Telkom sebagai Perusahaan yang bergerakdibidang jasa telekomunikasi berupaya untuk meningkatkan kualitas jasanya.Dalam USAhanya untuk meningkatkan kualitas jasa maka perlu diadakan suatupenelitian tentang tingkat kepuasan konsumen pengguna jasa telekomunikasiTelkomFlexi area Jember dibidang bauran pemasaran dan kualitas pelayananterhadap kualitas jasa yang telah diberikan kepada konsumen selama ini. Metodeanalisis yang digunakan dalam penelitian ini adalah dengan Metode AnalisisStatistik Inferensial untuk menguji pengaruh antara variable bebas terhadapveriabel tergantung dan menguji hipotesis yang diajukan. Teknik pengujianhipotesis dilakukan dengan menggunakan analisis regresi linier berganda untukmengetahui pengaruh kepuasan pelanggan TelkomFlexi area Jember. Hasil analisismenunjukkan bahwa fungsi regresi linear berganda yang dapat menjelaskanpengaruh variabel harga (X1), produk (X2), distribusi (X3), promosi (X4), keandalan (X5),daya tanggap (X6), keyakinan (X7), dan empathy (X8)terhadap faktor kepuasan pelanggan (Y) adalah Y=0,01523 + 0,033 X1– 0,115 X2 +0,107 X3 + 0,466 X4 + 0,0697 X5 + 0,0113 X6 + 0,00263 X7 + 0,116 X8 + ei,sehingga disimpulkan bahwa variabel-variabel tersebut secara simultan mempunyaipengaruh yang signifikan terhadap kepuasan pelanggan TelkomFlexi Area Jember
Empirical analysis of rough set categorical clustering techniques based on rough purity and value set
Clustering a set of objects into homogeneous groups is a fundamental operation
in data mining. Recently, attention has been put on categorical data clustering,
where data objects are made up of non-numerical attributes. The implementation of
several existing categorical clustering techniques is challenging as some are unable
to handle uncertainty and others have stability issues. In the process of dealing
with categorical data and handling uncertainty, the rough set theory has become
well-established mechanism in a wide variety of applications including databases.
The recent techniques such as Information-Theoretic Dependency Roughness (ITDR),
Maximum Dependency Attribute (MDA) and Maximum Significance Attribute (MSA)
outperformed their predecessor approaches like Bi-Clustering (BC), Total Roughness
(TR), Min-Min Roughness (MMR), and standard-deviation roughness (SDR). This
work explores the limitations and issues of ITDR, MDA and MSA techniques on
data sets where these techniques fails to select or faces difficulty in selecting their
best clustering attribute. Accordingly, two alternative techniques named Rough Purity
Approach (RPA) and Maximum Value Attribute (MVA) are proposed. The novelty
of both proposed approaches is that, the RPA presents a new uncertainty definition
based on purity of rough relational data base whereas, the MVA unlike other rough
set theory techniques uses the domain knowledge such as value set combined with
number of clusters (NoC). To show the significance, mathematical and theoretical
basis for proposed approaches, several propositions are illustrated. Moreover, the
recent rough categorical techniques like MDA, MSA, ITDR and classical clustering
technique like simple K-mean are used for comparison and the results are presented
in tabular and graphical forms. For experiments, data sets from previously utilized
research cases, a real supply base management (SBM) data set and UCI repository
are utilized. The results reveal significant improvement by proposed techniques for
categorical clustering in terms of purity (21%), entropy (9%), accuracy (16%), rough
accuracy (11%), iterations (99%) and time (93%).

THE OUTCOME OF ATG ON THE STEM CELL TRANSPLANTS FROM MATCHED UNRELATED DONOR, A SINGLE INSTITUTE EXPERIENCE
poster abstractBACKGROUND: Antithymocyte globulin (ATG) was found to decrease the morbidity of stem cell transplant (SCT) from matched unrelated donor (MUD) by decreasing the incidence of chronic graft vs host disease cGvHD, and at high doses, acute GvHD. We reviewed our results of MUD transplants where ATG was incorporated into the preparative regimen, and compared the results to patients prior to September 2006 where ATG was not used. The primary endpoints were the effect on GvHD and lethal infectious complications.
Method: All stem cell transplants from MUD performed after 2000 at IU hospital for treatment of hematological malignancies using a myelo-ablative regimen were retrospectively reviewed.
