How are falls and fear of falling associated with objectively measured physical activity in a cohort of community-dwelling older men?
BACKGROUND: Falls affect approximately one third of community-dwelling older adults each year and have serious health and social consequences. Fear of falling (FOF) (lack of confidence in maintaining balance during normal activities) affects many older adults, irrespective of whether they have actually experienced falls. Both falls and fear of falls may result in restrictions of physical activity, which in turn have health consequences. To date the relation between (i) falls and (ii) fear of falling with physical activity have not been investigated using objectively measured activity data which permits examination of different intensities of activity and sedentary behaviour.
METHODS: Cross-sectional study of 1680 men aged 71-92 years recruited from primary care practices who were part of an on-going population-based cohort. Men reported falls history in previous 12 months, FOF, health status and demographic characteristics. Men wore a GT3x accelerometer over the hip for 7 days.
RESULTS: Among the 12% of men who had recurrent falls, daily activity levels were lower than among non-fallers; 942 (95% CI 503, 1381) fewer steps/day, 12(95% CI 2, 22) minutes less in light activity, 10(95% CI 5, 15) minutes less in moderate to vigorous PA [MVPA] and 22(95% CI 9, 35) minutes more in sedentary behaviour. 16% (n = 254) of men reported FOF, of whom 52% (n = 133) had fallen in the past year. Physical activity deficits were even greater in the men who reported that they were fearful of falling than in men who had fallen. Men who were fearful of falling took 1766(95% CI 1391, 2142) fewer steps/day than men who were not fearful, and spent 27(95% CI 18, 36) minutes less in light PA, 18(95% CI 13, 22) minutes less in MVPA, and 45(95% CI 34, 56) minutes more in sedentary behaviour. The significant differences in activity levels between (i) fallers and non-fallers and (ii) men who were fearful of falling or not fearful, were mediated by similar variables; lower exercise self-efficacy, fewer excursions from home and more mobility difficulties.
CONCLUSIONS: Falls and in particular fear of falling are important barriers to older people gaining health benefits of walking and MVPA. Future studies should assess the longitudinal associations between falls and physical activity
Feasibility of a cognitive behavioural group intervention to reduce fear of falling and associated avoidance of activity in community-living older people: a process evaluation
BACKGROUND: Fear of falling and associated avoidance of activity are common among older people and may have negative consequences in terms of functional decline, quality of life and institutionalisation. We evaluated the effects of a cognitive behavioural group intervention to reduce fear of falling and associated avoidance of activity among older persons. This intervention showed favourable effects on fear of falling, avoidance of activity, daily activity, and several secondary outcomes. The aim of the present study is to assess the feasibility of this cognitive behavioural group intervention for participants and facilitators. METHODS: The intervention consisted of eight weekly group sessions lasting two hours each and a booster session after six months. Self-administered questionnaires, registration forms and interviews were used to collect data from participants (n = 168) and facilitators (n = 6) on the extent to which the intervention was performed according to protocol, participant attendance, participant adherence, and participants' and facilitators' opinion of the intervention. Quantitative data from the questionnaires and registration forms were analysed by means of descriptive statistics. Qualitative data were categorised based on matching contents of the answers. RESULTS: Facilitators reported no major protocol deviations. Twenty-six percent of the participants withdrew before the start of the programme. Of the persons who started the programme, 84% actually completed it. The participants reported their adherence as good, but facilitators had a less favourable opinion of this. The majority of participants still reported substantial benefits from the programme after six and twelve months of follow-up (71% and 61% respectively). Both participants and facilitators provided suggestions for improvement of the intervention. CONCLUSION: Results of this study show that the current cognitive behavioural group intervention is feasible for both participants and facilitators and fits in well with regular care. Minor refinement of the intervention, however, is warranted to further improve intervention effectiveness and efficiency. Based on these positive findings, we recommend implementing a refined version of this effective and feasible intervention in regular care. TRIAL REGISTRATION: ISRCTN43792817

