    An Equilibrium problem with an equilibrium constraint is a mathematical construct that can be applied to private competition in highway networks. In this paper we consider the problem of finding a Nash Equilibrium regarding competition in toll pricing on a network utilising 2 alternative algorithms. In the first algorithm, we utilise a Gauss Siedel fixed point approach based on the cutting constraint algorithm for toll pricing. In the second algorithm, we extend an existing sequential linear complementarity approach for finding Nash equilibrium subject to Wardrop Equilibrium constraints. Finally we consider how the equilibrium may change between the Nash competitive equilibrium and a collusive equilibrium where the two players co-operate to form the equivalent of a monopoly operation

    Effectiveness of anonymised information sharing and use in health service, police, and local government partnership for preventing violence related injury: experimental study and time series analysis

    Objective: To evaluate the effectiveness of anonymised information sharing to prevent injury related to violence. Design: Experimental study and time series analysis of a prototype community partnership between the health service, police, and local government partners designed to prevent violence. Setting: Cardiff, Wales, and 14 comparison cities designated "most similar" by the Home Office in England and Wales. Intervention After a 33 month development period, anonymised data relevant to violence prevention (precise violence location, time, days, and weapons) from patients attending emergency departments in Cardiff and reporting injury from violence were shared over 51 months with police and local authority partners and used to target resources for violence prevention. Main outcome measures: Health service records of hospital admissions related to violence and police records of woundings and less serious assaults in Cardiff and other cities after adjustment for potential confounders. Results: Information sharing and use were associated with a substantial and significant reduction in hospital admissions related to violence. In the intervention city (Cardiff) rates fell from seven to five a month per 100 000 population compared with an increase from five to eight in comparison cities (adjusted incidence rate ratio 0.58, 95% confidence interval 0.49 to 0.69). Average rate of woundings recorded by the police changed from 54 to 82 a month per 100 000 population in Cardiff compared with an increase from 54 to 114 in comparison cities (adjusted incidence rate ratio 0.68, 0.61 to 0.75). There was a significant increase in less serious assaults recorded by the police, from 15 to 20 a month per 100 000 population in Cardiff compared with a decrease from 42 to 33 in comparison cities (adjusted incidence rate ratio 1.38, 1.13 to 1.70). Conclusion: An information sharing partnership between health services, police, and local government in Cardiff, Wales, altered policing and other strategies to prevent violence based on information collected from patients treated in emergency departments after injury sustained in violence. This intervention led to a significant reduction in violent injury and was associated with an increase in police recording of minor assaults in Cardiff compared with similar cities in England and Wales where this intervention was not implemented

    An Equilibrium Problem with an Equilibrium Constraint (EPEC) is a mathematical construct that can be applied to private competition in highway networks. In this paper we consider the problem of finding a Nash Equilibrium in a situation of competition in toll pricing on a network utilising two alternative algorithms. In the first algorithm, we utilise a Gauss Seidel fixed point approach based on the cutting constraint algorithm for toll pricing. The second algorithm that we propose, a novel contribution of this paper, is the extension of an existing sequential linear complementarity programming approach for finding the competitive Nash equilibrium when there is a lower level equilibrium constraint. Finally we develop an intuitive approach to represent collusion between players and demonstrate that as the level of collusion goes from none to full collusion so the solution maps from the Nash to monopolistic solution. However we also show that there may be local solutions for the collusive monopoly which lie closer to the second best welfare toll solution than does the competitive Nash equilibrium

    A rank based social norms model of how people judge their levels of drunkenness whilst intoxicated

    Background: A rank based social norms model predicts that drinkers’ judgements about their drinking will be based on the rank of their breath alcohol level amongst that of others in the immediate environment, rather than their actual breath alcohol level, with lower relative rank associated with greater feelings of safety. This study tested this hypothesis and examined how people judge their levels of drunkenness and the health consequences of their drinking whilst they are intoxicated in social drinking environments. Methods: Breath alcohol testing of 1,862 people (mean age = 26.96 years; 61.86 % male) in drinking environments. A subset (N = 400) also answered four questions asking about their perceptions of their drunkenness and the health consequences of their drinking (plus background measures). Results: Perceptions of drunkenness and the health consequences of drinking were regressed on: (a) breath alcohol level, (b) the rank of the breath alcohol level amongst that of others in the same environment, and (c) covariates. Only rank of breath alcohol level predicted perceptions: How drunk they felt (b 3.78, 95 % CI 1.69 5.87), how extreme they regarded their drinking that night (b 3.7, 95 % CI 1.3 6.20), how at risk their long-term health was due to their current level of drinking (b 4.1, 95 % CI 0.2 8.0) and how likely they felt they would experience liver cirrhosis (b 4.8. 95 % CI 0.7 8.8). People were more influenced by more sober others than by more drunk others. Conclusion: Whilst intoxicated and in drinking environments, people base judgements regarding their drinking on how their level of intoxication ranks relative to that of others of the same gender around them, not on their actual levels of intoxication. Thus, when in the company of others who are intoxicated, drinkers were found to be more likely to underestimate their own level of drinking, drunkenness and associated risks. The implications of these results, for example that increasing the numbers of sober people in night time environments could improve subjective assessments of drunkenness, are discussed

