    Epiluminescence microscopy for the diagnosis of doubtful melanocytic skin lesions: Comparison of the ABCD rule of dermatoscopy and a new 7-point checklist based on pattern analysis

    Objective: To compare the reliability of a new 7-point checklist based on simplified epiluminescence microscopy (ELM) pattern analysis with the ABCD rule of dermatoscopy and standard pattern analysis for the diagnosis of clinically doubtful melanocytic skin lesions. Design: In a blind study, ELM images of 342 histologically proven melanocytic skin lesions were evaluated for the presence of 7 standard criteria that we called the 'ELM 7-point checklist.' For each lesion, 'overall' and 'ABCD scored' diagnoses were recorded. From a training set of 57 melanomas and 139 atypical non-melanomas, odds ratios were calculated to create a simple diagnostic model based on identification of major and minor criteria for the '7-point scored' diagnosis. A test set of 60 melanomas and 86 atypical non-melanomas was used for model validation and was then presented to 2 less experienced ELM observers, who recorded the ABCD and 7-point scored diagnoses. Settings: University medical centers. Patients: A sample of patients with excised melanocytic lesions. Main Outcome Measures: Sensitivity, specificity, and accuracy of the models for diagnosing melanoma. Results: From the total combined sets, the 7-point checklist gave a sensitivity of 95% and a specificity of 75% compared with 85% sensitivity and 66% specificity using the ABCD rule and 91% sensitivity and 90% specificity using standard pattern analysis (overall ELM diagnosis). Compared with the ABCD rule, the 7-point method allowed less experienced observers to obtain higher diagnostic accuracy values. Conclusions: The ELM 7-point checklist provides a simplification of standard pattern analysis because of the low number of features to identify and the scoring diagnostic system. As with the ABCD rule, it can be easily learned and easily applied and has proven to be reliable in diagnosing melanoma

    Distribution and phenotype of GJB2 mutations in 102 Sicilian patients with congenital non syndromic sensorineural hearing loss

    Objective: To evaluate the frequency of GJB2 mutations and their correlation with phenotype in Sicilian non-syndromic sensorineural hearing loss (NSHL) patients. Design: Sequencing of the coding region, basal promoter, exon 1, and donor splice site of the GJB2 gene; screening for the presence of the two common GJB6 deletions. Study sample: A cohort of 102 Sicilian NSHL patients. Results: Fifteen different mutations in GJB2 and seventeen different genotypes were detected. No GJB6 mutations were found. The hearing impairment was profound in the 64.72% of probands (mean PTA 0.25 – 4 kHz of 88.82 26.52 dB HL). A total of 81.37% of patients harboured at least one c.35delG allele; c.167delT and c.-23 1G A were identifi ed in 10.78% and the 9.8% of patients respectively; c.35delG homozygotes presented more severe hearing impairment (75.59% of profound hearing loss) and a higher mean PTA 0.25 – 4 kHz (96.79 21.11 dB HL) with respect to c.35delG/non-c.35delG and c.35delG/Wt patients (P 0.05). Conclusions: This work underlines the role of c.35delG, c.167delT and c.-23 1G A as the most frequent causes of NSHL in Sicily. The c.35delG frequency found is similar to those reported in other populations of the Mediterranean area. The analysis of genetic and audiologic data confi rmed a variability in the phenotype associated to a single genotype

    Thermocapillary actuation of liquid flow on chemically patterned surfaces

    We have investigated the thermocapillary flow of a Newtonian liquid on hydrophilic microstripes which are lithographically defined on a hydrophobic surface. The speed of the microstreams is studied as a function of the stripe width w, the applied thermal gradient |dT/dx| and the liquid volume V deposited on a connecting reservoir pad. Numerical solutions of the flow speed as a function of downstream position show excellent agreement with experiment. The only adjustable parameter is the inlet film height, which is controlled by the ratio of the reservoir pressure to the shear stress applied to the liquid stream. In the limiting cases where this ratio is either much smaller or much larger than unity, the rivulet speed shows a power law dependency on w, |dT/dx| and V. In this study we demonstrate that thermocapillary driven flow on chemically patterned surfaces can provide an elegant and tunable method for the transport of ultrasmall liquid volumes in emerging microfluidic technologies

