propagate into extended signaling arrays
Quasi-Normal Modes of Stars and Black Holes
Perturbations of stars and black holes have been one of the main topics of
relativistic astrophysics for the last few decades. They are of particular
importance today, because of their relevance to gravitational wave astronomy.
In this review we present the theory of quasi-normal modes of compact objects
from both the mathematical and astrophysical points of view. The discussion
includes perturbations of black holes (Schwarzschild, Reissner-Nordstr\"om,
Kerr and Kerr-Newman) and relativistic stars (non-rotating and
slowly-rotating). The properties of the various families of quasi-normal modes
are described, and numerical techniques for calculating quasi-normal modes
reviewed. The successes, as well as the limits, of perturbation theory are
presented, and its role in the emerging era of numerical relativity and

Fluorine-induced improvement of structural and optical properties of CdTe thin films for solar cell efficiency enhancement
CdTe thin films of different thicknesses were electrodeposited and annealed in air after different chemical treatments to study the effects of thickness and the different chemical treatments on these films for photovoltaic applications. The thicknesses of the samples range from 1.1 μm to 2.1 μm and the annealing process was carried out after prior CdCl2 treatment and CdCl2+CdF2 treatment as well as without any chemical treatment. Detailed optical and structural characterisation of the as-deposited and annealed CdTe thin films using UV-Vis spectrophotometry and x-ray diffraction reveal that incorporating fluorine in the well-known CdCl2 treatment of CdTe produces remarkable improvement in the optical and structural properties of the materials. This CdCl2+CdF2 treatment produced solar cell with efficiency of 8.3% compared to CdCl2 treatment, with efficiency of 3.3%. The results reveal an alternative method of post-deposition chemical treatment of CdTe which can lead to the production of CdTe-based solar cells with enhanced photovoltaic conversion efficiencies compared to the use of only CdCl2.

The incidence of unpleasant dreams after sub-anaesthetic ketamine
Ketamine is an N-methyl-D-aspartate (NMDA)receptor antagonist with psychotogenic effects and for whichthere are diverse reports of whether pleasant or unpleasantdreams result during anaesthesia, post-operatively or aftersub-anaesthetic use. The aim was to assess in healthy volunteers the incidence ofunpleasant dreams over the three nights after receiving asub-anaesthetic dose of ketamine, in comparison to placebo,and with retrospective home nightmare frequency as acovariate.Thirty healthy volunteers completed questionnairesabout retrospective home dream recall and were then giveneither ketamine or placebo. Ketamine resulted in significantly more meandream unpleasantness relative to placebo and caused athreefold increase in the odds ratio for the incidence of anunpleasant dream. The number of dreams reported over thethree nights did not differ between the groups. Theincidence of unpleasant dreams after ketamine use waspredicted by retrospectively assessed nightmare frequencyat home.Ketamine causes unpleasant dreams over thethree post-administration nights. This may be evidence of aresidual psychotogenic effect that is not found on standardself-report symptomatology measures or a result of disturbedsleep electrophysiology. Theory of disk accretion onto supermassive black holes
Accretion onto supermassive black holes produces both the dramatic phenomena
associated with active galactic nuclei and the underwhelming displays seen in
the Galactic Center and most other nearby galaxies. I review selected aspects
of the current theoretical understanding of black hole accretion, emphasizing
the role of magnetohydrodynamic turbulence and gravitational instabilities in
driving the actual accretion and the importance of the efficacy of cooling in
determining the structure and observational appearance of the accretion flow.
Ongoing investigations into the dynamics of the plunging region, the origin of
variability in the accretion process, and the evolution of warped, twisted, or

