    In the political arena in the district / city , competition between local and ethnic immigrants looks verystrong . Regional Head Election ( Election ) June 30, 2010 , held simultaneously in five districts / cities inLampung has prove it . The combination of Java and Lampung ethnic almost always arise until thegeopolitical characteristics of an area in Lampung could be represented on the winner of Regional HeadElection . In Metro Citie majority ethnic Javanese , for example , Lukman Hakim and Saleh Chandra Javabloody win the General Election . In Pesawaran district the ethnic composition was balanced betweenLampung and Java , the winner is Aries Password ( Lampung ) and Musiran ( Java ) . The same case is seenin East Lampung , although with suspect status , Satono , was dominate sound among 60 percents in themiddle of Java bloody . In South Lampung regency with dominant Lampung ethnic composition thanJavanese , so winning the Regents and Vice Regents elected, they are Waki Rycko Menoza SZP - EkiSetyanto . The purpose of this study is to determine " the role of ethnicity in the general election in SouthLampung regency at 2010”. The research methods are qualitative - descriptive type because descriptivestudies systematically describe a situation , problem , phenomenon , service or program , the provision ofinformation about a situation , problem , social life , or describe the attitude of the people responding to aparticular issue. The study shows a year before the General Election held in South Lampung regency ,political dynamics nomination process Regent and Vice Regent South Lampung regency preceded theexistence of rumors in the community about the pair potential between the " sons of ethnic " with " settlers " .The issue of ethnicity appears seven pairs of candidates Regent and Vice Regent South Lampung , the wholefamily tries to accommodate the heterogeneity of ethnic groups in South Lampung . Number 1 ( one ) ; H.Rycko Menoza , SE , SH , . MBA ( ethnic Lampung ) and H. Eky Setyanto ( Javanese ) won the SouthLampung Election at 2010. Theoretically ethnicity can be a serious issue in the election , but in SouthLampung at 2010, the issue of ethnicity could not be negative or even conflict access , so until the vote countends, it can take place smoothly and without significant disruption . The efforts of politicization of ethnicityin South Lampung Election can be clearly addressed by the community. Potential force of Rycko - Eky is inthe socio antrpologis adequate approach, which is combined synergistically and regardless of bad sounds tothis couple . Integrative and egalitarian approach undertaken by the couple is able to reach people's heartsas both are popular figure in the people of South , and is known to have positive social attitudes , as well asrepresenting the interests of ethnic Lampung and Java , as the majority of the population of South Lampungregency

    Mycobacterium ulcerans infection causes a neglected tropical disease known as Buruli ulcer that is now found in poor rural areas of West Africa in numbers that sometimes exceed those reported for another significant mycobacterial disease, leprosy, caused by M. leprae. Unique among mycobacterial diseases, M. ulcerans produces a plasmid-encoded toxin called mycolactone (ML), which is the principal virulence factor and destroys fat cells in subcutaneous tissue. Disease is typically first manifested by the appearance of a nodule that eventually ulcerates and the lesions may continue to spread over limbs or occasionally the trunk. The current standard treatment is 8 weeks of daily rifampin and injections of streptomycin (RS). The treatment kills bacilli and wounds gradually heal. Whether RS treatment actually stops mycolactone production before killing bacilli has been suggested by histopathological analyses of patient lesions. Using a mouse footpad model of M. ulcerans infection where the time of infection and development of lesions can be followed in a controlled manner before and after antibiotic treatment, we have evaluated the progress of infection by assessing bacterial numbers, mycolactone production, the immune response, and lesion histopathology at regular intervals after infection and after antibiotic therapy. We found that RS treatment rapidly reduced gross lesions, bacterial numbers, and ML production as assessed by cytotoxicity assays and mass spectrometric analysis. Histopathological analysis revealed that RS treatment maintained the association of the bacilli with (or within) host cells where they were destroyed whereas lack of treatment resulted in extracellular infection, destruction of host cells, and ultimately lesion ulceration. We propose that RS treatment promotes healing in the host by blocking mycolactone production, which favors the survival of host cells, and by killing M. ulcerans bacilli

