Present and future pharmacotherapeutic agents in heart failure: an evolving paradigm

    Get PDF
Many conditions culminate in heart failure (HF), a multi-organ systemic syndrome with an intrinsically poor prognosis. Pharmacotherapeutic agents that correct neurohormonal dysregulation and haemodynamic instability have occupied the forefront of developments within the treatment of HF in the past. Indeed, multiple trials aimed to validate these agents in the 1980s and early 1990s, resulting in a large and robust evidence-base supporting their use clinically. An established treatment paradigm now exists for the treatment of HF with reduced ejection fraction (HFrEF), but there have been very few notable developments in recent years. HF remains a significant health concern with an increasing incidence as the population ages. We may indeed be entering the surgical era for HF treatment, but these therapies remain expensive and inaccessible to many. Newer pharmacotherapeutic agents are slowly emerging, many targeting alternative therapeutic pathways, but with mixed results. Metabolic modulation and manipulation of the nitrate/nitrite/nitric oxide pathway have shown promise and could provide the answers to fill the therapeutic gap between medical interventions and surgery, but further definitive trials are warranted. We review the significant evidence base behind the current medical treatments for HFrEF, the physiology of metabolic impairment in HF, and discuss two promising novel agents, perhexiline and nitrite

Sterile Abscess in the Myocardium after Direct Intramyocardial Injection Related to Gene Therapy in a Swine Model

Cardiac gene therapy is one of the most promising approaches to cure patients with cardiac dysfunctions. Many ways of efficient gene transfer using viral vectors are tested, and some of them are already used in clinical settings. However, it is always important to be keenly alert to the possible complications when a new therapy is introduced. We present a case of myocardial sterile abscess in a swine model associated with a direct myocardial injection

Contemporary communication and social interaction are being increasingly influenced by media and technology, systems, and platforms that are based on digital principles and artificial intelligence. To date, there has been no clear set of interdisciplinary theories or models that go beyond the academic discourses within the individual disciplines. There has therefore been no clear framework within which to classify the current opportunities, challenges and responsibilities facing a digital society and how to approach education from a theoretical and conceptual perspective. In 2019, in recognition of the fact that digitalization and AI cannot be viewed from a single disciplinary perspective, a group of authors – consisting of scholars from computer science, media education and media studies – developed the Frankfurt Triangle (Brinda et al. 2020). The Frankfurt Triangle is the result of intensive interdisciplinary discussion. Its goal is to develop a collaborative theoretical and conceptual model on which to base the perspectives and dimensions of education in a digital world. The initial impetus for this augmentation of the Dagstuhl Declaration from 2016 came from two workshops took place in Frankfurt am Main, Germany. Incorporating a threefold perspective on a) structures and functions of technology and media, b) interrelations with culture and society, and c) interaction in terms auf use, agency and subjectification, the model provides an orientational framework for the analysis, reflection and active creation of artifacts and phenomena in a world shaped by digital media, technology, systems, and platforms. The model was originally developed in German and has not yet been translated into other languages. Furthermore, scientists have repeatedly pointed out at international conferences and in academic exchanges that no attempts have yet been made in other language areas to develop a comparable interdisciplinary model. Hence, in order to make the approach accessible to international scholars, the model and accompanying text have been translated into English. This paper describes the model, its function and usage, and outlines possible additional developments and future connections. The introductory text also briefly describes the context in which the model was developed, and its development since its inception.

Linked to this reality, the objective of this research is to identify the factors that influence the process of business development through the perspectives of incubators and incubated companies. Three incubators and three incubated companies were selected to participate in semi-structured individual interviews. Data was collected by means of recording. Factors resulting from the interviews were grouped into macro factors, indicating that the factors of success or failure perceived by the respondents are the Entrepreneur's Profile, Incubator, Financial Resources, and Market. It is possible to conclude that the entrepreneur's profile is the strongest factor, from the incubators' perspective, since it was mentioned by all the respondents. In contrast, the incubator, and how it performs its functions, was cited by all incubated companies, thus indicating, from the entrepreneurs' perspective, the strongest factor

Heart failure is characterised by impaired function and disturbed Ca2+ homeostasis. Transgenic increases in inhibitor-1 activity have been shown to improve Ca2 cycling and preserve cardiac performance in the failing heart. The aim of this study is to evaluate the effect of activating the inhibitor (I-1c) of protein phosphatase 1 (I-1) through gene transfer on cardiac function in a porcine model of heart failure induced by myocardial infarction (MI)
