Rejoinder: Time-dynamic profiling with application to hospital readmission among patients on dialysis.
Standard profiling analysis aims to evaluate medical providers, such as hospitals, nursing homes, or dialysis facilities, with respect to a patient outcome. The outcome, for instance, may be mortality, medical complications, or 30-day (unplanned) hospital readmission. Profiling analysis involves regression modeling of a patient outcome, adjusting for patient health status at baseline, and comparing each provider's outcome rate (e.g., 30-day readmission rate) to a normative standard (e.g., national "average"). Profiling methods exist mostly for non time-varying patient outcomes. However, for patients on dialysis, a unique population which requires continuous medical care, methodologies to monitor patient outcomes continuously over time are particularly relevant. Thus, we introduce a novel time-dynamic profiling (TDP) approach to assess the time-varying 30-day readmission rate. TDP is used to estimate, for the first time, the risk-standardized time-dynamic 30-day hospital readmission rate, throughout the time period that patients are on dialysis. We develop the framework for TDP by introducing the standardized dynamic readmission ratio as a function of time and a multilevel varying coefficient model with facility-specific time-varying effects. We propose estimation and inference procedures tailored to the problem of TDP and to overcome the challenge of high-dimensional parameters when examining thousands of dialysis facilities
Rewritable nanoscale oxide photodetector
Nanophotonic devices seek to generate, guide, and/or detect light using
structures whose nanoscale dimensions are closely tied to their functionality.
Semiconducting nanowires, grown with tailored optoelectronic properties, have
been successfully placed into devices for a variety of applications. However,
the integration of photonic nanostructures with electronic circuitry has always
been one of the most challenging aspects of device development. Here we report
the development of rewritable nanoscale photodetectors created at the interface
between LaAlO3 and SrTiO3. Nanowire junctions with characteristic dimensions
2-3 nm are created using a reversible AFM writing technique. These nanoscale
devices exhibit a remarkably high gain for their size, in part because of the
large electric fields produced in the gap region. The photoconductive response
is gate-tunable and spans the visible-to-near-infrared regime. The ability to
integrate rewritable nanoscale photodetectors with nanowires and transistors in
a single materials platform foreshadows new families of integrated

A Randomized Controlled Trial of Sertraline in Young Children With Autism Spectrum Disorder.
Objective: Selective serotonin reuptake inhibitors like sertraline have been shown in observational studies and anecdotal reports to improve language development in young children with fragile X syndrome (FXS). A previous controlled trial of sertraline in young children with FXS found significant improvement in expressive language development as measured by the Mullen Scales of Early Learning (MSEL) among those with comorbid autism spectrum disorder (ASD) in post hoc analysis, prompting the authors to probe whether sertraline is also indicated in nonsyndromic ASD. Methods: The authors evaluated the efficacy of 6 months of treatment with low-dose sertraline in a randomized, double-blind, placebo-controlled trial in 58 children with ASD aged 24 to 72 months. Results: 179 subjects were screened for eligibility, and 58 were randomized to sertraline (32) or placebo (26). Eight subjects from the sertraline arm and five from the placebo arm discontinued. Intent-to-treat analysis showed no significant difference from placebo on the primary outcomes (MSEL expressive language raw score and age equivalent combined score) or secondary outcomes. Sertraline was well tolerated, with no difference in side effects between sertraline and placebo groups. No serious adverse events possibly related to study treatment occurred. Conclusion: This randomized controlled trial of sertraline treatment showed no benefit with respect to primary or secondary outcome measures. For the 6-month period, treatment in young children with ASD appears safe, although the long-term side effects of low-dose sertraline in early childhood are unknown. One-Year Mortality after Contemporary Laparoscopic Bariatric Surgery: An Analysis of the Bariatric Outcomes Longitudinal Database.