EFICÁCIA DA INTERVENÇÃO FISIOTERAPÊUTICA ASSOCIADA OU NÃO À RESPIRAÇÃO POR PRESSÃO POSITIVA INTERMITENTE (RPPI) APÓS CIRURGIA CARDÍACA COM CIRCULAÇÃO EXTRACORPÓREA
Este artigo objetivou avaliar as alterações na função pulmonar (FP) e força muscular respiratória (FMR) e eficácia de dois protocolos distintos em pacientes submetidos à cirurgia cardíaca (CC) com circulação extracorpórea (CEC). Método: 21 pacientes foram divididos em: GPPI (n=8), realizaram exercícios respiratórios com RPPI, associados à intervenção fisioterapêutica (IF); e GIF (n=13) realizaram somente IF. A FP foi avaliada pela espirometria no pré e 5º pós-operatório (PO) e a FMR pelas pressões respiratórias máximas (PImax e PEmax) no pré, 1º e 5º PO. O teste de Wilcoxon, Friedman e Man-Whitney foram utilizados para comparações intra e intergrupos, respectivamente. Resultados: Não foram observadas diferenças significativas nos parâmetros antropométricos, aspectos clínicos e cirúrgicos entre os grupos. Foram encontrados valores significantemente menores no 5º PO em comparação à situação pré-operatória no GIF para a capacidade vital (CV), capacidade vital forçada (CVF), volume expiratório forçado no 1º segundo (VEF1), pico de fluxo (PF) e fluxo expiratório forçado (FEF 25-75%.). No entanto, para GPPI apenas o VEF1 permaneceu significativamente reduzido após a intervenção (p<0,05). Para a PImax e PEmax observou-se redução do pré-operatório para 1° PO em ambos os grupos. Porém, apenas a PEmax atingiu valores próximos aos dos pré-operatório até o 5ºPO em ambos os grupos, enquanto a PImax permaneceu significativamente reduzida no GIF. Na análise intergrupos não foram observadas diferenças significativas para nenhuma das variáveis analisadas. Conclusões: Concluímos que pacientes submetidos à CC com CEC sofrem prejuízos na FP e FMR, e que nenhum dos tratamentos aplicados (IF ou IF+RPPI) mostrou significante superioridade com relação ao outro

Expanding the knowledge about Leishmania species in wild mammals and dogs in the Brazilian savannah
Background: Wild, synanthropic and domestic mammals act as hosts and/or reservoirs of several Leishmania spp. Studies on possible reservoirs of Leishmania in different areas are fundamental to understand host-parasite interactions and develop strategies for the surveillance and control of leishmaniasis. In the present study, we evaluated the Leishmania spp. occurrence in mammals in two conservation units and their surroundings in Brasília, Federal District (FD), Brazil. Methods: Small mammals were captured in Brasília National Park (BNP) and Contagem Biological Reserve (CBR) and
dogs were sampled in residential areas in their vicinity. Skin and blood samples were evaluated by PCR using different molecular markers (D7 24Sα rRNA and rDNA ITS1). Leishmania species were identified by sequencing of PCR products. Dog blood samples were subjected to the rapid immunochromatographic test (DPP) for detection of anti-Leishmania infantum antibodies. Results: 179 wild mammals were studied and 20.1% had Leishmania DNA successfully detected in at least one sample. Six mammal species were considered infected: Clyomys laticeps, Necromys lasiurus, Nectomys rattus, Rhipidomys macrurus, Didelphis albiventris and Gracilinanus agilis. No significant difference, comparing the proportion of individuals with Leishmania spp., was observed between the sampled areas and wild mammal species. Most of the positive samples were collected from the rodent N. lasiurus, infected by L. amazonensis or L. braziliensis. Moreover, infections by Trypanosoma spp. were detected in N. lasiurus and G. agilis. All 19 dog samples were positive by DPP; however, only three (15.8%) were confirmed by PCR assays. DNA sequences of ITS1 dog amplicons showed 100% identity with L. infantum sequence. Conclusions: The results suggest the participation of six species of wild mammals in the enzootic transmission of Leishmania spp. in FD. This is the first report of L. amazonensis in N. lasiurus
Reduction in hospitalised COPD exacerbations during COVID-19: A systematic review and meta-analysis
BACKGROUND:
Reports have suggested a reduction in exacerbations of chronic obstructive pulmonary disease (COPD) during the coronavirus disease 2019 (COVID-19) pandemic, particularly hospital admissions for severe exacerbations. However, the magnitude of this reduction varies between studies.