A randomised, double-blind, placebo-controlled trial of repeated nebulisation of non-viral cystic fibrosis transmembrane conductance regulator (CFTR) gene therapy in patients with cystic fibrosis
BACKGROUND: Cystic fibrosis (CF) is a chronic, life-limiting disease caused by mutations in the CF
transmembrane conductance regulator (CFTR) gene leading to abnormal airway surface ion transport,
chronic lung infections, inflammation and eventual respiratory failure. With the exception of the
small-molecule potentiator, ivacaftor (Kalydeco®, Vertex Pharmaceuticals, Boston, MA, USA), which is
suitable for a small proportion of patients, there are no licensed therapies targeting the basic defect.
The UK Cystic Fibrosis Gene Therapy Consortium has taken a cationic lipid-mediated CFTR gene therapy
formulation through preclinical and clinical development.
OBJECTIVE: To determine clinical efficacy of the formulation delivered to the airways over a period of
1 year in patients with CF.
DESIGN: This was a randomised, double-blind, placebo-controlled Phase IIb trial of the CFTR gene–liposome
complex pGM169/GL67A. Randomisation was performed via InForm™ version 4.6 (Phase Forward
Incorporated, Oracle, CA, USA) and was 1 : 1, except for patients in the mechanistic subgroups (2 : 1).
Allocation was blinded by masking nebuliser chambers.
SETTINGS: Data were collected in the clinical and scientific sites and entered onto a trial-specific InForm,
version 4.6 database.
PARTICIPANTS: Patients with CF aged ≥ 12 years with forced expiratory volume in the first second (FEV1)
between 50% and 90% predicted and any combination of CFTR mutations. The per-protocol group
(≥ 9 doses) consisted of 54 patients receiving placebo (62 randomised) and 62 patients receiving gene
therapy (78 randomised).
INTERVENTIONS: Subjects received 5 ml of nebulised pGM169/G67A (active) or 0.9% saline (placebo) at
28 (±5)-day intervals over 1 year.
MAIN OUTCOME MEASURES: The primary end point was the relative change in percentage predicted FEV1
over the 12-month period. A number of secondary clinical outcomes were assessed alongside safety
measures: other spirometric values; lung clearance index (LCI) assessed by multibreath washout; structural
disease on computed tomography (CT) scan; the Cystic Fibrosis Questionnaire – Revised (CFQ-R), a
validated quality-of-life questionnaire; exercise capacity and monitoring; systemic and sputum inflammatory
markers; and adverse events (AEs). A mechanistic study was performed in a subgroup in whom transgene
deoxyribonucleic acid (DNA) and messenger ribonucleic acid (mRNA) was measured alongside nasal and
lower airway potential difference.
RESULTS: There was a significant (p = 0.046) treatment effect (TE) of 3.7% [95% confidence interval (CI)
0.1% to 7.3%] in the primary end point at 12 months and in secondary end points, including forced vital
capacity (FVC) (p = 0.031) and CT gas trapping (p = 0.048). Other outcomes, although not reaching
statistical significance, favoured active treatment. Effects were noted by 1 month and were irrespective
of sex, age or CFTR mutation class. Subjects with a more severe baseline FEV1 had a FEV1 TE of 6.4%
(95% CI 0.8% to 12.1%) and greater changes in many other secondary outcomes. However, the more
mildly affected group also demonstrated benefits, particularly in small airway disease markers such as LCI.
The active group showed a significantly (p = 0.032) greater bronchial chloride secretory response. No
difference in treatment-attributable AEs was seen between the placebo and active groups.
CONCLUSIONS: Monthly application of the pGM169/GL67A gene therapy formulation was associated with
an improvement in lung function, other clinically relevant parameters and bronchial CFTR function,
compared with placebo.
LIMITATIONS: Although encouraging, the improvement in FEV1 was modest and was not accompanied by
detectable improvement in patients’ quality of life.
FUTURE WORK: Future work will focus on attempts to increase efficacy by increasing dose or frequency,
the coadministration of a CFTR potentiator, or the use of modified viral vectors capable of
repeated administration.