BACKGROUND:Contemporary mortality after bariatric surgery is low and has been decreasing over the past 2 decades. Most studies have reported inpatient or 30-day mortality, which may not represent the true risk of bariatric surgery. The objective of this study was to examine 1-year mortality and factors predictive of 1-year mortality after contemporary laparoscopic bariatric surgery. STUDY DESIGN:Using the 2008 to 2012 Bariatric Outcomes Longitudinal Database (BOLD), data from 158,606 operations were analyzed, including 128,349 (80.9%) laparoscopic Roux-en-Y gastric bypass (LRYGB) and 30,257 (19.1%) laparoscopic sleeve gastrectomy (LSG) operations. Multivariate logistic regression was used to determine independent risk factors associated with 1-year mortality for each type of procedure. RESULTS:The 30-day and 1-year mortality rates for LRYGB were 0.13% and 0.23%, respectively, and for LSG were 0.06% and 0.11%, respectively. Risk factors for 1-year mortality included older age (LRYGB: adjusted odds ratio [AOR] 1.05 per year, p < 0.001; LSG: AOR 1.08 per year, p < 0.001); male sex (LRYGB: AOR 1.88, p < 0.001); higher BMI (LRYGB: AOR 1.04 per unit, p < 0.001; LSG: AOR 1.05 per unit, p = 0.009); and the presence of 30-day leak (LRYGB: AOR 25.4, p < 0.001; LSG: AOR 35.8, p < 0.001), 30-day pulmonary embolism (LRYGB: AOR 34.5, p < 0.001; LSG: AOR 252, p < 0.001), and 30-day hemorrhage (LRYGB: AOR 2.34, p = 0.001). CONCLUSIONS:Contemporary 1-year mortality after laparoscopic bariatric surgery is much lower than previously reported, at <0.25%. It is important to continually refine techniques and perioperative management in order to minimize leaks, hemorrhage, and pulmonary embolus after bariatric surgery because these complications contribute to a higher risk of mortality
Interactions between p-Akt and Smad3 in injured muscles initiate myogenesis or fibrogenesis
In catabolic conditions such as aging and diabetes, IGF signaling is impaired and fibrosis develops in skeletal muscles. To examine whether impaired IGF signaling initiates muscle fibrosis, we generated IGF-IR+/- heterozygous mice by crossing loxP-floxed IGF-IR (exon 3) mice with MyoD-cre mice. IGF-IR+/- mice were studied because we were unable to obtain homozygous IGF-IR-KO mice. in IGF-IR+/- mice, both growth and expression of myogenic genes (MyoD and myogenin; markers of satellite cell proliferation and differentiation, respectively) were depressed. Likewise, in injured muscles of IGF-IR+/- mice, there was impaired regeneration, depressed expression of MyoD and myogenin, and increased expression of TGF-beta 1, alpha-SMA, collagen I, and fibrosis. To uncover mechanisms stimulating fibrosis, we isolated satellite cells from muscles of IGF-IR+/- mice and found reduced proliferation and differentiation plus increased TGF-beta 1 production. in C2C12 myoblasts (a model of satellite cells), IGF-I treatment inhibited TGF-beta 1-stimulated Smad3 phosphorylation, its nuclear translocation, and expression of fibronectin. Using immunoprecipitation assay, we found an interaction between p-Akt or Akt with Smad3 in wild-type mouse muscles and in C2C12 myoblasts; importantly, IGF-I increased p-Akt and Smad3 interaction, whereas TGF-beta 1 decreased it. Therefore, in muscles of IGF-IR+/- mice, the reduction in IGF-IR reduces p-Akt, allowing for dissociation and nuclear translocation of Smad3 to enhance the TGF-beta 1 signaling pathway, leading to fibrosis. Bilingually motivated word segmentation for statistical machine translation
We introduce a bilingually motivated word segmentation approach to languages where word boundaries are not orthographically marked, with application to Phrase-Based Statistical Machine Translation (PB-SMT). Our approach is motivated from the insight that PB-SMT systems can be improved by optimizing the input representation to reduce the predictive power of translation models. We firstly present an approach to optimize the existing segmentation of both source and target languages for PB-SMT and demonstrate the effectiveness of this approach using a
Chinese–English MT task, that is, to measure the influence of the segmentation on the performance of PB-SMT systems. We report a 5.44% relative increase in Bleu score and a consistent increase according to other metrics. We then generalize this method for Chinese word segmentation without relying on any segmenters and show that using our segmentation PB-SMT can achieve more consistent state-of-the-art performance across two domains. There are two main
advantages of our approach. First of all, it is adapted to the specific translation task at hand by taking the corresponding source (target) language into account. Second, this approach does not rely on manually segmented training data so that it can be automatically adapted for different domains
A critical review and comparative analysis of cost management on prefabricated construction research (2000–2022)
Prefabrication construction has gained attention in the construction industry. The cost of prefabrication has got mixed reviews. Cost management in prefabricated construction (CMPC) includes many cost considerations such as initial design costs, supply chain cost considerations, maintenance costs and assembly costs. These costs are inter-linked and thorough understanding on cost management is essential. It is important to develop a holistic cost management system to capture all the economic, social and environmental aspects of prefabricated construction. The aim of this research is to conduct as a critical review and analysis of cost management in prefabricated construction holistically. The literature review selected 63 articles for this research study from 2000 to 2022. The research showed that there is an uptake in research on this research area since 2005. The study identified four main research categories in CMPC namely 1) cost estimating, 2) cost optimization, 3) economic performance and 4) cost management models. Previous studies mostly focused on estimating costs and comparison studies with conventional construction. Recently studies focused more on developing cost model to integrate supply chains and other considerations into cost evaluation. Based on the literature review, there are several future directions in CMPC. Cost estimating should now focus on identifying the effect of each cost determinant in project scenarios to provide more accurate results. Microstructure evolution of commercial pure titanium during equal channel angular pressing
The microstructure development of commercially pure titanium during 3 and 4 passes of equal channel angular pressing (ECAP) has been investigated in detail by optical microscopy and electron backscattered diffraction (EBSD). The flow line, texture, grain, subgrain and twinning structures along the whole length of the ECAP bar have been characterized systematically. A theoretically predicted triangular zone with a completely different flow line pattern and texture has been found at the extruded end of the sample. Details of the misorientation gradient inside grains and the continuous dynamic recrystallization (CDRX) mechanism are discussed. Low angle grain boundaries (LAGBs) in grains are seen to evolve into high angle grain boundaries (HAGBs) throughout the material.
