    Using Constraint Logic Programming in Memory Synthesis for General Purpose

    In this paper, we extend the set of library components which are usually considered in architectural synthesis by components with built-in chaining. For such components, the result of some internally computed arithmetic function is made available as an argument to some other function through a local connection. These components can be used to implement chaining in a data-path in a single component. Components with built-in chaining are combinatorial circuits. They correspond to ``complex gates in logic synthesis. If compared to implementations with several components, components with built-in chaining usually provide a denser layout, reduced power consumption, and a shorter delay time. Multiplier/accumulators are the most prominent example of such components. Such components require new approaches for library mapping in architectural synthesis.     Significant increase of patient information and satisfaction with longer initial consultation duration in breast cancer - first results of the WAVES study

    Aim of the study: The "WAVES" study (Widening Aims and giving patients a Voice for Expanded Structures in breast cancer care developed jointly by patients and physicians) aims to illuminate current breast cancer care structures with special focus on physician-patient-communication. Methods: The study is conducted within and funded by the BZKF (Bavarian Center for Cancer Research). Here, we present the results of the first preplanned analysis of the survey designed together with patients and patient advocates with the aim of adequately reflecting patients concerns. It is based on the evaluation of the first 1.000 patients who participated between 05/2022 and 08/2023, focusing on the duration of the first diagnosis consultation. Results: The participants were between 23 and 89 years old (mean: 59.18 years). There was a significant association between longer initial consultation duration and higher patient satisfaction (p < 0.001). When the first consultation lasted 30 minutes or more, patients stated more frequently that they felt better informed (p < 0.001) and had fully or substantially understood the content (p < 0.001). Conclusion: These results demonstrate a significantly higher satisfaction and better preparation of patients with initial breast cancer diagnosis if physicians’ communication lasted 30 minutes or more. Therefore the WAVES study clearly demonstrates the need for improved communication structures in terms of an appropriate time frame for breast cancer patients, which is not reflected in the current reimbursement structures

    Causal effect of plasminogen activator inhibitor type 1 on coronary heart disease

    Background--Plasminogen activator inhibitor type 1 (PAI-1) plays an essential role in the fibrinolysis system and thrombosis. Population studies have reported that blood PAI-1 levels are associated with increased risk of coronary heart disease (CHD). However, it is unclear whether the association reflects a causal influence of PAI-1 on CHD risk. Methods and Results--To evaluate the association between PAI-1 and CHD, we applied a 3-step strategy. First, we investigated the observational association between PAI-1 and CHD incidence using a systematic review based on a literature search for PAI-1 and CHD studies. Second, we explored the causal association between PAI-1 and CHD using a Mendelian randomization approach using summary statistics from large genome-wide association studies. Finally, we explored the causal effect of PAI-1 on cardiovascular risk factors including metabolic and subclinical atherosclerosis measures. In the systematic meta-analysis, the highest quantile of blood PAI-1 level was associated with higher CHD risk comparing with the lowest quantile (odds ratio=2.17; 95% CI: 1.53, 3.07) in an age- and sex-adjusted model. The effect size was reduced in studies using a multivariable-adjusted model (odds ratio=1.46; 95% CI: 1.13, 1.88). The Mendelian randomization analyses suggested a causal effect of increased PAI-1 level on CHD risk (odds ratio=1.22 per unit increase of log-transformed PAI-1; 95% CI: 1.01, 1.47). In addition, we also detected a causal effect of PAI-1 on elevating blood glucose and high-density lipoprotein cholesterol. Conclusions--Our study indicates a causal effect of elevated PAI-1 level on CHD risk, which may be mediated by glucose dysfunction

    Genetic associations at 53 loci highlight cell types and biological pathways relevant for kidney function.

    Reduced glomerular filtration rate defines chronic kidney disease and is associated with cardiovascular and all-cause mortality. We conducted a meta-analysis of genome-wide association studies for estimated glomerular filtration rate (eGFR), combining data across 133,413 individuals with replication in up to 42,166 individuals. We identify 24 new and confirm 29 previously identified loci. Of these 53 loci, 19 associate with eGFR among individuals with diabetes. Using bioinformatics, we show that identified genes at eGFR loci are enriched for expression in kidney tissues and in pathways relevant for kidney development and transmembrane transporter activity, kidney structure, and regulation of glucose metabolism. Chromatin state mapping and DNase I hypersensitivity analyses across adult tissues demonstrate preferential mapping of associated variants to regulatory regions in kidney but not extra-renal tissues. These findings suggest that genetic determinants of eGFR are mediated largely through direct effects within the kidney and highlight important cell types and biological pathways

    In modern computer systems the performance is dominated by the memory performance. Currently, there is neither a systematic design methodology nor a tool for the design of memory systems for general purpose computers.We present a first approach to CAD support for this crucial subtask of system level design. Dependencies between influencing factors and design decisions are explicitly represented by constraints, and constraint logic programming is used to make the design decisions. The memory design is optimized with respect to several objectives by iterating the (re)design cycle. Event driven simulation is used for evaluation of the intermediate results. The system is organized as an interactive design assistant

    German Heart Surgery Report 2018: The Annual Updated Registry of the German Society for Thoracic and Cardiovascular Surgery

    AbstractBased on a longtime voluntary registry, founded by the German Society for Thoracic and Cardiovascular Surgery (GSTCVS), well-defined data of all cardiac, thoracic, and vascular surgery procedures performed in 78 German heart surgery departments during the year 2018 are analyzed. For this period, a total of 174,902 procedures were submitted to the registry, 98,707 summarized as heart surgery procedures in the classical meaning. The unadjusted in-hospital survival rate for 33,999 isolated coronary artery bypass grafting procedures (relationship on-/off-pump 3.8:1) was 97.1% and for 34,915 isolated heart valve procedures, 14,396 transcatheter interventions included, it was 96.0%. Concerning short- and long-term circulatory supports, a total of 2,871 extracorporeal life support (ECLS)/extracorporeal membrane oxygenation (ECMO) implants, respectively, 942 assist device implantations (L-/R-/BVAD, TAH) were registered. In 2018, the number of isolated heart transplantations increased to 312, a growth of 23% compared with the previous year. The isolated lung transplantations reached 340, a rise of nearly 19%. This annual updated registry of the GSTCVS represents voluntary public reporting by accumulating actual information for nearly all heart surgical procedures in Germany, constitutes advancements in heart medicine, and is a basis for quality management for all participating institutions. In addition, the registry demonstrates that the provision of cardiac surgery in Germany is appropriate and nationwide patient treatment is guaranteed all the time.</jats:p

In 2019, the number of isolated heart transplantations increased to 333, a rise of 6.7% compared to the previous year. The isolated lung transplantations amounted to 311, a decrease of 8.5%. This annually updated registry of the GSTCVS represents voluntary public reporting by accumulating actual information for nearly all heart surgical procedures in Germany, constitutes advancements in heart medicine, and represents a basis for quality management for all participating institutions. In addition, the registry demonstrates that the provision of cardiac surgery in Germany is up to date, appropriate, and nationwide patient treatment is guaranteed all the time.</jats:p
