    Five-year follow-up of angiographic disease progression after medicine, angioplasty, or surgery

    <p>Abstract</p> <p>Background</p> <p>Progression of atherosclerosis in coronary artery disease is observed through consecutive angiograms. Prognosis of this progression in patients randomized to different treatments has not been established. This study compared progression of coronary artery disease in native coronary arteries in patients undergoing surgery, angioplasty, or medical treatment.</p> <p>Methods</p> <p>Patients (611) with stable multivessel coronary artery disease and preserved ventricular function were randomly assigned to CABG, PCI, or medical treatment alone (MT). After 5-year follow-up, 392 patients (64%) underwent new angiography. Progression was considered a new stenosis of ≥ 50% in an arterial segment previously considered normal or an increased grade of previous stenosis > 20% in nontreated vessels.</p> <p>Results</p> <p>Of the 392 patients, 136 underwent CABG, 146 PCI, and 110 MT. Baseline characteristics were similar among treatment groups, except for more smokers and statin users in the MT group, more hypertensives and lower LDL-cholesterol levels in the CABG group, and more angina in the PCI group at study entry. Analysis showed greater progression in at least one native vessel in PCI patients (84%) compared with CABG (57%) and MT (74%) patients (p < 0.001). LAD coronary territory had higher progression compared with LCX and RCA (P < 0.001). PCI treatment, hypertension, male sex, and previous MI were independent risk factors for progression. No statistical difference existed between coronary events and the development of progression.</p> <p>Conclusion</p> <p>The angioplasty treatment conferred greater progression in native coronary arteries, especially in the left anterior descending territories and treated vessels. The progression was independently associated with hypertension, male sex, and previous myocardial infarction.</p

    Relações cineantropométricas e fisiológicas durante exercício incremental em esteira rolante

    Testes incrementais em esteira rolante são muito utilizados para prescrição de treinamentos, havendo alterações fisiológicas e cinemáticas devido à natureza de incremento da intensidade do exercício. Contudo, a antropometria é um fator que não apresenta consenso na literatura. Neste sentido, os objetivos do presente estudo foram verificar o efeito de exercício incremental na concentração de lactato sanguíneo ([LAC]), e glicose ([GLI]), como também na frequência (FP) e amplitude de passadas (AP). Além disso, correlacionar valores de dados antropométricos (massa, altura, % de gordura) com as alterações existentes das [LAC] e [GLI]. Treze voluntários saudáveis realizaram teste incremental em esteira rolante (início a 8km/h, com incrementos de 1km/h a cada 3min até a exaustão). Antes e 3min após a realização do teste incremental foram registrados os valores de [LAC] e [GLI]. Durante cada intensidade do teste incremental foram realizadas avaliações da AP e FP por meio de filmagens no plano sagital. Como resultados, verificou-se que a [LAC] e [GLI] antes do teste incremental (1,94 ± 0,4mmol/L e 94,3 ± 7mg/dL, respectivamente) foram significantemente menores que os valores encontrados após o teste incremental (9,51 ± 2,7mmol/L e 126 ± 16mg/dL, respectivamente). Além disso, verificou-se aumento significante e gradativo da AP e FP ao longo do teste incremental. Physiological and kinematics parameters are changed by the progressive nature of this activity. However, there is no consensus on how anthropometric characteristics may affect the incremental test performance. Therefore, the aims of this study were (1) to verify the effects of the incremental treadmill test on the blood lactate concentration ([LAC]), glucose concentration ([GLU]), step frequency (SF) and step length (SL), and (2) to correlate anthropometric data (body mass, height, %body fat) to changes in [LAC] and [GLU]. Thirteen healthy males volunteered to this study and performed an incremental running test on a treadmill (start: 8 km/h, with progressive increases [1 km/h] at each three minute until exhaustion). [LAC] and [GLU] were registered before and 3 min after the incremental test performance. SF and SL were registered for each running speed by digital images at the sagital plane. As results, we verified that [LAC] and [GLU] before the incremental test (1.94 ± 0.4 mmol/L and 94.3 ± 7 mg/dL, respectively) were significantly lower (p < 0.05) than after the incremental test (9.51 ± 2.7 mmol/L and 126 ± 16. mg/dL, respectively). In addition, SF and SL presented significant increase (p < 0.05) throughout the incremental test. [LAC] presented inverse and significant correlations (p < 0.05) to height and body mass. Thus, an incremental treadmill running test affects kinematic (SF and SL) and physiological conditions (muscular fatigue, as consequence of the speed increase). Besides this, shorter and lighter subjects may present higher metabolite concentrations after this type of test

    Saúde Bucal autorreferida da população adulta brasileira: resultados da Pesquisa Nacional de Saúde 2013

