Outbreak of gastroenteritis caused by Yersinia pestis in Afghanistan.
Plague, which is most often caused by the bite of Yersinia pestis-infected fleas, is a rapidly progressing, serious disease that can be fatal without prompt antibiotic treatment. In late December 2007, an outbreak of acute gastroenteritis occurred in Nimroz Province of southern Afghanistan. Of the 83 probable cases of illness, 17 died (case fatality 20·5%). Being a case was associated with consumption or handling of camel meat (adjusted odds ratio 4·4, 95% confidence interval 2·2-8·8, P<0·001). Molecular testing of patient clinical samples and of tissue from the camel using PCR/electrospray ionization-mass spectrometry revealed DNA signatures consistent with Yersinia pestis. Confirmatory testing using real-time PCR and immunological seroconversion of one of the patients confirmed that the outbreak was caused by plague, with a rare gastrointestinal presentation. The study highlights the challenges of identifying infectious agents in low-resource settings; it is the first reported occurrence of plague in Afghanistan
Incentives facing UK-listed companies to comply with the risk reporting provisions of the UK Corporate Governance Code
Recent changes made to the UK Corporate Governance Code require UK firms to report new or enhanced narrative information concerning their principal risks, their
risk management processes and their future viability. This paper analyses whether the level and nature of voluntary compliance with these new requirements is consistent
with alternative economic and political visibility incentives. We analyse relevant sections of financial reports produced by industry matched samples of large-, mid-and small-cap UK listed firms during the transitional 2013-14 financial reporting years. Both specific and
generic readability attributes of the reports are measured. We find that virtually no firm in our sample has provided any viability statement. Empirical analysis of disclosures concerning principal risk assessment and review processes appear to be primarily motivated by political visibility
reasons. Examples of particularly good and cases of poor corporate risk reporting practices are also discussed. Possible implications for the actuarial profession are discussed
Fracture healing following high energy tibial trauma: Ilizarov versus Taylor Spatial Frame
Introduction: The optimal treatment of high energy tibial fractures remains controversial and a challenging orthopaedic problem. The role of external fi xators for all these tibial fractures has been shown to be crucial. Methods: A fi ve-year consecutive series was reviewed retrospectively, identifying two treatment groups: Ilizarov and Taylor Spatial Frame (TSF; Smith & Nephew, Memphis, TN, US). Fracture healing time was the primary outcome measure. Results: A total of 112 patients (85 Ilizarov, 37 TSF) were identifi ed for the review with a mean age of 45 years. This was higher in women (57 years) than in men (41 years). There was no signifi cant difference between frame types (p=0.83). The median healing time was 163 days in both groups. There was no signifi cant difference in healing time between smokers and non-smokers (180 vs 165 days respectively, p=0.07), open or closed fractures (p=0.13) or age and healing time (Spearman's r=0.12, p=0.18). There was no incidence of non-union or re-fracture following frame removal in either group. Conclusions: Despite the assumption of the rigid construct of the TSF, the median time to union was similar to that of the Ilizarov frame and the TSF therefore can play a signifi cant role in complex tibial fractures
Review of Physical Activity Benefits and Potential Considerations for Individuals with Surgical Fusion of Spine for Scoliosis
International Journal of Exercise Science 10(2): 166-177, 2017 Evidence-based recommendations for physical activity following spinal fusion surgeries for idiopathic scoliosis are limited, specifically in the adolescent population. Individuals with scoliosis treated operatively or non-operatively have been reported to participate in less than 1-3 days/week of even mildly strenuous physical exercises. Over 40% of individuals with scoliosis returned to sports at a level lower than pre-operative participation levels or did not return at all post spinal fusion. It is particularly important for human movement specialists, such as physical therapists, occupational therapists, athletic trainers and kinesiologists to assist these individuals effectively transition to and maintain engagement in physical activity. This review provides a snapshot of common considerations and potential factors influencing individuals with spinal-fusion for scoliosis to participate in safe physical activity
PCA-based lung motion model
Organ motion induced by respiration may cause clinically significant
targeting errors and greatly degrade the effectiveness of conformal
radiotherapy. It is therefore crucial to be able to model respiratory motion
accurately. A recently proposed lung motion model based on principal component
analysis (PCA) has been shown to be promising on a few patients. However, there
is still a need to understand the underlying reason why it works. In this
paper, we present a much deeper and detailed analysis of the PCA-based lung
motion model. We provide the theoretical justification of the effectiveness of
PCA in modeling lung motion. We also prove that under certain conditions, the
PCA motion model is equivalent to 5D motion model, which is based on physiology
and anatomy of the lung. The modeling power of PCA model was tested on clinical

Molecular testing for the clinical diagnosis of fibrolamellar carcinoma.
