    Oral capecitabine as an alternative to i.v. 5-fluorouracil-based adjuvant therapy for colon cancer: safety results of a randomized, phase III trial

    Background: Oral capecitabine achieves a superior response rate with an improved safety profile compared with bolus 5-fluorouracil-leucovorin (5-FU/LV) as first-line treatment for patients with metastatic colorectal cancer. We report here the results of a large phase III trial investigating adjuvant oral capecitabine compared with 5-FU/LV (Mayo Clinic regimen) in Dukes' C colon cancer. Patients and methods: Patients aged 18-75 years with resected Dukes' C colon carcinoma were randomized to receive 24 weeks of treatment with either oral capecitabine 1250 mg/m2 twice daily, days 1-14 every 21 days (n = 993), or i.v. bolus 5-FU 425 mg/m2 with i.v. leucovorin 20 mg/m2 on days 1-5, repeated every 28 days (n = 974). Results: Patients receiving capecitabine experienced significantly (P <0.001) less diarrhea, stomatitis, nausea/vomiting, alopecia and neutropenia, but more hand-foot syndrome than those receiving 5-FU/LV. Fewer patients receiving capecitabine experienced grade 3 or 4 neutropenia, febrile neutropenia/sepsis and stomatitis (P <0.001), although more experienced grade 3 hand-foot syndrome than those treated with 5-FU/LV (P <0.001). Capecitabine demonstrates a similar, favorable safety profile in patients aged <65 years or ≥65 years old. Conclusions: Based on its improved safety profile, capecitabine has the potential to replace 5-FU/LV as standard adjuvant treatment for patients with colon cancer.     Joule-heating Effects In the Amorphous Fe40ni40b20 Alloy

    The effects of Joule heating on the amorphous Fe40Ni40B20 alloy are investigated by measuring the time behavior of the electrical resistance of ribbon strips during such a treatment. The structural transformations occurring in subsequent stages of the process are studied by means of x-ray-diffraction, differential-scanning-calorimetry, and magnetic-permeability measurements. A continuous evolution from a fully amorphous to a fully crystalline structure may be followed. The crystallization mechanisms observed in Joule-heated samples differ from the ones occurring under conventional heating conditions. The electrical resistance displays a bump in the course of Joule heating. A quantitative model relating such a bump to the extra heat released to the sample by fast crystallization is proposed and discussed

    An update of the Worldwide Integrated Assessment (WIA) on systemic insecticides. Part 2: impacts on organisms and ecosystems

    New information on the lethal and sublethal effects of neonicotinoids and fipronil on organisms is presented in this review, complementing the previous WIA in 2015. The high toxicity of these systemic insecticides to invertebrates has been confirmed and expanded to include more species and compounds. Most of the recent research has focused on bees and the sublethal and ecological impacts these insecticides have on pollinators. Toxic effects on other invertebrate taxa also covered predatory and parasitoid natural enemies and aquatic arthropods. Little, while not much new information has been gathered on soil organisms. The impact on marine coastal ecosystems is still largely uncharted. The chronic lethality of neonicotinoids to insects and crustaceans, and the strengthened evidence that these chemicals also impair the immune system and reproduction, highlights the dangers of this particular insecticidal classneonicotinoids and fipronil. , withContinued large scale – mostly prophylactic – use of these persistent organochlorine pesticides has the potential to greatly decreasecompletely eliminate populations of arthropods in both terrestrial and aquatic environments. Sublethal effects on fish, reptiles, frogs, birds and mammals are also reported, showing a better understanding of the mechanisms of toxicity of these insecticides in vertebrates, and their deleterious impacts on growth, reproduction and neurobehaviour of most of the species tested. This review concludes with a summary of impacts on the ecosystem services and functioning, particularly on pollination, soil biota and aquatic invertebrate communities, thus reinforcing the previous WIA conclusions (van der Sluijs et al. 2015)