    Resumo Os agravos relacionados à saúde bucal dos brasileiros representam importante problema de saúde pública devido à sua prevalência e magnitude na população. Inquéritos de base populacional são ferramentas essenciais para a eleição de grupos e demandas prioritários de atenção. O objetivo deste artigo foi descrever a situação de saúde bucal autorreferida da população brasileira, segundo fatores sociodemográficos. Realizou-se um estudo descritivo, utilizando os dados da Pesquisa Nacional de Saúde de 2013. Indicadores referentes às práticas de higiene e condições de saúde bucal foram analisados. Homens, idosos, pretos e pardos, indivíduos sem instrução e com nível fundamental incompleto, residentes na área rural e na região nordeste apresentaram as mais baixas frequências dos indicadores de higiene bucal adequada e de autopercepção da saúde bucal como boa ou muito boa. Com relação à perda dentária, a total afetou cerca de 16,0 milhões. Tanto a perda dentária total quanto a de 13 ou mais dentes foi mais frequente em mulheres, indivíduos com 60 anos e mais de idade, com baixa escolaridade, residentes na área rural. Conclui-se que os dados da Pesquisa Nacional de Saúde – PNS 2013 reforçam a necessidade de políticas e ações que minimizem as desigualdades em saúde bucal, garantindo acesso aos subgrupos populacionais mais vulneráveis de acordo com suas necessidades de saúde

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI 2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesity.     Diminishing benefits of urban living for children and adolescents’ growth and development

    AbstractOptimal growth and development in childhood and adolescence is crucial for lifelong health and well-being1–6. Here we used data from 2,325 population-based studies, with measurements of height and weight from 71 million participants, to report the height and body-mass index (BMI) of children and adolescents aged 5–19 years on the basis of rural and urban place of residence in 200 countries and territories from 1990 to 2020. In 1990, children and adolescents residing in cities were taller than their rural counterparts in all but a few high-income countries. By 2020, the urban height advantage became smaller in most countries, and in many high-income western countries it reversed into a small urban-based disadvantage. The exception was for boys in most countries in sub-Saharan Africa and in some countries in Oceania, south Asia and the region of central Asia, Middle East and north Africa. In these countries, successive cohorts of boys from rural places either did not gain height or possibly became shorter, and hence fell further behind their urban peers. The difference between the age-standardized mean BMI of children in urban and rural areas was &lt;1.1 kg m–2 in the vast majority of countries. Within this small range, BMI increased slightly more in cities than in rural areas, except in south Asia, sub-Saharan Africa and some countries in central and eastern Europe. Our results show that in much of the world, the growth and developmental advantages of living in cities have diminished in the twenty-first century, whereas in much of sub-Saharan Africa they have amplified.</jats:p

    Immunosuppression use in primary antiphospholipid antibody-positive patients: Descriptive analysis of the AntiPhospholipid Syndrome Alliance for Clinical Trials and InternatiOnal Networking (APS ACTION) Clinical Database and Repository ("Registry")

    Background/Purpose: APS ACTION Registry was created to study the outcomes of patients with persistently positive antiphospholipid antibodies (aPL) with or without other systemic autoimmune disease (SAIDx). Given that immunosuppression (IS) is used for certain aPL manifestations, for example, thrombocytopenia (TP), our primary objective was to describe the indications for IS in aPL-positive patients without other SAIDx. Secondly, we report the type of IS used in patients with selected microvascular or non-thrombotic aPL manifestations. Methods: An online database is used to collect clinical data. The inclusion criteria are positive aPL based on the laboratory section of the APS Classification Criteria, tested at least twice within one year prior to enrollment. Patients are followed every 12 ± 3 months. For this descriptive retrospective and prospective analysis, we included aPL-positive patients without other SAIDx and excluded those with new SAIDx classification during follow-up. For each patient, we retrieved clinical data at baseline and follow-up including selected aPL manifestations (diffuse alveolar hemorrhage [DAH], antiphospholipidnephropathy [aPL-N], livedoid vasculopathy [LV]-related skin ulcers, TP, autoimmune hemolytic anemia [AIHA], cardiac valve disease [VD]), and IS medications. Results: Of 899 patients enrolled, 537 were included in this analysis (mean age 45 ± 13 years, female 377 [70%], APS Classification in 438 [82%], and at least one selected microvascular or non-thrombotic aPL manifestation in 141 (26%)). Of 537 patients, 76 (14%) were reported to use IS (ever), and 41/76 (54%) received IS primarily for selected aPL manifestation. In six of 8 (75%) DAH patients, 6/19 (32%) aPL-N, 4/28 (14%) LV, 25/88 (28%) TP, 6/11 (55%) AIHA, and 1/43 (2%) VD, the IS (excluding corticosteroids/hydroxychloroquine) indication was specific for selected aPL manifestation. Conclusion: In our international cohort, 14% of aPL-positive patients without other SAIDx were reported to receive IS; the indication was at least one of the selected microvascular and/or non-thrombotic aPL-related manifestations in half. Thrombocytopenia was the most frequent among those selected aPL-related manifestations; however, approximately onethird received IS specifically for that indication. Diffuse alveolar hemorrhage was frequently treated with IS followed by AIHA and aPL-N. Systematic controlled studies are urgently needed to better define the role of IS in APS
