    Get PDF
This is the final version. Available from IDOSI Publications via the link in this recordGenre- based studies have recently focused on different types of professional and academic texts to identify their communicative purpose(s), generic structure and linguistic features. However, analyzing the generic and linguistic structure of book prefaces as a subcategory of academic genres seems to have been significantly underresearched. This study focuses on academic book prefaces in three disciplines of management, metallurgy and mathematics to identify their generic structure, linguistic features and potential disciplinary differences. Identifying similar generic structure for book prefaces in the three disciplines, the researchers suggest that book prefaces in the above-mentioned areas are very similar in terms of their communicative purpose(s), generic structure and readers' expectation. Furthermore, the study did not identify any significant difference in the use of the moves and steps among the book prefaces in the three disciplines on the whole, which suggests that writers in the three disciplines generally use the same moves and steps with rather similar frequencies. Finally, analyzing the generic structure and linguistic features of book prefaces indicated that book prefaces fulfill a promotional purpose besides their common informative purpose

Introduction: The purpose of this article was to design a bilayer floating- drug delivery system using mucoadhesive microparticles of olanzapine as a model drug. Methods and Results: Mucoadhesive Microparticles were fabricated with coacervation method by using of mucoadhesive polymers such as sodium bicarbonate, chitosan and ethyl cellulose. The floating layer containing sodium bicarbonate, HPMC, carbapol, PVP and olanzapine mucoadhesive microparticles was compressed and an immediate release layer was added and then both layers were compressed. Floating properties of the tablets, the in vitro drug release, buoyancy lag-time and swelling index were evaluated. The results showed that incorporation of olanzapine mucoadhesive microparticles in floating tablet improved the release kinetic to biphasic so the drug was immediately release in 30 min after floating and lasted more than 8 hours. Also the release of olnzapine was  based on zero order kinetic. This suggested the synergic effect of micro particulate system as well as floating property in long time drug release in gastric fluid. Conclusions: This kind of tablet is suitable for long term delivery of olanzapine in upper gastric parts or segments

Background: Microsatellite instability, the main genetic element in HNPCC syndrome, is associated with a number of cancers, including ovarian epithelial carcinomas. These cancers have distinct characteristics compared to non-MSI related ones. Objectives: The present study aims at determining the prevalence of microsatellite instability in ovarian carcinomas and their associated factors in Iranian patients. Methods: Paraffin-embedded blocks, belonging to 37 patients with definite diagnosis of ovarian epithelial cancers, were retrieved from the archives. After DNA extraction from tumor tissue and PCR reaction, the results were assessed in accordance with melting curve analysis. Subsequently, the relationship among microsatellite status and tumor histology, grade, stage, and size were investigated statistically. Results: The predominant histological type was serous histology. Four out of 37 carcinomas were microsatellite unstable (10.8) and only 1 was MSI-high type (2.1). The MSI was more frequent among younger patients with unilateral, non-serous histology, non-high grade, and stage I tumors without omental involvement. After statistical analysis, the only significant relationship was found between histological type (non-serous) and microsatellite status. Conclusions: Microsatellite stable and unstable ovarian cancers may have different associations with various factors in a sample of Iranian women. The identification of these characteristics may help narrow down indications to test this prognostic and predictive genetic error. © 2017, Cancer Research Center (CRC), Shahid Beheshti University of Medical Sciences

Introduction: Healthcare professionals should seek the necessary resources to improve the quality of care. Given the cultural, social, and economic differences, in every health care system, there are increasing needs for the adapted versions of clinical practice guidelines (CPGs). This study aimed to introduce an innovative CPG adaptation approach for nurses working in a developing country. Methods: This study is comprised of three sections: a. An extensive systematic search of the literature, b. The adaptation process, c. Interviews, which were held with stakeholders, users and/or the target population. We applied all of the stages of guideline adaptation process according to Adaptation Resource Toolkit, with the integration of the findings of a systematic literature search and a qualitative content analysis in an "adolopted" new CPG. Results: In this study, we applied a mix of three methods namely adoption, adaptation and development (adolopment) of recommendations to save time, cost, and manpower efficiently. Moreover, we integrated the utilized qualitative research method and literature review with the adolopment approach to develop the recommendations. Conclusion: Given there is a paucity of nursing clinical practice guidelines (NCPGs) in the nursing management of cancer therapy-induced mucositis and to save time and costs, the findings emerging from the adoption, adaptation, and de novo guideline development by a panel of experts and qualitative content analysis (QCA) method were integrated to achieve a more comprehensive nursing practice guideline

Background. Primary pulmonary artery sarcomas are very rare and their histologic type, called leiomyosarcoma, is even rarer. Case Report. A 64-year-old woman presented with progressive weakness, fatigue, malaise, and dyspnea, and a marked elevation of pulmonary artery pressure was admitted. She was initially diagnosed with chronic pulmonary thromboembolism and chest computed tomography (CT) scan revealed that lobulated heterogeneous left hilar mass extended to precarinal and subcarinal space. MRI demonstrated a polypoid lesion at trunk with extension to left main pulmonary artery and its first branch. She was operated, a yellowish-shiny solid mass in pulmonary trunk was seen intraoperatively, and pulmonary endarterectomy was performed. Her tumor was pathologically diagnosed as pulmonary artery leiomyosarcoma. She died 3 months later after one chemotherapy course. Conclusion. Initially, the patient underwent surgery due to pulmonary embolism but, during the operation, the observed mass increased the probability of pulmonary artery neoplasm. Clinicians must consider pulmonary artery sarcoma when making the differential diagnosis for patients with pulmonary arteries masses. In addition to clinical prediction scores and CT and MRI findings to identify the patients with pulmonary artery sarcoma, PET scanning is the diagnosis of choice in differentiating embolism and neoplasm and is strongly recommended in these patients
