    Analyse sérologique de la toxoplasmose pergravidique: évaluation des risques et

    Introduction: La présente étude rapporte les données sérologiques de 306 sérums collectés chez des parturientes au CHU de Bobo Dioulasso et analysés rétrospectivement au CHU de Reims en 2011. Le but était de déterminer le statut sérologique de ces parturientes et d’en déduire la conduite à tenir. Méthodes: La recherche des IgG et des IgM anti toxoplasmiques était systématique. Les techniques d’agglutination haute sensibilisée et celle d’Immunocapture M ont servi à la recherche respective des anticorps spécifiques IgG et des IgM. Résultats: Sur 306 sérums analysés, 95 (31%) avaient des IgG positifs et aucun n’avait des IgM. Deux cent onze (211) sérums (69%) des sérums n’avaient ni IgG, ni IgM. Conclusion: Nos résultats montrent que 31% des femmes en dehors d’une immunodépression sous jacente, possèdent une immunité résiduelle vis à vis de Toxoplasma gondii et n’ont pas la nécessité d’avoir une surveillance sérologique pendant la grossesse. Cependant, 69% (211) des parturientes sont à risque d’une séroconversion, et devraient bénéficier de conseils hygiéno diététiques, associés à une surveillance sérologique durant la grossesse. Ces résultats montrent l’intérêt de mettre en place des mesures de prévention contre la toxoplasmose congénitale, étant l’une des affections materno - foetales les plus fréquentes par la mise en place d’un diagnostic prénatal de la toxoplasmose en routine dans notre hôpital.Pan African Medical Journal 2012; 12:4

    Effect of early tranexamic acid administration on mortality, hysterectomy, and other morbidities in women with post-partum haemorrhage (WOMAN): an international, randomised, double-blind, placebo-controlled trial

    Background Post-partum haemorrhage is the leading cause of maternal death worldwide. Early administration of tranexamic acid reduces deaths due to bleeding in trauma patients. We aimed to assess the effects of early administration of tranexamic acid on death, hysterectomy, and other relevant outcomes in women with post-partum haemorrhage. Methods In this randomised, double-blind, placebo-controlled trial, we recruited women aged 16 years and older with a clinical diagnosis of post-partum haemorrhage after a vaginal birth or caesarean section from 193 hospitals in 21 countries. We randomly assigned women to receive either 1 g intravenous tranexamic acid or matching placebo in addition to usual care. If bleeding continued after 30 min, or stopped and restarted within 24 h of the first dose, a second dose of 1 g of tranexamic acid or placebo could be given. Patients were assigned by selection of a numbered treatment pack from a box containing eight numbered packs that were identical apart from the pack number. Participants, care givers, and those assessing outcomes were masked to allocation. We originally planned to enrol 15 000 women with a composite primary endpoint of death from all-causes or hysterectomy within 42 days of giving birth. However, during the trial it became apparent that the decision to conduct a hysterectomy was often made at the same time as randomisation. Although tranexamic acid could influence the risk of death in these cases, it could not affect the risk of hysterectomy. We therefore increased the sample size from 15 000 to 20 000 women in order to estimate the effect of tranexamic acid on the risk of death from post-partum haemorrhage. All analyses were done on an intention-to-treat basis. This trial is registered with ISRCTN76912190 (Dec 8, 2008); ClinicalTrials.gov, number NCT00872469; and PACTR201007000192283. Findings Between March, 2010, and April, 2016, 20 060 women were enrolled and randomly assigned to receive tranexamic acid (n=10 051) or placebo (n=10 009), of whom 10 036 and 9985, respectively, were included in the analysis. Death due to bleeding was significantly reduced in women given tranexamic acid (155 [1·5%] of 10 036 patients vs 191 [1·9%] of 9985 in the placebo group, risk ratio [RR] 0·81, 95% CI 0·65–1·00; p=0·045), especially in women given treatment within 3 h of giving birth (89 [1·2%] in the tranexamic acid group vs 127 [1·7%] in the placebo group, RR 0·69, 95% CI 0·52–0·91; p=0·008). All other causes of death did not differ significantly by group. Hysterectomy was not reduced with tranexamic acid (358 [3·6%] patients in the tranexamic acid group vs 351 [3·5%] in the placebo group, RR 1·02, 95% CI 0·88–1·07; p=0·84). The composite primary endpoint of death from all causes or hysterectomy was not reduced with tranexamic acid (534 [5·3%] deaths or hysterectomies in the tranexamic acid group vs 546 [5·5%] in the placebo group, RR 0·97, 95% CI 0·87-1·09; p=0·65). Adverse events (including thromboembolic events) did not differ significantly in the tranexamic acid versus placebo group. Interpretation Tranexamic acid reduces death due to bleeding in women with post-partum haemorrhage with no adverse effects. When used as a treatment for postpartum haemorrhage, tranexamic acid should be given as soon as possible after bleeding onset.     Yam Cropping System in Cote d’Ivoire: Current Practices and Constraints

    Yam is the first food crop in Côte d'Ivoire. It is being cultivated under a traditional technique system with low output. Detailed profiling of the current practices and opportunities for improvement is essential to increase the benefits of yam for food security and income in the country. This study diagnosed the current practices, constraints, and requirements in order to gain insight on the yam cropping system for research and development in Côte d'Ivoire. The surveys were conducted in seven (7) regions in the main yam production area. Groups of 60 producers per region were investigated. Individual surveys were also conducted on-farm, particularly for diagnosing the physical environment and yam-based production systems. As results, yambased production systems are characterized by mixed cropping (cassava, corn and vegetable crops) and/or rotation with various other crops (rice, groundnut, vegetable crops, …). Preferred plots for yam cultivation are forest, savannah or fallow areas. The major constraints in yam cultivation were the effects of environmental stresses, labor scarcity, pest damage, tuber and seed storage difficulties as well as the low market value of yam. The most cultivated yam varieties are Bètè-Bètè and Florido for Dioscorea alata and Lokpa for Dioscorea rotundata. The cropping system, the species and varieties vary according to the region, the topo sequence and the type of soil. Knowledgebuilding that take into account constraints related to yam cultivation, for each production area, can serve as guidance for variety breeding and cultivation environment redefinition

    The rediscovery of Hibiscus fragilis DC. on Reunion Island confirms its endemicity on two islands of the Mascarene (Malvaceae).The rediscovery of a population of Hibiscus fragilis on Reunion Island removes the doubt expressed in the Mascarene about the origin of the species and more its type. This finding confirms that this species is endemic to both Reunion and Mauritius Islands. The presence of this species on Reunion Island would rank it as Critically Endangered in the IUCN Red List for this area, but improves its global status to Endangered. We estimated the size of this population, identified the factors threatening the species and suggested some measures to ensure its conservation. Résumé : La découverte d'une population d'Hibiscus fragilis DC. sur l’île de La Réunion permet d'éliminer le doute formulé dans la flore des Mascareignes pour cette espèce et de son type. Cette redécouverte montre que ce taxon est bien endémique des îles de La Réunion et de Maurice. La présence de cette plante à La Réunion la classe pour ce territoire dans la catégories CR, espèce en danger critique d’extinction, mais permet de faire évoluer son statut global sur la liste rouge IUCN vers le niveau EN, espèce en danger d’extinction. Nous avons estimé la taille de la population, mis en exergue les menaces qui pèsent sur cette plante et proposé des mesures afin de la sauvegarder