    Has VZV epidemiology changed in Italy? Results of a seroprevalence study

    The aim of the study was to evaluate if and how varicella prevalence has changed in Italy. In particular a seroprevalence study was performed, comparing it to similar surveys conducted in pre-immunization era. During 2013–2014, sera obtained from blood samples taken for diagnostic purposes or routine investigations were collected in collaboration with at least one laboratory/center for each region, following the approval of the Ethics Committee. Data were stratified by sex and age. All samples were processed in a national reference laboratory by an immunoassay with high sensitivity and specificity. Statutory notifications, national hospital discharge database and mortality data related to VZV infection were analyzed as well. A total of 3707 sera were collected and tested. In the studied period both incidence and hospitalization rates decreased and about 5 deaths per year have been registered. The seroprevalence decreased in the first year of life in subjects passively protected by their mother, followed by an increase in the following age classes. The overall antibody prevalence was 84%. The comparison with surveys conducted with the same methodology in 1996–1997 and 2003–2004 showed significant differences in age groups 1–19 y. The study confirms that in Italy VZV infection typically occurs in children. The impact of varicella on Italian population is changing. The comparison between studies performed in different periods shows a significant increase of seropositivity in age class 1–4 years, expression of vaccine interventions already adopted in some regions

    Circulating Cell-Free DNA in Dogs with Mammary Tumors: Short and Long Fragments and Integrity Index

    Circulating cell-free DNA (cfDNA) has been considered an interesting diagnostic/prognostic plasma biomarker in tumor-bearing subjects. In cancer patients, cfDNA can hypothetically derive from tumor necrosis/apoptosis, lysed circulating cells, and some yet unrevealed mechanisms of active release. This study aimed to preliminarily analyze cfDNA in dogs with canine mammary tumors (CMTs). Forty-four neoplastic, 17 non-neoplastic disease-bearing, and 15 healthy dogs were recruited. Necrosis and apoptosis were also assessed as potential source of cfDNA on 78 CMTs diagnosed from the 44 dogs. The cfDNA fragments and integrity index significantly differentiated neoplastic versus non-neoplastic dogs (P<0.05), and allowed the distinction between benign and malignant lesions (P<0.05). Even if without statistical significance, the amount of cfDNA was also affected by tumor necrosis and correlated with tumor size and apoptotic markers expression. A significant (P<0.01) increase of Bcl-2 in malignant tumors was observed, and in metastatic CMTs the evasion of apoptosis was also suggested. This study, therefore, provides evidence that cfDNA could be a diagnostic marker in dogs carrying mammary nodules suggesting that its potential application in early diagnostic procedures should be further investigated

    Neuronal nitric oxide synthase (NOS I) in the buffalo epididymis

    The localization of neuronal nitric oxide (NOS I) in the buffalo epididymis have been investigated by nicotinamide adenite dinucleotide phosphatase-diaphorase (NADPH-d) histochemichemistry to the light microscope (LM) and NOS immunoistochemistry to the scanning electron microscope (SEM), respectively. Histochemistry: examination of epididymis specimens revealed an intense NADPH-d staining in the basal cell epithelium and endothelium cells of blood vessel. The NADPH diaphorase staining was diffuse and granular only along the caput epididymal epithelium. NADPH diaphorase staining was less intense or absent in the corpus and in the cauda of epididymis. Dense NADPH diaphorase is labeling in the endothelium of blood vessels along the whole buffalo epididymis. Immunoistochemistry: intense NOS I immunoreactivity was detected in the caput epididymis specimen by immuno-SEM. The basal epithelium showed intense and wide-spread immunoreactivity. In the corpus and in the cauda of the epididymis not observed NOS I immunoreactivity. The specific localization of NOS I in buffalo epididymis suggest that nitric oxide may be involved to explain epididymal function: maturation and storage

