    Management and efficacy of intensified insulin therapy starting in outpatients

    Diabetic patients under multiple injection insulin therapy (i.e., intensified insulin therapy, IIT) usually start this treatment during hospitalization. We report here on the logistics, efficacy, and safety of IIT, started in outpatients. Over 8 months, 52 type I and type II diabetics were followed up whose insulin regimens consecutively had been changed from conventional therapy to IIT. Two different IIT strategies were compared: free mixtures of regular and intermediate (12 hrs)-acting insulin versus the basal and prandial insulin treatment with preprandial injections of regular insulin, and ultralente (24 hrs-acting) or intermediate insulin for the basal demand. After 8 months HbA1 levels had decreased from 10.6%±2.4% to 8.0%±1.3% (means±SD). There was no difference between the two regimens with respect to metabolic control; but type II patients maintained the lowered HbA1 levels better than type I patients. Only two patients were hospitalized during the follow-up time because of severe hypoglycemia. An increase of body weight due to the diet liberalization during IIT became a problem in one-third of the patients. Our results suggest that outpatient initiation of IIT is safe and efficacious with respect to near-normoglycemic control. Weight control may become a problem in IIT patients

    Penelitian ini mengupas permasalahan bagaimana problematika keagamaan remaja bidang ibadah salat, apa faktor yang menyebabkan munculnya problematika keagamaan remaja bidang salat, serta apa saja upaya–upaya yang dilakukan dalam penanggulangan problematika keagamaan yang dialami remaja bidang ibadah salat yang berfokus pada usia17-21 tahun di Desa Hutaraja Kecamatan Siabu Berdasarkan permasalahan di atas tujuan penelitian ini untuk mengetahui bagaimana problematika keagamaan remaja dibidang ibadah salat, apa faktor penyebab munculnya problematika keagamaan remaja bidang ibadah salat, serta upaya yang dilakukan dalam menanggulangi problematika keagamaan remaja bidang ibadah salat umur 17-21 tahun di Desa Hutaraja Kecamatan Siabu. Metode yang digunakan yaitu pendekatan analisa kualitatif deskriftip dengan mengggunakan survey lapangan dan instrumen pengumpulan data yang diambil adalah angket, wawancara dan observasi dengan sumber data 10 orang remaja umur 17-21 tahun, 5 orangtua remaja, serta 3 orang tokoh masyarakat/alim ulama. Analisa disusun dengan cara menyusun data secara sistematis dan mendeskripsikannya serta menarik kesimpulan. Dari penelitian yang dilakukan dapat ditemukan hasil bahwa problematika keagamaan remaja bidang ibadah salat yaitu orangtua kurang memberikan bimbingan kepada anak sehingga anak tidak bisa mengerjakan ibadah salat kemudian dapat juga dilihat dari hasil bahwa remaja lebih banyak yang tidak mengerjakan salat daripada yang mengerjakan salat. Faktor yang menyebabkan munculnya problematika keagamaan remaja bidang ibadah salat umur 17-21 Tahun di desa Hutaraja Kecamatan Siabu disebabkan Tingkat pengetahuan agama dan pendidikan orang tua hanya sampai SD dan SMP, sehingga tidak bisa memberikan bimbingan dan mengajari anak tentang tata cara salat, orangtua tidak mempunyai kesempatan menyuruh dan mengajari anak untuk salat dikarenakan sibuk mencari nafkah, kurangnya kesadaran orangtua dalam pengamalan ibadah salat kepada anak yang seharusnya diterapkan mulai sejak anak-anak. Upaya dalam menanggulangi problematika keagamaan remaja yaitu berawal dari diri sendiri baik orang tua dan remaja, dalam artian bahwa harus ada kemauan untuk menjalankan kewajiban. Dan mengikuti kegiatan-kegiatan keagamaan yang ada seperti: pengajian mingguan, untuk orang tua dan remaja. Dan memberikan arahan kepada orang tua agar selalu mendidik anak dengan baik dan mengajarkan kepada anak bagaimana pentingnya salat dalam kehidupan serta melaksanakan nilai-nilai agama yang bai

    Design and Analysis of a Bandwidth Aware Adaptive Multipath N-Channel Routing Protocol for 5G Internet of Things (IoT)

    Large numbers of mobile wireless nodes that can move randomly and join or leave the network at any moment make up mobile ad-hoc networks. A significant number of messages are delivered during information exchange in populated regions because of the Internet of Things' (IoT) exponential increase in connected devices. Congestion can increase transmission latency and packet loss by causing congestion. More network size, increased network traffic, and high mobility that necessitate dynamic topology make this problem worse. An adaptive Multipath Multichannel Energy Efficient (AMMEE) routing strategy is proposed in this study, in which route selection strategies depend on forecasted energy consumption per packet, available bandwidth, queue length, and channel utilization. While multichannel uses a channel-ideal assignment process to lessen network collisions, multipath offers various paths and balances network strain. The link bandwidth is split up into a few sub-channels in the multichannel mechanism. To reduce network collisions, several source nodes simultaneously access the channel bandwidth. The cooperative multipath multichannel technique offers several paths from a single source or from several sources to the destination without colliding or becoming congested. The AMMEE routing approach is the basis for path selection. A load- and bandwidth-aware routing mechanism in the proposed AMMEE chooses the path based on node energy and forecasts their lifetime, which improves network dependability. The outcome demonstrates a comparative analysis of various multichannel medium access control (MMAC) techniques, including Parallel Rendezvous Multi Channel Medium Access Protocol (PRMMAC), Quality of Service Ad hoc On Demand Multipath Distance Vector (QoS-AOMDV), Q-learning-based Multipath Routing (QMR), and Topological Change Adaptive Ad hoc On-demand Multipath Distance Vector (TA-AOMDV) and the proposed AMMEE method.
