    Safety and effectiveness of a new toric presbyopia-correcting posterior chamber silicone intraocular lens.

    PURPOSE: To evaluate the safety and effectiveness of the Trulign toric intraocular lens (IOL) in adults with cataract. SETTING: Eight private practices in the United States and 1 in Canada. DESIGN: Prospective randomized single-masked multicenter study. METHODS: A toric IOL (1.25 D, 2.00 D, or 2.75 D, determined by a toric calculator) was implanted in eligible patients with age-related cataract requiring a 16.00 to 27.00 diopter (D) spherical IOL power and with a predicted postoperative astigmatism of 0.83 to 2.50 D. Eyes within the lowest cylinder range (predicted postoperative astigmatism 0.83 to 1.32 D) were randomized in a 1:1 ratio between the 1.25 D toric IOL group and the nontoric accommodating IOL (Crystalens) control group. RESULTS: The toric 1.25 D group had a statistically significantly greater percentage reduction in absolute cylinder (P < .001) and uncorrected distance visual acuity (P = .002) than the control group at the 120- to 180-day visit. The mean monocular uncorrected vision at distance, intermediate, and near was 20/25, 20/22, and 20/39, respectively, with the 1.25 D, 2.00 D, and 2.75 D toric IOLs in aggregate (toric group). In addition, 96.1% of patients (123/128) had 5.0 degrees or less absolute IOL rotation postoperatively. Regarding safety, the endpoints for preservation of corrected visual acuity and the incidence of complications and adverse events were met. CONCLUSION: The toric IOL was safe and effective in reducing the effects of preoperative corneal astigmatism and provided excellent uncorrected distance and intermediate vision and functional near vision.     Production of Bc mesons in photon-photon and hadron-hadron collisions

    We discuss two-photon and hadronic production of Bc mesons in nonrelativistic bound state approximation and to lowest order in the coupling constants a and a s . It is shown that in photon-photon collisions, heavy quark fragmentation is dominated by recombination of b and c quarks up to the highest accessible transverse momenta. In contrast, in hadroproduction, which at high energies mainly involves gluon-gluon collisions, the fragmentation mechanism dominates at transverse momenta py > mRc, providing a simple and satisfactory approximation of the complete 0(a ) ) results in the high-px regime. Contradictions in previous publications on hadroproduction of Bc mesons are clarified. We also present predictions for cross sections and differential distributions at present and future accelerators

    Discriminating graviton exchange effects from other new physics scenarios in e^+e^- collisions

    We study the possibility of uniquely identifying the effects of graviton exchange from other new physics in high energy e^+e^- annihilation into fermion-pairs.     Análisis de la adecuación de los indicadores de calidad en la terapia de nutrición enteral en una unidad de cuidados intensivos