Result: between 1/2000 and 3/2009 seventy nine stem cell transplants were conducted using stem cells from MUD. 28 patients received ATG at a total dose of 7.5mg/kg vs 51 patients who did not receive ATG. Both groups were matched in term of age, sex, underline malignancies, degree of HLA-match, CMV serology, and conditioning regimens. Ninety-six percent of patient in ATG group received prophylaxis for GvHD using FK506/Sirolimus vs 14% in the no ATG group where a methotrexate based treatment was used (P<0.0001). The rate of Grade II-VI acute GVHD at day 100 was significantly lower in the ATG group compare to no ATG (14% vs 39%, P =0.011). Although however, the rate of chronic GVHD at 1 year was higher in ATG group than in the no ATG group, this was statistically not significant
(43% and 23%; P=0.2). The rates of overall fungal infections and lethal fungal infections were comparable (14% and 10%) for ATG vs (17% and 11%) for no ATG (p =0.70). The rate of primary CMV infection (i.e., in patient not receiving corticosteroid treatment for GVHD) was higher in ATG group, although not statistically significant (31% vs 17%, P=0.27). Day 100 mortality was 15% and 25% in ATG and no ATG group respectively, overall survival at 1 and 2 years was 47% and 31% for ATG group vs 49% and 36% for no ATG group (P>0.05), Median time to death was 8.6 months (CI95%, 1.8-15.4) and 11.9 months (CI95%, 8-15.7) with P=0.7. The mortality from GVHD at 4 months was 0% in ATG group vs 12% in no ATG group (P =0.08).While the mortality rate from bacterial infection and sepsis were equivalent, more patients in the ATG group who did not receiving corticosteroid treatment for aGVHD died from viral and fungal infection (15% vs 0% at 8 months, P=0.013).
Summary: While ATG was associated with a trend toward lower mortality rate at day 100 due to statistically significant decrease in incidence and mortality of aGVHD, it was associated with increase rate of delayed-onset acute GVHD and statistically significant high rate of lethal viral and fungal infection leading to similar overall survival at 1 and 2 years. This study demonstrates the lack of overall benefit of ATG at dose of 7.5mg/kg. Further study to investigate the outcome of using lower doses of ATG to lower the rate of lethal infections while still reducing the risk of GvHD is recommended
Health-related quality of life as measured with EQ-5D among populations with and without specific chronic conditions: A population-based survey in Shaanxi province, China

This is an open-access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.Introduction: The aim of this study was to examine health-related quality of life (HRQoL) as measured by EQ-5D and to investigate the influence of chronic conditions and other risk factors on HRQoL based on a distributed sample located in Shaanxi Province, China. Methods: A multi-stage stratified cluster sampling method was performed to select subjects. EQ-5D was employed to measure the HRQoL. The likelihood that individuals with selected chronic diseases would report any problem in the EQ-5D dimensions was calculated and tested relative to that of each of the two reference groups. Multivariable linear regression models were used to investigate factors associated with EQ VAS. Results: The most frequently reported problems involved pain/discomfort (8.8%) and anxiety/depression (7.6%). Nearly half of the respondents who reported problems in any of the five dimensions were chronic patients. Higher EQ VAS scores were associated with the male gender, higher level of education, employment, younger age, an urban area of residence, access to free medical service and higher levels of physical activity. Except for anemia, all the selected chronic diseases were indicative of a negative EQ VAS score. The three leading risk factors were cerebrovascular disease, cancer and mental disease. Increases in age, number of chronic conditions and frequency of physical activity were found to have a gradient effect. Conclusion: The results of the present work add to the volume of knowledge regarding population health status in this area, apart from the known health status using mortality and morbidity data. Medical, policy, social and individual attention should be given to the management of chronic diseases and improvement of HRQoL. Longitudinal studies must be performed to monitor changes in HRQoL and to permit evaluation of the outcomes of chronic disease intervention programs. © 2013 Tan et al.National Nature Science Foundation (No. 8107239
Differential marker expression by cultures rich in mesenchymal stem cells
Background: Mesenchymal stem cells have properties that make them amenable to therapeutic use. However, the acceptance of mesenchymal stem cells in clinical practice requires standardized techniques for their specific isolation. To date, there are no conclusive marker (s) for the exclusive isolation of mesenchymal stem cells. Our aim was to identify markers differentially expressed between mesenchymal stem cell and non-stem cell mesenchymal cell cultures. We compared and contrasted the phenotype of tissue cultures in which mesenchymal stem cells are rich and rare. By initially assessing mesenchymal stem cell differentiation, we established that bone marrow and breast adipose cultures are rich in mesenchymal stem cells while, in our hands, foreskin fibroblast and olfactory tissue cultures contain rare mesenchymal stem cells. In particular, olfactory tissue cells represent non-stem cell mesenchymal cells. Subsequently, the phenotype of the tissue cultures were thoroughly assessed using immuno-fluorescence, flow-cytometry, proteomics, antibody arrays and qPCR. Results: Our analysis revealed that all tissue cultures, regardless of differentiation potential, demonstrated remarkably similar phenotypes. Importantly, it was also observed that common mesenchymal stem cell markers, and fibroblast-associated markers, do not discriminate between mesenchymal stem cell and non-stem cell mesenchymal cell cultures. Examination and comparison of the phenotypes of mesenchymal stem cell and non-stem cell mesenchymal cell cultures revealed three differentially expressed markers – CD24, CD108 and CD40. Conclusion: We indicate the importance of establishing differential marker expression between mesenchymal stem cells and non-stem cell mesenchymal cells in order to determine stem cell specific markers
