    Performance of Different Routings in Online Environment

    A recent trend in routing research is to avoid inefficiencies in network-level routing by allowing hosts to either choose routes themselves (e.g., source routing) or use overlay routing networks (e.g., Detour or RON). Such approaches result in selfish routing, because routing decisions are no longer based on system-wide criteria but are instead designed to optimize host-based or overlay-based metrics. A series of theoretical results showing that selfish routing can result in suboptimal system behavior have cast doubts on this approach. In this paper, we use a game-theoretic approach to investigate the performance of selfish routing in Internet-like environments based on realistic topologies and traffic demands in our simulations. We show that in contrast to theoretical worst cases, selfish routing achieves close to optimal average latency in such environments. However, such performance benefits come at the expense of significantly increased congestion on certain links. Moreover, the adaptive nature of selfish overlays can significantly reduce the effectiveness of traffic engineering by making network traffic less predictable

    Percutaneous vertebroplasty is not a risk factor for new osteoporotic compression fractures: results from VERTOS II

    Background and purpose: Pv is increasingly used as treatment for osteoporotic vcfs. However, controversy exists as to whether pv increases the risk for new vcfs during follow-up. The purpose of our research was to assess the incidence of new vcfs in patients with acute vcfs randomized to pv and conservative therapy. Materials and methods: Vertos ii is a prospective multicenter randomized controlled trial comparing pv with conservative therapy in 202 patients. Incidence, distribution, and timing of new vcfs during follow-up were assessed from spine radiographs. In addition, further height loss during follow-up of treated vcfs was measured. Results: After a mean follow-up of 11.4 Months (Median, 12.0; Range, 1-24 months), 18 New vcfs occurred in 15 of 91 patients after pv and 30 new vcfs in 21 of 85 patients after conservative therapy. This difference was not significant (P = .44). There was no higher fracture risk for adjacent-versus-distant vertebrae. Mean time to new vcf was 16.2 Months after pv and 17.8 Months after conservative treatment (Logrank, p = .45). The baseline number of vcfs was the only risk factor for occurrence (Or, 1.43; 95% Ci, 1.05-1.95) And number (P = .01) Of new vcfs. After conservative therapy, further height loss of treated vertebrae occurred more frequently (35 Of 85 versus 11 of 91 patients, p < .001) And was more severe (P < .001) Than after pv. Conclusions: Incidence of new vcfs was not different after pv compared with conservative therapy after a mean of 11.4 Months' follow-up. The only risk factor for new vcfs was the number of vcfs at baseline. Pv contributed to preservation of stature by decreasing both the incidence and severity of further height loss in treated vertebrae

    ANTIEPILEPTIC EFFECT OF CALCIUM CHANNEL BLOCKERS ALONE AND IN COMBINATION WITH PHENYTOIN ON MES INDUCED EPILEPSY IN RATS

    Background:&nbsp;The currently available antiepileptic drugs have a low therapeutic index, and provide satisfactory seizure control in only 60-70% of patients. Calcium channel blocker has shown potentials of a useful add-on drug for the existing antiepileptic drugs.&nbsp;Objectives:&nbsp;To study the effect of calcium channel blockers (CCBs) nifedipine and verapamil on maximal electroshock (MES)-induced convulsions and also to evaluate their effect in combination with Phenytoin. Methods:&nbsp;For this study, Male Wistar rats were used. Effects of nifedipine (5 mg/kg), verapamil (20mg/kg) alone and in combination with phenytoin (25mg/kg) were studied in MES model. Abolition of hind limb tonic extension and reduction of duration of tonic extension was considered as the index for antiepileptic activity in MES.&nbsp;Results:&nbsp;Verapamil and nifedipine produced significant antiepileptic effect alone. Verapamil and nifedipine potentiated the antiepileptic effect of phenytoin sodium.&nbsp;Conclusion:&nbsp;Dose of phenytoin sodium can be reduced in epileptic patients receiving verapamil or nifedipine for some other clinical conditions. Verapamil and nifedipine can also be used alone in the treatment of generalized tonic clonic seizures. However it needs further confirmation to establish clinical utility of calcium channel blockers.     Mechanisms for Introduction of Pseudo Ductility in Fiber Reinforced Polymer Composites- A Review

    Advanced polymer matrix composites are gaining the market in their way due to their exceptional specific stiffness, specific strength, fatigue, and corrosion resistance in the field of Auto-Tech, Aero-Tech, Biotech, etc. However, the lack of ductility and catastrophic failure has limited their application in these areas. Hence there is a need to explore means and protocols for designing the reduced factor of safety with high-performance toughened composites. To address this problem, a new generation of high-performance composites with pseudo-ductile or ductile behavior is needed. The ongoing High-Performance Ductile Composite Technology (HiPerDuCT) program jointly between the University of Bristol U.K and Imperial College London to address this challenge by developing newer materials. The fiber architectures made under this project gave a more gradual failure rather than catastrophic failure which improves the mechanical properties. This paper mainly focuses on addressing this evolution of pseudo ductility in fiber-reinforced composites. In addition to this, an attempt has been made to newer possible fiber positions in matrix materials for inducing reasonable ductility in composites

    A rare case report of neurolymphomatosis with NK-T-cell lymphoma

    Neurolymphomatosis (NL) is a rare entity. It is usually a manifestation of B-cell lymphoma. Here we describe a case of neurolymphomatosis in a 72 year old man who was in remission for Nasal T-cell lymphoma and presented to us with subacute onset painful mononeuritis like weakness involving all 4 limbs sequentially over a period of 4 months. he was initially treated as CIDP with steroids but later in view of progression he was evaluated further. His FDG PET-CT showed increased uptake in left brachial plexus which was confirmed by subsequent MRI neurography. He underwent biopsy of left brachial plexus and diagnosis of neurolymphomatosis was confirmed. He was treated with intravenous immunoglobulins (2 gm/kg) over 5 days with minimal improvement in his neurological symptoms, his shoulder pain improved and he could feed himself with right hand. He was offered palliative treatment in view of poor prognosis seen in neurolymphomatosis. He succumbed to the illness 3 months later