    New insights into North Sea deep crustal structure and extension from transdimensional ambient noise tomography

    SUMMARY The deep crustal structure beneath the North Sea is poorly understood since it is constrained by only a few seismic reflection and refraction profiles. However, it is widely acknowledged that the mid to lower crust plays important roles in rift initiation and evolution, particularly when large-scale sutures and/or terrane boundaries are present, since these inherited features can focus strain or act as inhibitors to extensional deformation. Ancient tectonic features are known to exist beneath the iconic failed rift system of the North Sea, making it an ideal location to investigate the complex interplay between pre-existing regional heterogeneity and rifting. To this end, we produce a 3-D shear wave velocity model from transdimensional ambient seismic noise tomography to constrain crustal properties to ∼30 km depth beneath the North Sea and its surrounding landmasses. Major North Sea sedimentary basins appear as low shear wave velocity zones that are a good match to published sediment thickness maps. We constrain relatively thin crust (13–18 km) beneath the Central Graben depocentres that contrasts with crust elsewhere at least 25–30 km thick. Significant variations in crustal structure and rift symmetry are identified along the failed rift system that appears to be related to the locations of Laurentia–Avalonia–Baltica palaeoplate boundaries. We constrain first-order differences in structure between palaeoplates; with strong lateral gradients in crustal velocity related to Laurentia–Avalonia–Baltica plate juxtaposition and reduced lower crustal velocities in the vicinity of the Thor suture, possibly representing the remnants of a Caledonian accretionary complex. Our results provide fresh insight into the pivotal roles that ancient terranes can play in the formation and failure of continental rifts and may help explain the characteristics of other similar continental rifts globally.</jats:p

    A novel μCT analysis reveals different responses of bioerosion and secondary accretion to environmental variability

    Corals build reefs through accretion of calcium carbonate (CaCO3) skeletons, but net reef growth also depends on bioerosion by grazers and borers and on secondary calcification by crustose coralline algae and other calcifying invertebrates. However, traditional field methods for quantifying secondary accretion and bioerosion confound both processes, do not measure them on the same time-scale, or are restricted to 2D methods. In a prior study, we compared multiple environmental drivers of net erosion using pre- and post-deployment micro-computed tomography scans (μCT; calculated as the % change in volume of experimental CaCO3 blocks) and found a shift from net accretion to net erosion with increasing ocean acidity. Here, we present a novel μCT method and detail a procedure that aligns and digitally subtracts pre- and post-deployment μCT scans and measures the simultaneous response of secondary accretion and bioerosion on blocks exposed to the same environmental variation over the same time-scale. We tested our method on a dataset from a prior study and show that it can be used to uncover information previously unattainable using traditional methods. We demonstrated that secondary accretion and bioerosion are driven by different environmental parameters, bioerosion is more sensitive to ocean acidity than secondary accretion, and net erosion is driven more by changes in bioerosion than secondary accretion

    Weakly nonlinear theory for a gate-type curved array in waves

    We analyse the effect of gate surface curvature on the nonlinear behaviour of an array of gates in a semi-infinite channel. Using a perturbation-harmonic expansion, we show the occurrence of new detuning and damping terms in the Ginzburg-Landau evolution equation, which are not present in the case of flat gates. Unlike the case of linearised theories, synchronous excitation of trapped modes is now possible because of interactions between the wave field and the curved boundaries at higher orders. Finally, we apply the theory to the case of surging wave energy converters (WECs) with curved geometry and show that the effects of nonlinear synchronous resonance are substantial for design purposes. Conversely, in the case of subharmonic resonance we show that the effects of surface curvature are not always beneficial as previously thought
