Encephalopathy with behavioral and psychiatric features–firstantibody proven case of Anti-NMDA receptor encephalitis from Pakistan
Background: Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is associated with psychiatric symptoms, memory disturbances, seizures, dyskinesia, and catatonia along with other constellation of symptoms. It is more common in females and usually associated with underlying ovarian teratoma. Treatment involves immunosuppression. This is the first antibody proven case from Pakistan.
Case Report: A 16 year old girl with a short history of sore throat and no other prior illness, presented with 12 day history of behavioral changes (can you please describe some behavioral changes?) and seizures, followed by decreased responsiveness, involuntary limb movements and facial grimacing. She also had intermittent high grade fever for 3 days. On examination she had spontaneous eye opening with continuous involuntary oro buccal and lingual movements. CT/MRI brain was normal. Spinal tap showed increased WBCs with lymphocytic predominance. EEG showed severe diffuse encephalopathy with no clear epileptiform discharges. HSV PCR came out to be negative. She was treated with Ceftriaxone, Acyclovir, Ampicillin, Vancomycin, Phenytoin and Levetiracetam. Acyclovir was later stopped after negative HSV PCR results. On suspicion of non infectious (autoimmune) encephalitis, treatment with IVIg was started and serum anti NMDA receptor antibodies were sent, which came out to be positive. Ultrasound pelvis showed no teratoma. Parents on follow up reported gradual resolution of symptoms and resumption of usual activities.
Conclusion: Anti NMDA receptor encephalitis is associated with anti NMDA receptor antibodies is responsive to autoimmune treatment
Simplified Abrasion Test Methodology for Candidate EVA Glove Lay-Ups
During the Apollo Program, space suit outerlayer fabrics were badly abraded after performing just a few extravehicular activities (EVAs). For example, the Apollo 12 commander reported abrasive wear on the boots that penetrated the outerlayer fabric into the thermal protection layers after less than 8 hrs of surface operations. Current plans for the exploration planetary space suits require the space suits to support hundreds of hours of EVA on a lunar or Martian surface, creating a challenge for space suit designers to utilize materials advances made over the last 40 years and improve on the space suit fabrics used in the Apollo Program. Over the past 25 years the NASA Johnson Space Center Crew and Thermal Systems Division has focused on tumble testing as means of simulating wear on the outer layer of the space suit fabric. Most recently, in 2009, testing was performed on 4 different candidate outer layers to gather baseline data for future use in design of planetary space suit outer layers. In support of the High Performance EVA Glove Element of the Next Generation Life Support Project, testing a new configuration was recently attempted in which require 10% of the fabric per replicate of that need in 2009. The smaller fabric samples allowed for reduced per sample cost and flexibility to test small samples from manufacturers without the overhead to have a production run completed. Data collected from this iteration was compared to that taken in 2009 to validate the new test method. In addition the method also evaluated the fabrics and fabric layups used in a prototype thermal micrometeoroid garment (TMG) developed for EVA gloves under the NASA High Performance EVA Glove Project. This paper provides a review of previous abrasion studies on space suit fabrics, details methodologies used for abrasion testing in this particular study, results of the validation study, and results of the TMG testing
Observation of Complex Bound States in the Spin-1/2 Heisenberg XXZ Chain Using Local Quantum Quenches
We consider the nonequilibrium evolution in the spin-1/2 XXZ Heisenberg chain for fixed magnetization after a local quantum quench. This model is equivalent to interacting spinless fermions. Initially an infinite magnetic field is applied to n consecutive sites and the ground state is calculated. At time t=0 the field is switched off and the time evolution of observables such as the z component of spin is computed using the time evolving block decimation algorithm. We find that the observables exhibit strong signatures of linearly propagating spinon and bound state excitations. These persist even when integrability-breaking perturbations are included. Since bound states ("strings") are notoriously difficult to observe using conventional probes such as inelastic neutron scattering, we conclude that local quantum quenches are an ideal setting for studying their properties. We comment on implications of our results for cold atom experiments
Family life cycle disruption in rural communities: The case of the Lake Shelbyville reservoir
This project assessed on an ex post facto basis selected impacts upon families and individuals due to the land acquisition program associated with the construction of the Shelbyville Reservoir in east-central Illinois begun in 1962. Research focused on Okaw Township, which borders Lake Shelbyville to the west in Shelby County. The in-depth, individual and family case studies indicated that farm family businesses are not the products of a single generation, or a single family. They are intimately tied to the intergenerational land transfer process which occurs among interconnected families living in close proximity. Thus, the time perspective in examining changes brought about by land acquisition and relocation must be extended on the generational level if the full range of impacts on the local cultural ecology is to be represented. Areawide changes in the availability and price of land made the difficult task of reestablishing a family farm even harder for selected families. The Corps' land acquisition policies in 1962 did not consider the generations involved in the establishment of family farms. Furthermore, local residents were critical of the Corps' policies and personnel during the land acquisition process. That historical complaint summarized by the quote, "the Corps is not a good neighbor," has even today hindered the Corps' ability to manage its relations with area residents and local government units. An understanding of the personal and family lifecycle perspectives on local cultural ecology, coupled with an appreciation of the degree to which rural social processes are bound in time, can strengthen State of Illinois comprehensive planning, decision-making, and review procedures for water and related resource development.U.S. Department of the InteriorU.S. Geological SurveyOpe
Carcinoma ex pleomorphic adenoma in a minor salivary gland: report of a case
Background: Carcinoma ex pleomorphic adenoma is exceedingly rare in minor salivary glands of the oral cavity. We present a case of carcinoma ex pleomorphic adenoma (CEPA) of the buccal mucosa in a 47-year-old Turkish patient. The buccal mass was of a size of 1.5cm located in the left cheek. Pleomorphic adenoma was the tentative diagnosis. Methods: The tumor was removed under local anesthesia. Histopathologic evaluation revealed a preexisting pleomorphic adenoma associated with adenoid tumor component with tubulo-cystic and papillary or pseudopapillary structures; CEPA was diagnosed. Capsular integrity was incomplete with infiltration by islands of metaplastic/dysplastic epithelium. Results: Secondary surgery of the site was performed. No tumor tissue could be detected in the resection specimen. The patient is free of recurrence since 9month
Shear History Extensional Rheology Experiment II (SHERE II) Microgravity Rheology with Non-Newtonian Polymeric Fluids
The primary objective of SHERE II is to study the effect of torsional preshear on the subsequent extensional behavior of filled viscoelastic suspensions. Microgravity environment eliminates gravitational sagging that makes Earth-based experiments of extensional rheology challenging. Experiments may serve as an idealized model system to study the properties of lunar regolith-polymeric binder based construction materials.