    Corticosteroid-Induced Immunosuppression ultimately does not compromise the efficacy of antibiotherapy in murine mycobacterium ulcerans Infection

    Buruli ulcer (BU) is a necrotizing disease of the skin, subcutaneous tissue and bone caused by Mycobacterium ulcerans. It has been suggested that the immune response developed during the recommended rifampicin/streptomycin (RS) antibiotherapy is protective, contributing to bacterial clearance. On the other hand, paradoxical reactions have been described during or after antibiotherapy, characterized by pathological inflammatory responses. This exacerbated inflammation could be circumvented by immunosuppressive drugs. Therefore, it is important to clarify if the immune system contributes to bacterial clearance during RS antibiotherapy and if immunosuppression hampers the efficacy of the antibiotic regimen. METHODOLOGY/PRINCIPAL FINDINGS: We used the M. ulcerans infection footpad mouse model. Corticosteroid-induced immunosuppression was achieved before experimental infection and maintained during combined RS antibiotherapy by the administration of dexamethasone (DEX). Time-lapsed analyses of macroscopic lesions, bacterial burdens, histology and immunohistochemistry were performed in M. ulcerans-infected footpads. We show here that corticosteroid-immunosuppressed mice are more susceptible to M. ulcerans, with higher bacterial burdens and earlier ulceration. Despite this, macroscopic lesions remised during combined antibiotic/DEX treatment and no viable bacteria were detected in the footpads after RS administration. This was observed despite a delayed kinetics in bacterial clearance, associated with a local reduction of T cell and neutrophil numbers, when compared with immunocompetent RS-treated mice. In addition, no relapse was observed following an additional 3 month period of DEX administration.     Activities of Rifampin, Rifapentine and Clarithromycin Alone and in Combination against Mycobacterium ulcerans Disease in Mice

    Buruli ulcer (BU) is found throughout the world but is particularly prevalent in West Africa. Until 2004, treatment for this disfiguring disease was surgical excision followed by skin grafting, procedures often requiring months of hospitalization. More recently, an 8-week regimen of oral rifampin and streptomycin administered by injection has become the standard of care recommended by the World Health Organization. However, daily injections require sterile needles and syringes to prevent spread of blood borne pathogens and streptomycin has potentially serious side effects, most notably hearing loss. We tested an entirely oral regimen, substituting the long acting rifapentine for rifampin and clarithromycin for streptomycin. We also evaluated each drug separately. We found that rifapentine alone is as good as rifampin plus streptomycin, but the simultaneous addition of effective clarithromycin doses, at least in the mouse, reduces the activity of both rifampin and rifapentine, making it difficult to assess the efficacy of the oral regimens in the model. Studies of serum drug concentrations indicated that separating treatment times by one hour or reducing the clarithromycin dose to one active in humans should overcome this issue in experimental and clinical BU treatment, respectively

    Laboratory diagnosis of Buruli ulcer : challenges and future perspectives

    Current options to control Buruli ulcer (BU) are limited, as no effective vaccine is available and knowledge on transmission mechanisms of the causative agent, Mycobacterium ulcerans, is incomplete. Early case detection and rapid initiation of treatment are key elements to prevent the development of large, disfiguring ulcers often associated with permanent physical disability and stigma. BU has been reported from 34 countries, with the greatest disease burden in West Africa and steadily increasing case numbers in south-eastern Australia. The disease can present in a variety of clinical manifestations, including relatively unspecific, painless nodules, plaques, and edema, which may eventually progress to chronic, ulcerative lesions. The clinical diagnosis of BU is therefore complicated by a broad differential diagnosis, particularly in tropical areas, where the prevalence of other skin conditions with a similar appearance is high. With the introduction of combination antibiotic therapy, replacing excision surgery as the standard treatment for BU, pre-treatment confirmation of the clinical diagnosis has further gained in importance to avoid the redundant use of anti-mycobacterial drugs. At present, available confirmatory diagnostic tests either lack sufficient sensitivity/specificity or are centralized and thus often not accessible to patients living in remote, rural areas of Africa. In recognition of this disparity, WHO and other stakeholders have called for new diagnostic tools for BU that can be applied at district hospitals or primary healthcare facilities.
