Comparison of breast and bowel cancer screening uptake patterns in a common cohort of South Asian women in England
Background: Inequalities in uptake of cancer screening by ethnic minority populations are well documented in a
number of international studies. However, most studies to date have explored screening uptake for a single cancer
only. This paper compares breast and bowel cancer screening uptake for a cohort of South Asian women invited to
undertake both, and similarly investigates these women's breast cancer screening behaviour over a period of fifteen
years.
Methods: Screening data for rounds 1, 2 and 5 (1989-2004) of the NHS breast cancer screening programme and for
round 1 of the NHS bowel screening pilot (2000-2002) were obtained for women aged 50-69 resident in the English
bowel screening pilot site, Coventry and Warwickshire, who had been invited to undertake breast and bowel cancer
screening in the period 2000-2002. Breast and bowel cancer screening uptake levels were calculated and compared
using the chi-squared test.
Results: 72,566 women were invited to breast and bowel cancer screening after exclusions. Of these, 3,539 were South
Asian and 69,027 non-Asian; 18,730 had been invited to mammography over the previous fifteen years (rounds 1 to 5).
South Asian women were significantly less likely to undertake both breast and bowel cancer screening; 29.9% (n =
1,057) compared to 59.4% (n = 40,969) for non-Asians (p < 0.001). Women in both groups who consistently chose to
undertake breast cancer screening in rounds 1, 2 and 5 were more likely to complete round 1 bowel cancer screening.
However, the likelihood of completion of bowel cancer screening was still significantly lower for South Asians; 49.5% vs.
82.3% for non-Asians, p < 0.001. South Asian women who undertook breast cancer screening in only one round were
no more likely to complete bowel cancer screening than those who decided against breast cancer screening in all
three rounds. In contrast, similar women in the non-Asian population had an increased likelihood of completing the
new bowel cancer screening test. The likelihood of continued uptake of mammography after undertaking screening in
round 1 differed between South Asian religio-linguistic groups. Noticeably, women in the Muslim population were less
likely to continue to participate in mammography than those in other South Asian groups.
Conclusions: Culturally appropriate targeted interventions are required to reduce observed disparities in cancer
screening uptakes
Clinicopathological Profile and Surgical Treatment of Abdominal Tuberculosis: A Single Centre Experience in Northwestern Tanzania.
Abdominal tuberculosis continues to be a major public health problem worldwide and poses diagnostic and therapeutic challenges to general surgeons practicing in resource-limited countries. This study was conducted to describe the clinicopathological profile and outcome of surgical treatment of abdominal tuberculosis in our setting and compare with what is described in literature. A prospective descriptive study of patients who presented with abdominal tuberculosis was conducted at Bugando Medical Centre (BMC) in northwestern Tanzania from January 2006 to February 2012. Ethical approval to conduct the study was obtained from relevant authorities. Statistical data analysis was performed using SPSS version 17.0. Out of 256 patients enrolled in the study, males outnumbered females. The median age was 28 years (range = 16-68 years). The majority of patients (77.3%) had primary abdominal tuberculosis. A total of 127 (49.6%) patients presented with intestinal obstruction, 106 (41.4%) with peritonitis, 17 (6.6%) with abdominal masses and 6 (2.3%) patients with multiple fistulae in ano. Forty-eight (18.8%) patients were HIV positive. A total of 212 (82.8%) patients underwent surgical treatment for abdominal tuberculosis. Bands /adhesions (58.5%) were the most common operative findings. Ileo-caecal region was the most common bowel involved in 122 (57.5%) patients. Release of adhesions and bands was the most frequent surgical procedure performed in 58.5% of cases. Complication and mortality rates were 29.7% and 18.8% respectively. The overall median length of hospital stay was 32 days and was significantly longer in patients with complications (p < 0.001). Advanced age (age ≥ 65 years), co-morbid illness, late presentation, HIV positivity and CD4+ count < 200 cells/μl were statistically significantly associated with mortality (p < 0.0001). The follow up of patients were generally poor as only 37.5% of patients were available for follow up at twelve months after discharge. Abdominal tuberculosis constitutes a major public health problem in our environment and presents a diagnostic challenge requiring a high index of clinical suspicion. Early diagnosis, early anti-tuberculous therapy and surgical treatment of the associated complications are essential for survival
