Baret & Caballero v. France: unanimous refusal of access to posthumous reproduction with an uneasy aftermath
Given that the Strasbourg Club dedicated a discussion to the interesting case of Baret & Caballero v. France so recently, I can imagine that any reader would think: do we really need another blog post on this French case so soon? As an embryo law enthusiast, I must give a biased ‘yes’. This case not only addresses the meaning of reproductive rights under Article 8 of the European Convention on Human Rights (the Convention) but also raises questions about how the European Court of Human Rights (the Court) assessed the applicants’ access to (assisted) reproduction technologies under the notion of a ‘wide margin of appreciation.
Value of cyclin A immunohistochemistry for cancer risk stratification in Barrett esophagus surveillance A multicenter case-control study
The value of endoscopic Barrett esophagus (BE) surveillance based on histological diagnosis of low-grade dysplasia (LGD) remains
debated given the lack of adequate risk stratification. The aim of this study was to evaluate the predictive value of cyclin A expression
and to combine these results with our previously reported immunohistochemical p53, AMACR, and SOX2 data, to identify a panel of
biomarkers predicting neoplastic progression in BE.
We conducted a case–control study within a prospective cohort of 720 BE patients. BE patients who progressed to high-grade
dysplasia (HGD, n=37) or esophageal adenocarcinoma (EAC, n=13), defined as neoplastic progression, were classified as cases
and patients without neoplastic progression were classified as controls (n=575). Cyclin A expression was determined by
immunohistochemistry in all 625 patients; these results were combined with the histological diagnosis and our previous p53,
AMACR, and SOX2 data in loglinear regression models. Differences in discriminatory ability were quantified as changes in area under
the ROC curve (AUC) for predicting neoplastic progression.
Cyclin A surface positivity significantly increased throughout the metaplasia–dysplasia–carcinoma sequences and was seen in 10%
(107/1050) of biopsy series without dysplasia, 33% (109/335) in LGD, and 69% (34/50) in HGD/EAC. Positive cyclin A expression was
associated with an increased risk of neoplastic progression (adjusted relative risk (RRa) 2.4; 95% CI: 1.7–3.4). Increases in AUC were
substantial for P53 (+0.05), smaller for SOX2 (+0.014), minor for cyclin A (+0.003), and none for AMARC (0.00).
Cyclin A immunopositivity was associated with an increased progression risk in BE patients. However, compared to p53 and SOX2,
the incremental value of cyclin A was limited. The use of biomarkers has the potential to significantly improve risk stratification in BE
Suspected Lynch syndrome associated MSH6 variants: A functional assay to determine their pathogenicity
Lynch syndrome (LS) is a hereditary cancer predisposition caused by inactivating mutations in DNA mismatch repair (MMR) genes. Mutations in the MSH6 DNA MMR gene account for approximately 18% of LS cases. Many LS-associated sequence variants are nonsense and frameshift mutations that clearly abrogate MMR activity. However, missense mutations whose functional implications are unclear are also frequently seen in suspected-LS patients. To conclusively diagnose LS and enroll patients in appropriate surveillance programs to reduce morbidity as well as mortality, the functional consequences of these variants of uncertain clinical significance (VUS) must be defined. We present an oligonucleotide-directed mutagenesis screen for the identification of pathogenic MSH6 VUS. In the screen, the MSH6 variant of interest is introduced into mouse embryonic stem cells by site-directed mutagenesis. Subsequent selection for MMR-deficient cells using the DNA damaging agent 6-thioguanine (6TG) allows the identification of MMR abrogating VUS because solely MMR-deficient cells survive 6TG exposure. We demonstrate the efficacy of the genetic screen, investigate the phenotype of 26 MSH6 VUS and compare our screening results to clinical data from suspected-LS patients carrying these variant alleles

Clinical risk factors of colorectal cancer in patients with serrated polyposis syndrome: a multicentre cohort analysis
