Purpose. The study focuses on characteristics of public positions from the perspective of the communicative approach which is considered to be an actual practice of protest communication in the Russian Federation. The example taken in the study is the Russian Public Initiative petition platform.Methodology. The methodology is based on scientific theses on the contingence of technological and social and cultural changes, discursive reality construction, mediation of the political sphere and systemic-communicative autonomy of protest.Results. The spatiotemporal, collective-personal and objective systemic-communicative dimensions of Russian-speaking online petitions are described; their integrative nature is revealed; the specifics of the discursive construction of the problem, a complex addressee, a petition addressee and Internet solidarity are characterized.Practical implications. The results of the study make certain contribution to further understanding of modern protest, the correlation of technological and social and cultural changes, politics and mass media sphere; these results are of interest to specialists in the field of social conflicts management.
Process skill rather than motor skill seems to be a predictor of costs for rehabilitation after a stroke in working age; a longitudinal study with a 1 year follow up post discharge

Abstract

Background

In recent years a number of costs of stroke studies have been conducted based on incidence or prevalence and estimating costs at a given time. As there still is a need for a deeper understanding of factors influencing these costs the aim of this study was to calculate the direct and indirect costs in a younger (<65) sample of stroke patients and to explore factors affecting the costs.

Methods

Fifty-eight patients included in a study of home rehabilitation and followed for 1 year after discharge from the rehabilitation unit, were interviewed about their use of health care services, assistance, medications and assistive devices. Costs (defined as the cost for society) were calculated. A linear regression of cost and variables of functioning, ability, community integration and health-related quality of life was done.

Results

Inpatient care contributed substantially to the direct cost with a mean length of stay of 92 days. Rehabilitation during the first year constituted of an average of 28 days in day clinics, 38 physiotherapy sessions and 20 occupational therapy sessions. The total direct mean cost was 80 020 € and the indirect cost 35 129 €. The direct costs were influenced by the process skill (the ability to plan and perform a given task and to adapt when needed) and presence of aphasia. Indirect costs for informal care giving increased for patients with a lower health-related quality of life as well as a low score on home integration.

Conclusion

Costs are high in this group of young (< 65 years) stroke patients compared to other studies, partly due to the length of the stay and partly to loss of productivity.</p
Evaluation of Biological Properties of New Selective Differential Medium for Cholera Vibrios Isolation Based on the Results of Laboratоry Trials
Previously developed selective differential medium for V. cholerae growth was modernized. The modernized medium called SDMV-M was shown to possess the required sensitivity and germination index. The growth of E. coli was entirely inhibited, that of P. vulgaris was inhibited considerably. The medium possessed good differentiating ability: orange V. cholerae colonies were clearly distinguished from concomitant microorganisms. In the laboratory trials SDMV-M demonstrated some advantages as regards V. cholerae isolation from contaminated faeces in comparison with the reference medium TCBS
Choice of generic antihypertensive drugs for the primary prevention of cardiovascular disease - A cost-effectiveness analysis

Abstract

Background

Hypertension is one of the leading causes of cardiovascular disease (CVD). A range of antihypertensive drugs exists, and their prices vary widely mainly due to patent rights. The objective of this study was to explore the cost-effectiveness of different generic antihypertensive drugs as first, second and third choice for primary prevention of cardiovascular disease.

Methods

We used the Norwegian Cardiovascular Disease model (NorCaD) to simulate the cardiovascular life of patients from hypertension without symptoms until they were all dead or 100 years old. The risk of CVD events and costs were based on recent Norwegian sources.

Results

In single-drug treatment, all antihypertensives are cost-effective compared to no drug treatment. In the base-case analysis, the first, second and third choice of antihypertensive were calcium channel blocker, thiazide and angiotensin-converting enzyme inhibitor. However the sensitivity and scenario analyses indicated considerable uncertainty in that angiotensin receptor blockers as well as, angiotensin-converting enzyme inhibitors, beta blockers and thiazides could be the most cost-effective antihypertensive drugs.

Conclusions

Generic antihypertensives are cost-effective in a wide range of risk groups. There is considerable uncertainty, however, regarding which drug is the most cost-effective.</p