Fibrolamellar carcinoma has a distinctive morphology and immunophenotype, including cytokeratin 7 and CD68 co-expression. Despite the distinct findings, accurate diagnosis of fibrolamellar carcinoma continues to be a challenge. Recently, fibrolamellar carcinomas were found to harbor a characteristic somatic gene fusion, DNAJB1-PRKACA. A break-apart fluorescence in situ hybridization (FISH) assay was designed to detect this fusion event and to examine its diagnostic performance in a large, multicenter, multinational study. Cases initially classified as fibrolamellar carcinoma based on histological features were reviewed from 124 patients. Upon central review, 104 of the 124 cases were classified histologically as typical of fibrolamellar carcinoma, 12 cases as 'possible fibrolamellar carcinoma' and 8 cases as 'unlikely to be fibrolamellar carcinoma'. PRKACA FISH was positive for rearrangement in 102 of 103 (99%) typical fibrolamellar carcinomas, 9 of 12 'possible fibrolamellar carcinomas' and 0 of 8 cases 'unlikely to be fibrolamellar carcinomas'. Within the morphologically typical group of fibrolamellar carcinomas, two tumors with unusual FISH patterns were also identified. Both cases had the fusion gene DNAJB1-PRKACA, but one also had amplification of the fusion gene and one had heterozygous deletion of the normal PRKACA locus. In addition, 88 conventional hepatocellular carcinomas were evaluated with PRKACA FISH and all were negative. These findings demonstrate that FISH for the PRKACA rearrangement is a clinically useful tool to confirm the diagnosis of fibrolamellar carcinoma, with high sensitivity and specificity. A diagnosis of fibrolamellar carcinoma is more accurate when based on morphology plus confirmatory testing than when based on morphology alone
Thermal history modeling of the H chondrite parent body
The cooling histories of individual meteorites can be empirically
reconstructed by using ages from different radioisotopic chronometers with
distinct closure temperatures. For a group of meteorites derived from a single
parent body such data permit the reconstruction of the cooling history and
properties of that body. Particularly suited are H chondrites because precise
radiometric ages over a wide range of closure temperatures are available. A
thermal evolution model for the H chondrite parent body is constructed by using
all H chondrites for which at least three different radiometric ages are
available. Several key parameters determining the thermal evolution of the H
chondrite parent body and the unknown burial depths of the H chondrites are
varied until an optimal fit is obtained. The fit is performed by an 'evolution
algorithm'. Empirical data for eight samples are used for which radiometric
ages are available for at least three different closure temperatures. A set of
parameters for the H chondrite parent body is found that yields excellent
agreement (within error bounds) between the thermal evolution model and
empirical data of six of the examined eight chondrites. The new thermal model
constrains the radius and formation time of the H chondrite parent body
(possibly (6) Hebe), the initial burial depths of the individual H chondrites,
the average surface temperature of the body, the average initial porosity of
the material the body accreted from, and the initial 60Fe content of the H

PTTG/securin activates expression of p53 and modulates its function
BACKGROUND: Pituitary tumor transforming gene (PTTG) is a novel oncogene that is expressed abundantly in most tumors. Overexpression of PTTG induces cellular transformation and promotes tumor formation in nude mice. PTTG has been implicated in various cellular processes including sister chromatid separation during cell division as well as induction of apoptosis through p53-dependent and p53-independent mechanisms. The relationship between PTTG and p53 remains unclear, however. RESULTS: Here we report the effects of overexpression of PTTG on the expression and function of p53. Our results indicate that overexpression of PTTG regulates the expression of the p53 gene at both the transcriptional and translational levels and that this ability of PTTG to activate the expression of p53 gene is dependent upon the p53 status of the cell. Deletion analysis of the p53 gene promoter revealed that only a small region of the p53 gene promoter is required for its activation by PTTG and further indicated that the activation of p53 gene by PTTG is an indirect effect that is mediated through the regulation of the expression of c-myc, which then interacts with the p53 gene promoter. Our results also indicate that overexpression of PTTG stimulates expression of the Bax gene, one of the known downstream targets of p53, and induces apoptosis in a human embryonic kidney cell line (HEK293). This stimulation of bax expression by PTTG is indirect and is mediated through modulation of p53 gene expression. CONCLUSIONS: Overexpression of PTTG activates the expression of p53 and modulates its function, with this action of PTTG being mediated through the regulation of c-myc expression. PTTG also up-regulates the activity of the bax promoter and increases the expression of bax through modulation of p53 expression
TRMM Latent Heating Retrieval and Comparisons with Field Campaigns and Large-Scale Analyses
Rainfall production is a fundamental process within the Earth's hydrological cycle because it represents both a principal forcing term in surface water budgets, and its energetics corollary, latent heating (LH), is one of the principal sources of atmospheric diabatic heating. Latent heat release itself is a consequence of phase changes between the vapor, liquid, and frozen states of water. The vertical distribution of LH has a strong influence on the atmosphere, controlling large-scale tropical circulations, exciting and modulating tropical waves, maintaining the intensities of tropical cyclones, and even providing the energetics of midlatitude cyclones and other mobile midlatitude weather systems. Moreover, the processes associated with LH result in significant non-linear changes in atmospheric radiation through the creation, dissipation and modulation of clouds and precipitation. Yanai et al. (1973) utilized the meteorological data collected from a sounding network to present a pioneering work on thermodynamic budgets, which are referred to as the apparent heat source (Q1) and apparent moisture sink (Q2). Yanai's paper motivated the development of satellite-based LH algorithms and provided a theoretical background for imposing large-scale advective forcing into cloud-resolving models (CRMs). These CRM-simulated LH and Q1 data have been used to generate the look-up tables used in LH algorithms. This paper examines the retrieval, validation, and application of LH estimates based on rain rate quantities acquired from the Tropical Rainfall Measuring Mission satellite (TRMM). TRMM was launched in November 1997 as a joint enterprise between the American and Japanese space agencies -- with overriding goals of providing accurate four-dimensional estimates of rainfall and LH over the global Tropics and subtropics equatorward of 35o. Other literature has acknowledged the achievement of the first goal of obtaining an accurate rainfall climatology. This paper describes the second major goal of obtaining credible LH estimates as well as their applications within TRMM's zone of coverage, the standard TRMM LH products, and areas for further improvement
