Microangiopathic complications in type 1 diabetes mellitus: differences in severity when isolated or associated with autoimmune polyendocrinopathies

CONTEXT: The development and evolution of different chronic diabetic complications may present variations among the different types and conditions of this disease.OBJECTIVE: To evaluate the degree of microangiopathy in Type 1 diabetes mellitus (DM1) associated with autoimmune polyendocrinopathies (OSAD) or isolated DM1 (iDM1).PATIENTS: OSAD (n=17) and iDM1 (n=13) were over 15 years old at diagnosis of DM and were matched for diabetes duration (13.9 ± 8.2 and 13.2 ± 5.9 years, respectively) and metabolic control (HbA1c: 6.4 ± 1.9 and 6.8 ± 1.4%).MAIN OUTCOME MEASURES: Urinary albumin excretion (UAE; ELISA), the inversion of serum creatinine (1/C) level and indirect ophthalmoscopy. RESULTS: Although the prevalence of hypertension was similar in both groups, the OSAD had inferior levels of UAE (7.4 ± 2.5 vs. 17.3 ± 9.2 µg/min; p< 0.05). Nephropathy was detected in 12% of the OSAD (none of them macroproteinuric) and in 39% of the iDM1 . The UAE in the iDM1 correlated negatively with 1/C values (r= -0.7, p< 0.005), but the same did not occur in the OSAD (r= 0.2, ns). Among patients with retinopathy, the severe form was found in 29% of the OSAD and in 46% of the iDM1.CONCLUSIONS: OSAD was associated with a lower degree of microangiopathy, in spite of age at diagnosis, duration of diabetes and the metabolic control. In contrast with the iDM1 , the increase in UAE of OSAD was not associated with reductions in GFR.CONTEXTO: O desenvolvimento e evolução das complicações crônicas do diabetes mellitus podem apresentar diiferenças entre os vários tipos e condições desta doença. OBJETIVO: Avaliar o grau de microangiopatia em pacientes com diabetes mellitus do Tipo 1 (DM1) associado ( OSAD) ou não (DM1i) a endocrinopatias auto-imunes. PACIENTES: Os OSAD (n= 17) e os DM1i (n= 13) eram pareados para o tempo de duração do diabetes (13.9 ± 8.2 e 13.2 ± 5.9 anos, respectivamente) e controle metabólico (HbA1c: 6.4 ± 1.9 e 6.8 ± 1.4%). VARIÁVEIS ESTUDADAS: A excreção urinária de albumina (EUA), o inverso da creatinina sérica (1/C) e o grau de retinopatia diabética (oftalmoscopia indireta). RESULTADOS: A prevalência de hipertensão arterial foi semelhante nos grupos e os OSAD tinham níveis inferiores de EUA (7.4 ± 2.5 vs. 17.3 ± 9.2 µg/min; p< 0.05). A nefropatia diabética foi detectada em 12% dos OSAD (nenhum deles com macroproteinúria) e em 39% dos DM1i. Nos DM1i havia uma correlação inversa entre a EUA e o 1/C (r= -0,7, p< 0,005), o mesmo não ocorria no OSAD (r= 0,2, ns). Retinopatia em grau severo: 29% do OSAD e 46% do DM1i. CONCLUSÕES: O presente estudo sugere que os OSAD apresentam uma evolução melhor das microangiopatias quando comparado ao DM1i nas mesmas condições. Effect of Loneliness on Oral Health

Loneliness is one of the biggest concerns related to wellbeing in the current decade. It is yet to be understood are all the possible effects that loneliness can have in someone’s health. In the current study, the relationship between loneliness and oral health is investigated, considering that oral health can be used in many cases to predict one’s overall health. Since loneliness is correlated to depression and anxiety, which both have been shown to negatively impact oral health, it is expected that loneliness would have a negative impact on oral health. The research will be conducted using an online questionnaire administered via SONA systems to collect data of 700 FIU students. Loneliness scales such as the UCLA revised loneliness scale will be used to measure one’s loneliness level. In order to collect participants’ oral health level, modified versions of the scales Pediatric Quality Of Life Inventory Oral Health Scale, Brief Oral Health Status Examination, Hiroshima University Dental Behavioral Inventory and the Blue Cross Blue Shield of Massachusetts Oral Health Self-Assessment will be used. Demographic scale will also be used to measure other variables and no cost will be attributed to the distribution of the questionnaires or the study as a hole. This study will be the first to address the effects of loneliness in college level students’ oral health in America. It is of extremely significance to help the understanding of the connection between loneliness and oral health. This could provide the means for diminishing loneliness and improving oral health and general health in our society, especially considering the increased loneliness frequency in modern society

