A retrospective cross-sectional study: Fresh cycle endometrial thickness is a sensitive predictor of inadequate endometrial thickness in frozen embryo transfer cycles

BACKGROUND: The purpose of this study is to assess predictors of inadequate endometrial cavity thickness (ECT), defined as < 8 mm, in frozen embryo transfer (FET) cycles. METHODS: This is a retrospective cross-sectional study at an academic fertility center including 274 women who underwent their first endometrial preparation with estradiol for autologous FET in our center from 2001-2009. Multivariable logistic regression was performed to determine predictors of inadequate endometrial development in FET cycles. RESULTS: Neither age nor duration of estrogen supplementation were associated with FET endometrial thickness. Lower body mass index, nulliparity, previous operative hysteroscopy and thinner fresh cycle endometrial lining were associated with inadequate endometrial thickness in FET cycles. A maximum thickness of 11.5 mm in a fresh cycle was 80% sensitive and 70% specific for inadequate frozen cycle thickness. CONCLUSIONS: Previous fresh cycle endometrial cavity thickness is associated with subsequent FET cycle endometrial cavity thickness. Women with a fresh cycle thickness of 11.5 mm or less may require additional intervention to achieve adequate endometrial thickness in preparation for a frozen cycle

Doctoral‐Level Counseling Students’ Experiences of Social Class Microaggressions

The authors recruited 11 doctoral‐level counseling students to participate in a study exploring the lived experiences of people who have encountered social class microaggressions (SCMs). Findings (consisting of 6 themes) suggest that SCMs are a distinct phenomenon arising from interpersonal and environmental exchanges that damage recipients. Variability in the practice of fertility preservation for patients with cancer

Fertility is important to women and men with cancer. While options for fertility preservation (FP) are available, knowledge regarding the medical application of FP is lacking. Therefore we examined FP practices for cancer patients among reproductive endocrinologists (REs). A 36 item survey was sent to board-certified REs. 98% of respondents reported counseling women with cancer about FP options. Oocyte and embryo cryopreservation were universally offered by these providers, but variability was noted in reported management of these cases-particularly for women with breast cancer. 86% of the respondents reported using letrozole during controlled ovarian stimulation (COS) in patients with estrogen receptor positive (ER+) breast cancer to minimize patient exposure to estrogen. 49% of respondents who reported using letrozole in COS for patients with ER+ breast cancer reported that they would also use letrozole in COS for women with ER negative breast cancer. Variability was also noted in the management of FP for men with cancer. 83% of participants reported counseling men about sperm banking with 22% recommending against banking for men previously exposed to chemotherapy. Overall, 79% of respondents reported knowledge of American Society for Clinical Oncology FP guidelines-knowledge that was associated with providers offering gonadal tissue cryopreservation (RR 1.82, 95% CI 1.14-2.90). These findings demonstrate that RE management of FP in cancer patients varies. Although some variability may be dictated by local resources, standardization of FP practices and communication with treating oncologists may help ensure consistent recommendations and outcomes for patients seeking FP

The Pursuit of Work-Life Balance by High School Sport Practitioners in the Athletic Director-Head Coach Dual Role

With wide-reaching benefits afforded student-athletes through their participation and communities investing time and resources into the promotion of meaningful athletic programs, leaders within a high school athletic department face substantial expectations within their role. For those who decide to embrace a dual role of athletic director and head coach, the increased responsibilities facilitate greater levels of role conflict and role overload that can negatively impact the ability to achieve work-life balance within one’s personal life if not properly addressed. Therefore, the purpose of this research was to examine the strategies employed by these high school sport dual role practitioners for pursuing work-life balance. Semi-structured interviews were conducted with 15 participants represented across school classification (i.e., 1A, 2A, 3A, and 4A) and sport coached, and a thematic analysis yielded three main themes (i.e., recognizing the challenge, setting boundaries, and incorporating family into work) that detail the efforts implemented for work-life balance. From a theoretical perspective, findings add further insight into the impact of role conflict and role overload on work-life balance, especially in the context of high school athletics, while practitioners in the athletic director-head coach dual role can use the perspectives shared by participants to ensure they are effectively balancing their professional and personal life amidst these role demands

A Segregação racial em Goiânia: representação dos dados de cor ou raça (IBGE, 2010)

ResumoO presente trabalho consiste em uma abordagem geográfica e uma representação cartográfica dos processos de segregação e de diferenciação socioespacial combinados com a dimensão racial, em Goiânia, com base nos dados de cor/raça e renda do Censo Demográfico (IBGE, 2010). Em primeiro lugar, trazemos leituras do campo da Geografia das relações raciais na sociedade brasileira, voltadas para o espaço urbano, com foco nos referidos processos. Em seguida, tratamos da construção de uma cartografia racial e das questões metodológicas da pesquisa. Discutimos vários estudos realizados acerca da segregação social na capital goiana e elaboramos vários mapas de “espaços de maioria branca” e “espaços de maioria negra”, associados com a identificação de cor ou raça e os níveis de classe, distribuídos por bairros da cidade. Por fim concluímos que os processos em pauta têm uma estreita correlação com a diferença e com a desigualdade racial na cidade, fenômeno que acontece em outras metrópoles brasileiras.Palavras-chave: Diferenciação socioespacial, Segregação socioespacial, Relações raciais, Cartografia racial. AbstractThis paper consists of a geographical approach and a cartographic representation of the processes of segregation and socio-spatial differentiation combined with the racial dimension in Goiania based on color data / race and income of the Census Brazilian Institute of Geography and Statistics (IBGE, 2010). At first, we bring readings from the field of Geography of race relations in Brazilian society, facing urban space, focusing on those cases. Then we discuss about the construction of a racial mapping and methodological research questions. We brought several studies on the social segregation in Goias and prepared capital several maps of "mostly white spaces" and "spaces of black majority", associated with the identification of color or race and class levels spread across city neighborhoods. Finally we conclude that the processes under discussion  have a close correlation with the difference and the racial inequality in the city, a phenomenon that happens in other Brazilian cities.Keywords: socio-spatial differentiation, socio-spatial segregation, race relations, racial cartography. Resumen El presente trabajo consiste en un enfoque geográfico y una representación cartográfica de los procesos de segregación y de diferenciación socioespacial combinados con la dimensión racial, en Goiânia, basado en datos de color/raza y renta del Censo Demográfico (IBGE, 2010).En primer lugar, traemos lecturas del campo de la Geografía de las relaciones raciales en la sociedad brasileña, direccionadas para el espacio urbano, con foco en los referidos procesos. Streptococcus Pneumoniae Intracranial Abscess and Post-Infectious Vasculitis

Intracranial abscesses are rare complications of Streptococcus pneumoniae infections, and to our knowledge, there have been no case reports of post-infectious vasculitis developing in such patients. Here we describe the case of a 48-year-old post-splenectomy male who developed post-infectious vasculitis following S. pneumoniae otitis media complicated by mastoiditis, osteomyelitis, meningitis, and intracranial abscess. Clinicians ought to be aware of the possible adverse outcomes of invasive S. pneumoniae and the limitations of current treatment options