Modulation of 11β-hydroxysteroid dehydrogenase as a strategy to reduce vascular inflammation
Atherosclerosis is a chronic inflammatory disease in which initial vascular damage leads to extensive macrophage and lymphocyte infiltration. Although acutely glucocorticoids suppress inflammation, chronic glucocorticoid excess worsens atherosclerosis, possibly by exacerbating systemic cardiovascular risk factors. However, glucocorticoid action within the lesion may reduce neointimal proliferation and inflammation. Glucocorticoid levels within cells do not necessarily reflect circulating levels due to pre-receptor metabolism by 11β-hydroxysteroid dehydrogenases (11β-HSDs). 11β-HSD2 converts active glucocorticoids into inert 11-keto forms. 11β-HSD1 catalyses the reverse reaction, regenerating active glucocorticoids. 11β-HSD2-deficiency/ inhibition causes hypertension, whereas deficiency/ inhibition of 11β-HSD1 generates a cardioprotective lipid profile and improves glycemic control. Importantly, 11β-HSD1-deficiency/ inhibition is atheroprotective, whereas 11β-HSD2-deficiency accelerates atherosclerosis. These effects are largely independent of systemic risk factors, reflecting modulation of glucocorticoid action and inflammation within the vasculature. Here, we consider whether evidence linking the 11β-HSDs to vascular inflammation suggests these isozymes are potential therapeutic targets in vascular injury and atherosclerosis

QCD and strongly coupled gauge theories : challenges and perspectives
We highlight the progress, current status, and open challenges of QCD-driven physics, in theory and in experiment. We discuss how the strong interaction is intimately connected to a broad sweep of physical problems, in settings ranging from astrophysics and cosmology to strongly coupled, complex systems in particle and condensed-matter physics, as well as to searches for physics beyond the Standard Model. We also discuss how success in describing the strong interaction impacts other fields, and, in turn, how such subjects can impact studies of the strong interaction. In the course of the work we offer a perspective on the many research streams which flow into and out of QCD, as well as a vision for future developments.Peer reviewe
Pengaruh Pgpr (Plant Growth Promoting Rhizobacteria) Dan Pupuk Kandang Kambing Terhadap Pertumbuhan Dan Hasil Kedelai (Glycine Max L.)

Indonesia pada tahun 2019 mencapai 2.967.695 ton tahun-1
, sedangkan produksi
kedelai pada tahun 2019 sebesar 424.189 ton tahun-1
(Kementan, 2020).
Penambahan pupuk kandang kambing dapat mengimbangi penggunaan pupuk
anorganik. Salah satu upaya dalam memaksimalkan pupuk kandang kambing yang
tidak cepat tersedia yaitu dengan menggunakan Plant Growth Promoting
Rhizobacteria (PGPR) yang merupakan kelompok mikroorganisme tanah yang
menguntungkan. Tujuan dari penelitian ini yaitu mempelajari interaksi antara
konsentrasi PGPR dan dosis pupuk kandang kambing pada tanaman kedelai serta
mendapatkan konsentrasi PGPR dan dosis pupuk kandang kambing yang tepat
terhadap pertumbuhan dan hasil pada tanaman kedelai. Hipotesis dari penelitian ini
yaitu terdapat interaksi dengan pemberian PGPR dan pupuk kandang kambing pada
tanaman kedelai, pemberian PGPR pada level tertentu memberikan pengaruh nyata
terhadap pertumbuhan dan hasil pada tanaman kedelai dan pemberian pupuk
kandang kambing memberikan pengaruh nyata terhadap pertumbuhan dan hasil
pada tanaman kedelai.
Penelitian ini telah dilaksanakan pada bulan Juli hingga September 2023 di
Kecamatan Junrejo, Kelurahan Dadaprejo, Kota Batu, Jawa Timur. Rancangan
penelitian yang digunakan yaitu RAK faktorial yang terdiri dari 2 faktor. Faktor
pertama yaitu konsentrasi PGPR diberikan sebanyak 3 kali yaitu pada 14, 28 dan
42 HST dengan konsentrasi 15 ml l-1
, konsentrasi 20 ml l-1
. Faktor kedua yaitu
perlakuan pengaplikasian pupuk kandang kambing yang diberikan 1 kali pada saat
pengolahan lahan dengan dosis 10 ton ha-1
, 15 ton ha-1
. Kedua faktor tersebut
dikombinasikan sehingga diperoleh 9 kombinasi perlakuan dengan 3 kali ulangan
sehingga didapatkan 27 unit percobaan. Setiap perlakuan terdiri dari 56 tanaman,
sehingga total tanaman dalam percobaan sebanyak 1.512 tanaman. Variabel
pengamatan yang diamati yaitu tinggi tanaman, jumlah daun, luas daun, umur
berbunga, umur terbentuk polong, jumlah polong per tanaman, bobot polong per
tanaman, bobot biji per tanaman, bobot kering tanaman dan hasil panen. Hasil data
pengamatan dianalisis dengan menggunakan Analysis of Variance (ANOVA) atau
uji F Hitung pada taraf 5%. Jika hasil analisis ragam menunjukkan adanya
perbedaan nyata, maka dilakukan uji BNJ (Beda Nyata Jujur) dengan taraf 5%
untuk mengetahui perbedaan antar perlakuan.