    Alcohol and the dental team:Relevance, risk, role and responsibility

    An enquiry about alcohol use, whenever a patient presents for dental treatment, is now firmly established within the taking of a social history. Dental professionals are well placed to provide relevant alcohol advice. Indeed, it is now embedded within the training of undergraduates as required by the General Dental Council (GDC) in Preparing for Practice. 1 Practitioners therefore need to be aware of recent changes in alcohol guidelines commissioned by the UK Chief Medical Officers. This paper explores alcohol-related harm, screening tools to facilitate an enquiry, and our roles and responsibilities for providing alcohol advice accepting the limited time available within the dental appointment. CPD/Clinical Relevance: Alcohol has both local and systemic effects. Understanding these effects, the recently updated guidelines and available screening tools are important steps towards supporting dental professionals in the provision of alcohol-related advice. </jats:p

    Bactericidal action of positive and negative ions in air

    In recent years there has been renewed interest in the use of air ionisers to control of the spread of airborne infection. One characteristic of air ions which has been widely reported is their apparent biocidal action. However, whilst the body of evidence suggests a biocidal effect in the presence of air ions the physical and biological mechanisms involved remain unclear. In particular, it is not clear which of several possible mechanisms of electrical origin (i.e. the action of the ions, the production of ozone, or the action of the electric field) are responsible for cell death. A study was therefore undertaken to clarify this issue and to determine the physical mechanisms associated with microbial cell death. In the study seven bacterial species (Staphylococcus aureus, Mycobacterium parafortuitum, Pseudomonas aeruginosa, Acinetobacter baumanii, Burkholderia cenocepacia, Bacillus subtilis and Serratia marcescens) were exposed to both positive and negative ions in the presence of air. In order to distinguish between effects arising from: (i) the action of the air ions; (ii) the action of the electric field, and (iii) the action of ozone, two interventions were made. The first intervention involved placing a thin mica sheet between the ionisation source and the bacteria, directly over the agar plates. This intervention, while leaving the electric field unaltered, prevented the air ions from reaching the microbial samples. In addition, the mica plate prevented ozone produced from reaching the bacteria. The second intervention involved placing an earthed wire mesh directly above the agar plates. This prevented both the electric field and the air ions from impacting on the bacteria, while allowing any ozone present to reach the agar plate. With the exception of Mycobacterium parafortuitum, the principal cause of cell death amongst the bacteria studied was exposure to ozone, with electroporation playing a secondary role. However in the case of Mycobacterium parafortuitum, electroporation resulting from exposure to the electric field appears to have been the principal cause of cell inactivation. The results of the study suggest that the bactericidal action attributed to negative air ions by previous researchers may have been overestimated

    On a role of quadruple component of magnetic field in defining solar activity in grand cycles

    In this paper we revise our prediction of solar activity using a solar background magnetic field as a proxy by the inclusion of eigen vectors of solar magnetic waves produced by quadruple magnetic sources, in addition to the principal eigen modes generated by two-layer dipole sources (Zharkova et al., 2015). By considering the interference of two dipole and one quadruple waves we produce the revised summary curve for the last 400 years accounting for the additional minima of solar activity occurred at the beginning of 19th (Dalton minimum) and 20th centuries. Using the dynamo model with meridional circulation and selecting the directions of circulation for quadruple waves, we estimate the parameters of quadrupole waves best fitting the observations in the past grand cycle. The comparison shows that the quadruple wave has to be generated in the inner layer of the solar convective zone, in order to provide the additional minima observed in 19 and 20 centuries, thus, naturally accounting for Gleissberg centennial cycle. The dynamo wave simulated for the dipole and quadruple sources reveals much closer correspondence of the resulting summary curve derived from the principal components of magnetic field variations to the solar activity oscillations derived from the average sunspot numbers in the current grand cycle

    All-Wales licensed premises intervention (AWLPI): a randomised controlled trial to reduce alcohol-related violence

    Background: Alcohol-related violence in and in the vicinity of licensed premises continues to place a considerable burden on the United Kingdom’s (UK) health services. Robust interventions targeted at licensed premises are therefore required to reduce the costs of alcohol-related harm. Previous evaluations of interventions in licensed premises have a number of methodological limitations and none have been conducted in the UK. The aim of the trial was to determine the effectiveness of the Safety Management in Licensed Environments intervention designed to reduce alcohol-related violence in licensed premises, delivered by Environmental Health Officers, under their statutory authority to intervene in cases of violence in the workplace.&lt;p&gt;&lt;/p&gt; Methods/Design: A national randomised controlled trial, with licensed premises as the unit of allocation. Premises were identified from all 22 Local Authorities in Wales. Eligible premises were those with identifiable violent incidents on premises, using police recorded violence data. Premises were allocated to intervention or control by optimally balancing by Environmental Health Officer capacity in each Local Authority, number of violent incidents in the 12 months leading up to the start of the project and opening hours. The primary outcome measure is the difference in frequency of violence between intervention and control premises over a 12 month follow-up period, based on a recurrent event model. The trial incorporates an embedded process evaluation to assess intervention implementation, fidelity, reach and reception, and to interpret outcome effects, as well as investigate its economic impact.&lt;p&gt;&lt;/p&gt; Discussion: The results of the trial will be applicable to all statutory authorities directly involved with managing violence in the night time economy and will provide the first formal test of Health and Safety policy in this environment. If successful, opportunities for replication and generalisation will be considered.&lt;p&gt;&lt;/p&gt