    Introdução: A terapia nutricional enteral (TNE) é essencial para recuperar ou manter o estado nutricional do paciente. Porém, pode não garantir o alcance das necessidades nutricionais e pode gerar diversas complicações. Visando o melhor atendimento ao paciente, foram criadas ferramentas de qualidade como os indicadores de qualidade em terapia nutricional enteral (IQTNE). Objetivos: Analisar IQTNE em uma unidade de terapia intensiva (UTI). Métodos: Estudo retrospectivo composto por dados coletados durante oito meses, dos prontuários de pacientes sob TNE que estiveram internados em uma UTI de um hospital privado. Os IQTNE analisados foram: (1) frequência de realização de triagem nutricional; (2) frequência de dias de administração adequada de energia; (3) frequência de dias de administração adequada de proteína; (4) frequência de diarreia; (5) frequência de saída inadvertida de sonda de nutrição; (6) frequência de obstrução de sonda de nutrição e (7) taxa de adequação do volume infundido em relação ao prescrito. Resultados: A amostra incluiu dados de 122 pacientes sob TNE com média de idade de 76,9±16,4 anos e 29,9±28,6 dias de internamento. O IQTNE (1) evidenciou média de 97,3±3,1%, o IQTNE (2) 88,1±3,8%, IQTNE (3) 89,8±4,8%, o IQTNE (4) 9,2±6,3%, contudo, obteve não conformidade em 50% dos meses analisados, IQTNE (5) 3,9±2,2%, com inadequação em 25% dos meses analisados, IQTNE (6), 0,8±1,5% e o IQTNE (7) obteve 94,3±3,3% de adequação. As intercorrências observadas influenciaram negativamente a adequação da TNE instituída. Conclusão: Em média, todos os IQTNE analisados atingiram a meta pré-estabelecida, entretanto, atenção deve ser dada quanto à sintomatologia de diarreia e saída inadvertida da sonda de nutrição.Introduction: Enteral nutritional therapy (ERT) is essential to restore or maintain the patient's nutritional status. However, it may not guarantee the reach of nutritional needs and may generate several complications. Aiming at better patient care, quality tools have been created as quality indicators in enteral nutritional therapy (IQTNE). Objectives: To analyze IQTNE in an intensive care unit (ICU). Methods: This was a retrospective study composed of data collected during eight months from the medical charts of patients under ERT who were admitted to an ICU of a private hospital. The IQTNE analyzed were: (1) frequency of nutritional screening; (2) frequency of days of adequate energy administration; (3) frequency of days of adequate protein administration; (4) frequency of diarrhea; (5) inadvertent output frequency of nutrition probe; (6) frequency of nutrition probe obstruction and (7) adequacy rate of volume infused over prescribed. Results: The sample included data from 122 patients under ERT with mean age of 76.9 ± 16.4 years and 29.9 ± 28.6 days of hospitalization. The IQTNE (1) showed a mean of 97.3 ± 3.1%, IQTNE (2) 88.1 ± 3.8%, IQTNE (3) 89.8 ± 4.8%, IQTNE (4) 9 , IQTNE (5) 3.9 ± 2.2%, with inadequacy in 25% of the months analyzed, IQTNE (6), 0.8% ± 1.5% and the IQTNE (7) obtained 94.3 ± 3.3% of adequacy. The observed complications negatively influenced the adequacy of the instituted ERT. Conclusion: On average, all IQTNs analyzed reached the pre-established goal; however, attention should be given to the symptoms of diarrhea and inadvertent outflow of the nutrition tube.Introducción: la terapia nutricional enteral (TNE) es esencial para recuperar o mantener el estado nutricional del paciente. Sin embargo, puede no garantizar el alcance de las necesidades nutricionales y puede generar varias complicaciones. Con el objetivo de mejorar la atención al paciente, se crearon herramientas de calidad como indicadores de calidad en la terapia de nutrición enteral (ICTNE). Objetivos: Analizar ICTNE en una unidad de cuidados intensivos (UCI). Métodos: Este fue un estudio retrospectivo compuesto por datos recopilados durante ocho meses de los registros de pacientes con TNE que ingresaron en una UCI de un hospital privado. Los ICTNE analizados fueron: (1) frecuencia de cribado nutricional; (2) frecuencia de días de gestión energética adecuada; (3) frecuencia de días de administración adecuada de proteínas; (4) frecuencia de diarrea; (5) frecuencia involuntaria de salida de la sonda de nutrición; (6) frecuencia de obstrucción del tubo de nutrición y (7) tasa de adecuación del volumen infundido en relación con lo prescrito. Resultados: La muestra incluyó datos de 122 pacientes con TNE con una edad media de 76.9 ± 16.4 años y 29.9 ± 28.6 días de hospitalización. El ICTNE (1) mostró un promedio de 97.3 ± 3.1%, el ICTNE (2) 88.1 ± 3.8%, el ICTNE (3) 89.8 ± 4.8%, el ICTNE (4) 9 , 2 ± 6.3%, sin embargo, obtuvo incumplimiento en el 50% de los meses analizados, ICTNE (5) 3.9 ± 2.2%, con insuficiencia en el 25% de los meses analizados, ICTNE (6), 0.8 ± 1.5% y el ICTNE (7) obtuvo 94.3 ± 3.3% de adecuación.     Atualização no tratamento do transtorno bipolar: o impacto da psicoeducação familiar

    Abstract: Objective: to conduct a systematic review of the literature regarding the impact of family psychoeducation on the treatment of bipolar disorder. Methodology: PubMed, Lilacs, Web of A search was conducted in Science, Science Direct and Scopus using the descriptors: family psychoeducation and bipolar disorder. The last 13 years were considered. Results: 542 articles were found and after considering inclusion and exclusion criteria, 29 were selected for the study. Three other articles nominated by specialist were also used. Overall, patients diagnosed with bipolar disorder should be treated with drug therapy combined with psychotherapy. Most studies showed that the conventional treatment associated with psychoeducation reduced relapse rates and hospitalizations and that long-term programs generated better results. A comparison between supportive psychotherapy and family psychotherapy showed that it was only effective in patients who received the latter. Conclusion: although the role of pharmacological treatment is well established for the patient, (OU “ESTABLISHED IN LITERATURE”?) almost all articles consulted claim that if family psychoeducation is associated with medication, it can help with the early detection of warning signs of a crisis, reduce hospitalization rates and help reduce the number of drugs taken.Resumo: Objetivo: realizar uma revisão sistemática da literatura dos últimos 13 anos acerca do impacto da psicoeducação da família na adesão ao tratamento do paciente com perturbação bipolar. Metodologia: PubMed, Lilacs, Web of Science, Science Direct e Scopus foram pesquisados utilizando os descritores Family psychoeducation e bipolar disorder. Resultado: foram encontrados 542 artigos disponíveis, considerando critérios de inclusão e exclusão, como ano de publicação, público-alvo e tipo de intervenção; 29 foram selecionados para o estudo. Outros três artigos indicados por especialista também foram utilizados. Segundo os estudos, os pacientes com perturbação bipolar devem ser tratados com terapia medicamentosa associada à psicoterapia de apoio. A maioria dos estudos mostrou que o tratamento convencional associado à psicoeducação ocasionou redução das taxas de recaídas e internamentos, sendo que programas de longo prazo geraram melhores resultados. Na comparação entre psicoterapia de apoio e psicoterapia familiar houve benefício apenas nos pacientes que receberam esta última intervenção. Conclusão: embora o papel do tratamento farmacológico esteja bem estabelecido para o paciente na literatura, a quase totalidade dos artigos pesquisados afirma que a psicoeducação familiar, se for associada à medicação, pode auxiliar na deteção precoce dos sinais de alerta das crises, reduzir internamentos e possibilitar a diminuição das medicações utilizadas