METHODS:
Electronic databases were searched from January 2020 to May 2021. Two independent reviewers screened titles and abstracts and, when necessary, full text to determine if studies met inclusion criteria. A modified version of the Newcastle-Ottawa Scale was used to assess study quality. A narrative summary of eligible studies was synthesised, and meta-analysis was conducted using a random effect model to pool the rate ratio and 95% confidence intervals (95% CI) for hospital admissions. Exacerbation reduction was compared against the COVID-19 Containment and Health Index.
RESULTS:
A total of 13 of 745 studies met the inclusion criteria and were included in this review, with data from nine countries. Nine studies could be included in the meta-analysis. The pooled rate ratio of hospital admissions for COPD exacerbations during the pandemic period was 0.50 (95% CI 0.44–0.57). Findings on the rate of community-treated exacerbations were inconclusive. Three studies reported a significant decrease in the incidence of respiratory viral infections compared with the pre-pandemic period. There was not a significant relationship between exacerbation reduction and the COVID-19 Containment and Health Index (rho = 0.20, p = 0.53).
CONCLUSION:
There was a 50% reduction in admissions for COPD exacerbations during the COVID-19 pandemic period compared to pre-pandemic times, likely associated with a reduction in respiratory viral infections that trigger exacerbations. Future guidelines should consider including recommendations on respiratory virus infection control measures to reduce the burden of COPD exacerbations beyond the pandemic period
Potential impact of invasive alien species on ecosystem services provided by a tropical forested ecosystem: a case study from Montserrat
Local stakeholders at the important but
vulnerable Centre Hills on Montserrat consider that
the continued presence of feral livestock (particularly
goats and pigs) may lead to widespread replacement of
the reserve’s native vegetation by invasive alien trees
(Java plum and guava), and consequent negative
impacts on native animal species. Since 2009, a
hunting programme to control the feral livestock has
been in operation. However long-term funding is not
assured. Here, we estimate the effect of feral livestock
control on ecosystem services provided by the forest to
evaluate whether the biodiversity conservation rationale
for continuation of the control programme is
supported by an economic case. A new practical tool
(Toolkit for Ecosystem Service Site-based Assessment)
was employed to measure and compare ecosystem
service provision between two states of the
reserve (i.e. presence and absence of feral livestock
control) to estimate the net consequences of the
hunting programme on ecosystem services provided
by the forest. Based on this we estimate that cessation
of feral livestock management would substantially
reduce the net benefits provided by the site, including a
46 % reduction in nature-based tourism (from
228,000) and 36 % reduction in harvested
wild meat (from 132,000). The
overall net benefit generated from annual ecosystem
service flows associated with livestock control in thereserve, minus the management cost, was $214,000
per year. We conclude that continued feral livestock
control is important for maintaining the current level
of ecosystem services provided by the reserve
Don’t forget the porpoise: acoustic monitoring reveals fine scale temporal variation between bottlenose dolphin and harbour porpoise in Cardigan Bay SAC
Populations of bottlenose dolphin and harbour porpoise inhabit Cardigan Bay, which was designated a Special Area of Conservation (SAC), with bottlenose dolphin listed as a primary feature for its conservation status. Understanding the abundance, distribution and habitat use of species is fundamental for conservation and the implementation of management. Bottlenose dolphin and harbour porpoise usage of feeding sites within Cardigan Bay SAC was examined using passive acoustic monitoring. Acoustic detections recorded with calibrated T-PODs (acoustic data loggers) indicated harbour porpoise to be present year round and in greater relative abundance than bottlenose dolphin. Fine-scale temporal partitioning between the species occurred at three levels: (1) seasonal differences, consistent between years, with porpoise detections peaking in winter months and dolphin detections in summer months; (2) diel variation, consistent across sites, seasons and years, with porpoise detections highest at night and dolphin detections highest shortly after sunrise; and (3) tidal variation was observed with peak dolphin detections occurring during ebb at the middle of the tidal cycle and before low tide, whereas harbour porpoise detections were highest at slack water, during and after high water with a secondary peak recorded during and after low water. General Additive Models (GAMs) were applied to better understand the effects of each covariate. The reported abundance and distribution of the two species, along with the temporal variation observed, have implications for the design and management of protected areas. Currently, in the UK, no SACs have been formally designated for harbour porpoise while three exist for bottlenose dolphins.