Exploring perceptions of advertising ethics: an informant-derived approach
Whilst considerable research exists on determining consumer responses to pre-determined statements within numerous ad ethics contexts, our understanding of consumer thoughts regarding ad ethics in general remains lacking. The purpose of our study therefore is to provide a first illustration of an emic and informant-based derivation of perceived ad ethics. The authors use multi-dimensional scaling as an approach enabling the emic, or locally derived deconstruction of perceived ad ethics. Given recent calls to develop our understanding of ad ethics in different cultural contexts, and in particular within the Middle East and North Africa (MENA) region, we use Lebanon—the most ethically charged advertising environment within MENA—as an illustrative context for our study. Results confirm the multi-faceted and pluralistic nature of ad ethics as comprising a number of dimensional themes already salient in the existing literature but in addition, we also find evidence for a bipolar relationship between individual themes. The specific pattern of inductively derived relationships is culturally bound. Implications of the findings are discussed, followed by limitations of the study and recommendations for further research
The effects of integrated care: a systematic review of UK and international evidence
BACKGROUND: Healthcare systems around the world have been responding to the demand for better integrated models of service delivery. However, there is a need for further clarity regarding the effects of these new models of integration, and exploration regarding whether models introduced in other care systems may achieve similar outcomes in a UK national health service context. METHODS: The study aimed to carry out a systematic review of the effects of integration or co-ordination between healthcare services, or between health and social care on service delivery outcomes including effectiveness, efficiency and quality of care. Electronic databases including MEDLINE; Embase; PsycINFO; CINAHL; Science and Social Science Citation Indices; and the Cochrane Library were searched for relevant literature published between 2006 to March 2017. Online sources were searched for UK grey literature, and citation searching, and manual reference list screening were also carried out. Quantitative primary studies and systematic reviews, reporting actual or perceived effects on service delivery following the introduction of models of integration or co-ordination, in healthcare or health and social care settings in developed countries were eligible for inclusion. Strength of evidence for each outcome reported was analysed and synthesised using a four point comparative rating system of stronger, weaker, inconsistent or limited evidence. RESULTS: One hundred sixty seven studies were eligible for inclusion. Analysis indicated evidence of perceived improved quality of care, evidence of increased patient satisfaction, and evidence of improved access to care. Evidence was rated as either inconsistent or limited regarding all other outcomes reported, including system-wide impacts on primary care, secondary care, and health care costs. There were limited differences between outcomes reported by UK and international studies, and overall the literature had a limited consideration of effects on service users. CONCLUSIONS: Models of integrated care may enhance patient satisfaction, increase perceived quality of care, and enable access to services, although the evidence for other outcomes including service costs remains unclear. Indications of improved access may have important implications for services struggling to cope with increasing demand. TRIAL REGISTRATION: Prospero registration number: 42016037725
Early Gnathostome Phylogeny Revisited: Multiple Method Consensus
This is an open access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.A series of recent studies recovered consistent phylogenetic scenarios of jawed vertebrates, such as the paraphyly of placoderms with respect to crown gnathostomes, and antiarchs as the sister group of all other jawed vertebrates. However, some of the hylogenetic relationships within the group have remained controversial, such as the positions of Entelognathus, ptyctodontids, and the Guiyu-lineage that comprises Guiyu, Psarolepis and Achoania. The revision of the dataset in a recent study reveals a modified phylogenetic hypothesis, which shows that some of these phylogenetic conflicts were sourced from a few inadvertent miscodings. The interrelationships of early gnathostomes are addressed based on a combined new dataset with 103 taxa and 335 characters, which is the most comprehensive morphological dataset constructed to date. This dataset is investigated in a phylogenetic context using maximum parsimony (MP), Bayesian inference (BI) and maximum likelihood (ML) approaches in an attempt to explore the consensus and incongruence between the hypotheses of early gnathostome interrelationships recovered from different methods. Our findings consistently corroborate the paraphyly of placoderms, all `acanthodians' as a paraphyletic stem group of chondrichthyans, Entelognathus as a stem gnathostome, and the Guiyu-lineage as stem sarcopterygians. The incongruence using different methods is less significant than the consensus, and mainly relates to the positions of the placoderm Wuttagoonaspis, the stem chondrichthyan Ramirosuarezia, and the stem osteichthyan LophosteusÐthe taxa that are either poorly known or highly specialized in character complement. Given that the different performances of each phylogenetic approach, our study provides an empirical case that the multiple phylogenetic analyses of

The demand to obtain answers to highly complex problems within strong-field gravity has been met with significant progress in the numerical solution of Einstein's equations - along with some spectacular results - in various setups. We review techniques for solving Einstein's equations in generic spacetimes, focusing on fully nonlinear evolutions but also on how to benchmark those results with perturbative approaches.