E-sports e a aplicação da legislação do esporte e do trabalho

Objective: The present study aims to describe the similarities between e-Sports and the conventional Sports, in order to emulate the viability of conventional Sports law to regulate the relations formalized with cyber athletes, at least until the emergence of a specific legal regulation on e-Sports. The interaction between Labor Law and those relations will be focused too, in order to confer social rights security, especially constitutional human rights involved on that debate.Methodology: The article is a theoretical approach that seeks to recognize how the dynamics of e-Sports encompass the legal science of Sports Law, based on the deductive method, supported by market analysis, legal research and bibliographic review.Originality/Relevance: Nowadays some discussions suggest a controversy about the e-Sports legal framework at Brazil, especially about the application of the conventional Sports Law and interaction with the Labor Law discipline. This controversy is not justifiable, either because e-Sports constitute a sport in the authentic sense, or because all work relationships in this field are bound to the legal parameters of adjudication of social rights provided for in the Constitution. This constitutional approach is important because the legitimacy of e-Sports initiatives depends on that, putting on the core of the business consider the real legal limits emerging of the State regulations applicably incidents on contracts signed with cyber athletes. In the course of Revolution 4.0, those approximations between conventional ways and electronic ones will be the tonic of the society transition onto technological new way of life. Describing those interactions is essential to provide new ways of State´s legal regulations, attributing liberty on business dynamics and ensuring social rights on the labor relations. The parameters of ethics on business here emerges as exigencies by ESG (Environment, Social and Governance) administration models, which is embraced by relevant theories, as the John Elkington´s one, especially on his concept of triple bottom line.Main results: The activity that has been called e-Sports and its vertiginous growth puts pressure on the legal system to reach appropriateness solutions for the controversies that arises from the relationships between players, clubs, associations and all of stakeholders rounding this innovative market. Immediately, the application of Sports and Labor Law emerges as a palliative solution, but specific new regulations are necessary, in order to provide development ways to the businesses, focusing on liberty for the market dynamics and on security for the social rights.Theoretical/methodological contributions: The article seeks to emulate the attraction of sports and labor legislation for e-Sports regency, guaranteeing legal certainty to the already established relationships, and developing increase ways for business on this market, with citizens social rights preservation, especially constitutional ones.Objetivo de estudio: El presente estudio tiene como objetivo describir las similitudes entre los e-Sports y los deportes convencionales, con el fin de emular la viabilidad del derecho deportivo convencional para regular las relaciones formalizadas con los cyber deportistas, al menos hasta que surja una normativa legal específica sobre los e-Sports. También se enfocará la interacción entre el Derecho del Trabajo y estas relaciones, con el fin de dar seguridad a los derechos sociales, en especial a los derechos humanos constitucionales involucrados en este debate.Metodología/enfoque: El artículo es un abordaje teórico que busca reconocer cómo la dinámica de los e-Sports engloba la ciencia jurídica del Derecho del Deporte, a partir del método deductivo, apoyado en el análisis de mercado, la investigación jurídica y la revisión bibliográfica.Originalidad/Relevancia: Actualmente, algunas discusiones sugieren una controversia sobre el marco legal de los e-Sports en Brasil, especialmente sobre la aplicación del Derecho Deportivo convencional y la interacción con la disciplina del Derecho del Trabajo. Integration of collagen matrices into the urethra when implanted as onlay graft

