    Hovedbudskap Hyperbar oksygenbehandling er et mulig behandlingsalternativ for personer med kronisk strålecystitt etter kreftbehandling i underlivet. Behandlingen innebærer at pasienten puster inn rent oksygen under forhøyet lufttrykk i et trykkammer. Behandlingen varer gjerne i 90 til 120 minutter, og gis fem til syv ganger i uken, totalt 30 til 40 behandlinger. Kunnskapsgrunnlaget om effekten av hyperbar oksygenbehandling består av to små randomiserte kontrollerte studier. Hyperbar oksygenbehandling sammenliknet med standard behandling (ikke spesifisert) gir: • Trolig en liten forbedring i livskvalitet • Trolig en forbedring i urinveissymptomer • Muligens en liten eller ingen forskjell i smerter målt med SF-36 Det er usikkert hvorvidt det er noen forskjell i effekt mellom hyperbar oksygenbehandling og hyaluronsyre ved blødende strålecystitt. En forenklet helseøkonomisk vurdering viser at: • Total kostnad per pasient er NOK 116 010 • Budsjettvirkninger over fem år NOK 16 010 133 For å kunne gjennomføre en helseøkonomisk analyse som kan svare på prioriteringskriteriene for alvorlighet og for ressursbruk trenges bedre informasjon om sykdomsforløp, nåværende standard behandling og livskvalitetsvekter.Key messages Hyperbaric oxygen treatment is a possible treatment option for radiation-induced cystitis following cancer treatment in the pelvic area. Patients receiving hyperbaric oxygen treatment breathe pure oxygen in a high-pressure chamber. The treatment session lasts for approximately one to two, and is provided five to seven days per week, 30 to 40 treatments in total. The evidence base for effects of hyperbaric oxygen treatment consists of two, small randomised controlled trials. Hyperbaric oxygen treatment compared to standard treatment (not specified): • Probably slightly improves quality of life • Probably improves urinary symptoms • May not make any differnce in pain as measured by SF-36 It is uncertain whether there is any difference in effects between hyperbaric oxygen treatment and hyaluronic acid in patients with haemorrhagic cystitis. A simplified economic assessment shows: • Total cost per patient is NOK 116 010 • Five-year budget impact is NOK 16 010 133 To conduct a health economic analysis that can address the priority criteria for severity and resource use, better information is needed about disease progression, current standard treatment and quality-of-life expressed as utility weights.publishedVersio

    “You made me look bad. And that’s not good”:The millennial cultification of Fatal Deviation, Ireland’s only martial arts film

    The ultra-low-budget video-shot Fatal Deviation (1998) has become renowned in cult cinema circles as Ireland’s only martial arts action film and as a unique example of ‘badfilm’, both critiqued and celebrated for its technical ineptness. This article interrogates its classification as ‘badfilm’, analysing the ways in which it both meets and fails to meet expectations concerning its generic classification, and the role of the soundtrack in shaping these expectations. Rather than the sound being considered ‘bad’ purely due to a failure to meet recognized technical standards, I argue that critique of the soundtrack demonstrates a conflict of sonic taste frameworks and hierarchies. In addition, Fatal Deviation’s cultification is a peculiarly millennial phenomenon, associated with technological changes and changes in the social uses of media technologies from the 1990s onwards that have shifted in tandem, and I argue therefore that this process of cultification is historically contingent.</p