Hasil analisis ragam menunjukkan bahwa tidak terdapat interaksi antara
pemberian konsentrasi PGPR dan pupuk kandang kambing. Pengaplikasian
konsentrasi PGPR 20 ml l-1
dapat meningkatkan variabel pertumbuhan tanaman
kedelai mulai dari tinggi tanaman, jumlah daun, umur berbunga, umur terbentuk
polong dan luas daun. Peningkatan pada variabel hasil meliputi jumlah polong per
tanaman, bobot polong per tanaman, bobot kering total per tanaman dan bobot biji per tanaman. Pemberian dosis pupuk kandang kambing 15 ton ha-1 mampu
meningkatkan variabel pertumbuhan tanaman kedelai diantaranya tinggi tanaman,
jumlah daun dan luas daun. Peningkatan pada variabel hasil diantaranya yaitu
jumlah polong per tanaman, bobot polong per tanaman, bobot kering total per
tanaman dan bobot biji per tanaman
Population‐based cohort study of outcomes following cholecystectomy for benign gallbladder diseases
Background The aim was to describe the management of benign gallbladder disease and identify characteristics associated with all‐cause 30‐day readmissions and complications in a prospective population‐based cohort. Methods Data were collected on consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing all‐cause 30‐day readmissions and complications were analysed by means of multilevel, multivariable logistic regression modelling using a two‐level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 8909 patients undergoing cholecystectomy from 167 hospitals. Some 1451 cholecystectomies (16·3 per cent) were performed as an emergency, 4165 (46·8 per cent) as elective operations, and 3293 patients (37·0 per cent) had had at least one previous emergency admission, but had surgery on a delayed basis. The readmission and complication rates at 30 days were 7·1 per cent (633 of 8909) and 10·8 per cent (962 of 8909) respectively. Both readmissions and complications were independently associated with increasing ASA fitness grade, duration of surgery, and increasing numbers of emergency admissions with gallbladder disease before cholecystectomy. No identifiable hospital characteristics were linked to readmissions and complications. Conclusion Readmissions and complications following cholecystectomy are common and associated with patient and disease characteristics
Burden of Culture Confirmed Enteric Fever Cases in Karachi, Pakistan: Surveillance for Enteric Fever in Asia Project (SEAP), 2016-2019
Background: The Surveillance for Enteric Fever in Asia Project (SEAP) is a multicenter, multicountry study conducted in Pakistan, Nepal, and Bangladesh. The objectives of the study were to characterize disease incidence among patients with enteric fever. We report the burden of enteric fever at selected sites of Karachi, Pakistan. Methods: During September 2016 to September 2019, prospective surveillance was conducted at inpatient, outpatient, surgical departments, and laboratory networks of Aga Khan University Hospital, Kharadar General Hospital, and surgery units of National Institute of Child Health and Jinnah Postgraduate Medical Centre. Socio-demographic, clinical, and laboratory data were obtained from all suspected or confirmed enteric fever cases. Results: Overall, 22% (2230/10†094) of patients enrolled were culture-positive for enteric fever. 94% (2093/2230) of isolates were Salmonella Typhi and 6% (137/2230) were S. Paratyphi. 15% of isolates multi-drug resistant (MDR) to first-line antibiotics and 60% were extensively drug-resistant (XDR), resistant to first-line antibiotics, fluoroquinolones and third generation cephalosporin. Conclusion: Enteric fever cases have increased during the last 3 years with large proportion of drug resistant S. Typhi cases. However, the burden of paratyphoid is still relatively low. Strengthening the existing surveillance system for enteric fever and antimicrobial resistance at the national level is recommended in Pakistan to inform prevention measures. While typhoid vaccination can significantly decrease the burden of typhoid and may also impact antimicrobial resistance, water, sanitation, and hygiene improvement is highly recommended to prevent the spread of enteric fever
Prognostic model to predict postoperative acute kidney injury in patients undergoing major gastrointestinal surgery based on a national prospective observational cohort study.
Background: Acute illness, existing co-morbidities and surgical stress response can all contribute to postoperative acute kidney injury (AKI) in patients undergoing major gastrointestinal surgery. The aim of this study was prospectively to develop a pragmatic prognostic model to stratify patients according to risk of developing AKI after major gastrointestinal surgery. Methods: This prospective multicentre cohort study included consecutive adults undergoing elective or emergency gastrointestinal resection, liver resection or stoma reversal in 2-week blocks over a continuous 3-month period. The primary outcome was the rate of AKI within 7 days of surgery. Bootstrap stability was used to select clinically plausible risk factors into the model. Internal model validation was carried out by bootstrap validation. Results: A total of 4544 patients were included across 173 centres in the UK and Ireland. The overall rate of AKI was 14·2 per cent (646 of 4544) and the 30-day mortality rate was 1·8 per cent (84 of 4544). Stage 1 AKI was significantly associated with 30-day mortality (unadjusted odds ratio 7·61, 95 per cent c.i. 4·49 to 12·90; P < 0·001), with increasing odds of death with each AKI stage. Six variables were selected for inclusion in the prognostic model: age, sex, ASA grade, preoperative estimated glomerular filtration rate, planned open surgery and preoperative use of either an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker. Internal validation demonstrated good model discrimination (c-statistic 0·65). Discussion: Following major gastrointestinal surgery, AKI occurred in one in seven patients. This preoperative prognostic model identified patients at high risk of postoperative AKI. Validation in an independent data set is required to ensure generalizability