Objective: To assess the integration of decellularized heterologous collagen matrices into the urethra, when implanted with no cells or when seeded with autologous smooth muscle cells.Materials and Methods: Eighteen New Zealand rabbits were randomly assigned to two groups: Group I (n = 9) - animals undergoing urethral segment resection with interposition of a patch of heterologous collagen matrix seeded with autologous smooth muscle cells; Group II (n = 9) - animals undergoing resection of a urethral segment with interposition of a decellularized heterologous collagen matrix patch. Two animals from each group were sacrificed on postoperative days seven, fourteen and twenty-eight; three animals from each group were sacrificed at the end of three postoperative months. At the end of the third month one animal from each group underwent urethroscopy for urethral integrity assessment and one animal from each group had its microcirculation image captured by a SDF device (Side-stream Dark Field - Microscan Analysis Software). One animal from each group in each euthanasia period underwent cystourethrography so as the urethra could be viewed at flow time. the matrices integration was assessed through histological examination using hematoxylin and eosin (H&E), Masson trichrome (MT), Picrosirius red and Von Willebrand staining. in a blind study with two pathologists all the slides were studied.Results: the matrices whether seeded or not with autologous muscle cells were able to restore the architecture of the urethra, but were eliminated from the first week on, before incorporation. Presentaré mi primer libro El arrastre con el objetivo de analizar su composición a través de los diferentes apartados en los que se divide el presente. El apartado del motivo de la herida es el cual tomaré de manera más extendida para el análisis de mi trabajo, siendo el más importante ya que es la herida el motivo que atraviesa el libro-objeto, pasando a través de todos sus poemas. Se ve esta herida compuesta por pequeñas llagas que forman una gran herida que se aprecia a medida que se va leyendo el trabajo. Esto para descubrir a el/la sujeto tan herido física y psicológicamente que se fracturó la psique La importancia de este prefacio es la presentación y análisis del motivo de la herida, para dar una posible explicación de cómo es el libro en general.The main objective of this preface is, in addition of introduce my first book El arrastre, analyze its composition through the different sections in those who the present divide. The section of the motif of the wound is the component that I will take way more spread out for it’s analysis, being the most important since is the wound the motif that goes through the object-book, going through all its poems. This wound compound a big wound that can be appreciated to measure by reading the work. This to discover the subject so injured physical and psychologically that their psyche broke. Complexo de Édipo invertido: casos clínicos a partir de uma visão psicanalítica

O presente trabalho tem como objetivo trilhar o tema da homossexualidade nas obras de Freud, iniciando em 1900 sobre “A interpretação dos sonhos”, até ponto crucial em que estabelece a homossexualidade como uma saída pelo complexo de édipo, chamando-a de édipo “invertido”. Seguindo o pensamento de Freud, o complexo de édipo será aprofundado e melhor esclarecido entre complexo de édipo positivo, da qual sairá a heterossexualidade; e o complexo de édipo negativo, da qual sairá a homossexualidade, ponto principal desse trabalho. O que suscitou um primeiro interesse nesse tema, foram casos atendidos em consultório particular, que em comum apresentavam a homossexualidade, suscitando uma hipótese para uma homossexualidade identificada com o feminino e o outro caso uma homossexualidade de cunho narcísico. A partir dessas hipóteses recorri a teoria psicanalítica podendo confirmar a interlocução da clínica com a teoria. E ainda foi possível pensar que, em ambos os casos, a transformação e a elaboração do complexo de édipo pode continuar a acontecer, no sentido de um encontro com o sujeito autônomoThe present work aims to trace the theme of homosexuality in the works of Freud, beginning in 1900 on "The interpretation of dreams", to the crucial point in which it establishes homosexuality as an exit through the complex of oedipus, calling it inverted oedipus. Following Freud's thought, the oedipus complex will be deepened and better clarified between a positive oedipus complex from which heterosexuality will emerge; and the negative oedipus complex, from which homosexuality will emerge, the main point of this work. What aroused a first interest in this subject, were cases attended in private practice, which in common presented homosexuality, raising a hypothesis for a homosexuality identified with the feminine and the other case a homosexuality of a narcissistic nature. From these hypotheses, I turned to psychoanalytic theory and could confirm the interlocution of the clinic with the theory. FUNCIONAMENTO DA REDE DE APOIO ÀS ESCOLAS DE EDUCAÇÃO INFANTIL DE TIBERÍADES: RELATO DE EXPERIÊNCIA DE UM PSICÓLOGO EDUCACIONAL

Neste artigo descrevo a minha experiência como psicólogo educacional no contexto do que habitualmente educadores denominam “rede de apoio”, situada na cidade de Tiberíades, ao norte de Israel. As redes de apoio, salvaguardadas suas especificidades, consistem na utilização dos recursos comunitários para cuidar, primordialmente, do desenvolvimento de alunos da rede escolar. Como psicólogo educacional, estou lotado no Serviço de Psicologia Educacional – um dos componentes da rede - e minha atuação abrange exclusivamente 14 (catorze) das 48 (quarenta e oito) escolas de Educação Infantil (Jardins) da cidade. Os casos apresentados têm tripla finalidade: expor as principais demandas dos Jardins, elucidar o funcionamento da rede de apoio nos seus aspectos gerais e exemplificar algumas possibilidades de atuação do psicólogo educacional nas escolas de Educação Infantil