Walks4work: Rationale and study design to investigate walking at lunchtime in the workplace setting
Background: Following recruitment of a private sector company, an 8week lunchtime walking intervention was implemented to examine the effect of the intervention on modifiable cardiovascular disease risk factors, and further to see if walking environment had any further effect on the cardiovascular disease risk factors. Methods. For phase 1 of the study participants were divided into three groups, two lunchtime walking intervention groups to walk around either an urban or natural environment twice a week during their lunch break over an 8week period. The third group was a waiting-list control who would be invited to join the walking groups after phase 1. In phase 2 all participants were encouraged to walk during their lunch break on self-selecting routes. Health checks were completed at baseline, end of phase 1 and end of phase 2 in order to measure the impact of the intervention on cardiovascular disease risk. The primary outcome variables of heart rate and heart rate variability were measured to assess autonomic function associated with cardiovascular disease. Secondary outcome variables (Body mass index, blood pressure, fitness, autonomic response to a stressor) related to cardiovascular disease were also measured. The efficacy of the intervention in increasing physical activity was objectively monitored throughout the 8-weeks using an accelerometer device. Discussion. The results of this study will help in developing interventions with low researcher input with high participant output that may be implemented in the workplace. If effective, this study will highlight the contribution that natural environments can make in the reduction of modifiable cardiovascular disease risk factors within the workplace. © 2012 Brown et al.; licensee BioMed Central Ltd
Statistical process control of mortality series in the Australian and New Zealand Intensive Care Society (ANZICS) adult patient database: implications of the data generating process
for the ANZICS Centre for Outcome and Resource Evaluation (CORE) of the Australian and New Zealand Intensive Care Society (ANZICS)BACKGROUND Statistical process control (SPC), an industrial sphere initiative, has recently been applied in health care and public health surveillance. SPC methods assume independent observations and process autocorrelation has been associated with increase in false alarm frequency. METHODS Monthly mean raw mortality (at hospital discharge) time series, 1995–2009, at the individual Intensive Care unit (ICU) level, were generated from the Australia and New Zealand Intensive Care Society adult patient database. Evidence for series (i) autocorrelation and seasonality was demonstrated using (partial)-autocorrelation ((P)ACF) function displays and classical series decomposition and (ii) “in-control” status was sought using risk-adjusted (RA) exponentially weighted moving average (EWMA) control limits (3 sigma). Risk adjustment was achieved using a random coefficient (intercept as ICU site and slope as APACHE III score) logistic regression model, generating an expected mortality series. Application of time-series to an exemplar complete ICU series (1995-(end)2009) was via Box-Jenkins methodology: autoregressive moving average (ARMA) and (G)ARCH ((Generalised) Autoregressive Conditional Heteroscedasticity) models, the latter addressing volatility of the series variance. RESULTS The overall data set, 1995-2009, consisted of 491324 records from 137 ICU sites; average raw mortality was 14.07%; average(SD) raw and expected mortalities ranged from 0.012(0.113) and 0.013(0.045) to 0.296(0.457) and 0.278(0.247) respectively. For the raw mortality series: 71 sites had continuous data for assessment up to or beyond lag ₄₀ and 35% had autocorrelation through to lag ₄₀; and of 36 sites with continuous data for ≥ 72 months, all demonstrated marked seasonality. Similar numbers and percentages were seen with the expected series. Out-of-control signalling was evident for the raw mortality series with respect to RA-EWMA control limits; a seasonal ARMA model, with GARCH effects, displayed white-noise residuals which were in-control with respect to EWMA control limits and one-step prediction error limits (3SE). The expected series was modelled with a multiplicative seasonal autoregressive model. CONCLUSIONS The data generating process of monthly raw mortality series at the ICU level displayed autocorrelation, seasonality and volatility. False-positive signalling of the raw mortality series was evident with respect to RA-EWMA control limits. Deep learning control for digital feedback systems: Improved performance with robustness against parameter change
Training data for a deep learning (DL) neural network (NN) controller are obtained from the input and output signals of a conventional digital controller that is designed to provide the suitable control signal to a specified plant within a feedback digital control system. It is found that if the DL controller is sufficiently deep (four hidden layers), it can outperform the conventional controller in terms of settling time of the system output transient response to a unit-step reference signal. That is, the DL controller introduces a damping effect. Moreover, it does not need to be retrained to operate with a reference signal of different magnitude, or under system parameter change. Such properties make the DL control more attractive for applications that may undergo parameter variation, such as sensor networks. The promising results of robustness against parameter changes are calling for future research in the direction of robust DL control