OBJECTIVE: Serrated polyposis syndrome (SPS) is accompanied by an increased risk of colorectal cancer (CRC). Patients fulfilling the clinical criteria, as defined by the WHO, have a wide variation in CRC risk. We aimed to assess risk factors for CRC in a large cohort of patients with SPS and to evaluate the risk of CRC during surveillance. DESIGN: In this retrospective cohort analysis, all patients with SPS from seven centres in the Netherlands and two in the UK were enrolled. WHO criteria were used to diagnose SPS. Patients who only fulfilled WHO criterion-2, with IBD and/or a known hereditary CRC syndrome were excluded. RESULTS: In total, 434 patients with SPS were included for analysis; 127 (29.3%) were diagnosed with CRC. In a per-patient analysis ≥1 serrated polyp (SP) with dysplasia (OR 2.07; 95% CI 1.28 to 3.33), ≥1 advanced adenoma (OR 2.30; 95% CI 1.47 to 3.67) and the fulfilment of both WHO criteria 1 and 3 (OR 1.60; 95% CI 1.04 to 2.51) were associated with CRC, while a history of smoking was inversely associated with CRC (OR 0.36; 95% CI 0.23 to 0.56). Overall, 260 patients underwent surveillance after clearing of all relevant lesions, during which two patients were diagnosed with CRC, corresponding to 1.9 events/1000 person-years surveillance (95% CI 0.3 to 6.4). CONCLUSION: The presence of SPs containing dysplasia, advanced adenomas and/or combined WHO criteria 1 and 3 phenotype is associated with CRC in patients with SPS. Patients with a history of smoking show a lower risk of CRC, possibly due to a different pathogenesis of disease. The risk of developing CRC during surveillance is lower than previously reported in literature, which may reflect a more mature multicentre cohort with less selection bias
Accuracy of endoscopic staging and targeted biopsies for routine gastric intestinal metaplasia and gastric atrophy evaluation study protocol of a prospective, cohort study: the estimate study
Introduction Patients with chronic atrophic gastritis (CAG)
and intestinal metaplasia (IM) are at risk of developing
gastric adenocarcinoma. Their diagnosis and management
currently rely on histopathological guidance after random
endoscopic biopsy sampling (Sydney biopsy strategy). This
approach has significant flaws such as under-diagnosis,
poor reproducibility and poor correlation between
endoscopy and histology. This prospective, international
multicentre study aims to establish whether endoscopyled risk stratification accurately and reproducibly predicts
CAG and IM extent and disease stage.
Methods and analysis Patients with CAG and/or
IM on standard white light endoscopy (WLE) will be
prospectively identified and invited to undergo a second
endoscopy performed by an expert endoscopist using
enhanced endoscopic imaging techniques with virtual
chromoendoscopy. Extent of CAG/IM will be endoscopically
staged with enhanced imaging and compared with standard
WLE. Histopathological risk stratification through targeted
biopsies will be compared with endoscopic disease staging
and to random biopsy staging on WLE as a reference. At
least 234 patients are required to show a 10% difference
in sensitivity and accuracy between enhanced imaging
endoscopy-led staging and the current biopsy-led staging
protocol of gastric atrophy with a power (beta) of 80% and a
0.05 probability of a type I error (alpha).
Ethics and dissemination The study was approved by the
respective Institutional Review Boards (Netherlands: MEC2018-078; UK: 19/LO/0089). The findings will be published in
peer-reviewed journals and presented at scientific meetings
Recent advances in the detection and management of early gastric cancer and its precursors
Despite declines in incidence, gastric cancer remains a disease with a poor prognosis and limited treatment options due to its often late stage of diagnosis. In contrast, early gastric cancer has a good to excellent prognosis, with 5-year survival rates as high as 92.6% after endoscopic resection. There remains an East-West divide for this disease, with high incidence countries such as Japan seeing earlier diagnoses and reduced mortality, in part thanks to the success of a national screening programme. With missed cancers still prevalent at upper endoscopy in the West, and variable approaches to assessment of the high-risk stomach, the quality of endoscopy we provide must be a focus for improvement, with particular attention paid to the minority of patients at increased cancer risk. High-definition endoscopy with virtual chromoendoscopy is superior to white light endoscopy alone. These enhanced imaging modalities allow the experienced endoscopist to accurately and robustly detect high-risk lesions in the stomach. An endoscopy-led staging strategy would mean biopsies could be targeted to histologically confirm the endoscopic impression of premalignant lesions including atrophic gastritis, gastric intestinal metaplasia, dysplasia and early cancer. This approach to quality improvement will reduce missed diagnoses and, combined with the latest endoscopic resection techniques performed at expert centres, will improve early detection and ultimately patient outcomes.