    Longitudinal effects of childhood recreational gymnastics participation on bone development: The Young Recreational Gymnast Study

    PurposePrevious research in the Young Recreational Gymnast Study (2006–2014) found bone benefits from involvement in recreational gymnastics during young childhood. The purpose of this study was to identify any longitudinal effects of recreational gymnastics exposure during childhood on adolescent bone health.MethodsFor the present analysis, longitudinal data were available from 118 participants (66 female participants; 33 gymnasts) of the original 178 who were recruited and assessed on between one and five measurement occasions (median 3) between 2008 and 2020. Peripheral quantitative computed tomography (pQCT) scans were completed at both the distal and shaft sites of their non-dominant radius and tibia. Multilevel random-effects models were constructed to assess differences in the development of bone content, density, and estimated bone strength between those exposed and not exposed to recreational gymnastics while controlling for biological age, sex, body weight, limb length, and physical activity.ResultsIndividuals who were exposed to recreational gymnastics during childhood were found to have greater total area, total content, bone strength index, trabecular area, trabecular content, and trabecular density at the distal radius compared to physically active controls. There were no differences at the radial shaft, distal tibia, or tibial shaft.ConclusionInvolvement in childhood recreational gymnastics may provide long-term benefits at the distal radius as individuals enter young adulthood

    Quality of life of palliative chemotherapy naive patients with advanced adenocarcinoma of the stomach or esophagogastric junction treated with irinotecan combined with 5-fluorouracil and folinic acid: results of a randomised phase III trial

    41st Annual Meeting of the American-Society-of-Clinical-Oncology -- MAY 13-17, 2005 -- Orlando, FLWOS: 000268881000007PubMed ID: 19568958The quality of life (QL) of advanced gastric cancer patients receiving irinotecan, folinic acid and 5-fluorouracil (5-FU) (IF arm) or cisplatin with 5-FU (CF arm) is presented. Patients with measurable or evaluable advanced gastric cancer received IF weekly for 6/7 weeks or CF q4 weeks. QL was assessed using the EORTC QLQ-C30 at baseline, subsequently every 8 weeks until progression and thereafter every 3 months until death. The QL data were analysed using several statistical methods including summary measures and pattern-mixture modelling. A total of 333 patients were randomised and treated (IF 170, CF 163). The time-to-progression for IF and CF was 5.0 and 4.2 months (P = 0.088), respectively. The overall compliance rates for QL questionnaire completion were 60 and 56% in the IF and CF arms, respectively. Significant treatment differences were observed for the physical functioning scale (P = 0.024), nausea\vomiting (P = 0.001) and EQ-5D thermometer (P = 0.020) in favour of the IF treatment arm. There was a trend in favour of IF over CF in time-to-progression. The IF group also demonstrated a better safety profile than CF and a better QL on a number of multi-item scales, suggesting that IF offers an alternative first-line platinum-free treatment option for advanced gastric cancer.Amer Soc Clin Onco

    Field Demonstration of an In Situ Passive Biological Acid Mine Drainage Treatment Process

    An innovative, biological treatment process has been designed by MSE Technology Applications, Inc to treat and control acid mine drainage (AMD). A field-scale demonstration is being conducted under the Mine Waste Technology Program using water flowing from a historic mine in Montana. A successful demonstration of an in situ process will allow it to be employed at numerous other abandoned or remote mines, which discharge AMD, an acidic, metal-contaminated water. Conventional treatment methods are costly and are often impossible to use at remote locations. MSE’s technology uses a biological system to clean up AMD. The In situ Passive Biological Treatment Process utilises nonpathogenic bacteria, which are able to neutralise acidic mine water and remove contained toxic metals. These bacteria live within a series of pile-type reactors inside the mine. The bacteria are fed inexpensive waste products. The resulting discharge is clean water flowing from the mine. This presentation will show that this technology offers a natural cleanup method for the nation’s numerous abandoned mines.
