Statistical multifragmentation model with discretized energy and the generalized Fermi breakup. I. Formulation of the model
The Generalized Fermi Breakup recently demonstrated to be formally equivalent
to the Statistical Multifragmentation Model, if the contribution of excited
states are included in the state densities of the former, is implemented. Since
this treatment requires the application of the Statistical Multifragmentation
Model repeatedly on the hot fragments until they have decayed to their ground
states, it becomes extremely computational demanding, making its application to
the systems of interest extremely difficult. Based on exact recursion formulae
previously developed by Chase and Mekjian to calculate the statistical weights
very efficiently, we present an implementation which is efficient enough to
allow it to be applied to large systems at high excitation energies. Comparison
with the GEMINI++ sequential decay code shows that the predictions obtained
with our treatment are fairly similar to those obtained with this more

Solitons in a medium with linear dissipation and localized gain
We present a variety of dissipative solitons and breathing modes in a medium
with localized gain and homogeneous linear dissipation. The system posses a
number of unusual properties, like exponentially localized modes in both
focusing and defocusing media, the existence of modes in focusing media at
negative propagation constant values, the simultaneous existence of stable
symmetric and anti-symmetric localized modes when the gain landscape possesses

We demonstrate the close similarity of a generalized Fermi breakup model, in
which densities of excited states are taken into account, to the microcanonical
statistical multifragmentation model used to describe the desintegration of

Time-Symmetric Quantum Theory of Smoothing
Smoothing is an estimation technique that takes into account both past and
future observations, and can be more accurate than filtering alone. In this
Letter, a quantum theory of smoothing is constructed using a time-symmetric
formalism, thereby generalizing prior work on classical and quantum filtering,
retrodiction, and smoothing. The proposed theory solves the important problem
of optimally estimating classical Markov processes coupled to a quantum system
under continuous measurements, and is thus expected to find major applications
in future quantum sensing systems, such as gravitational wave detectors and

collisions of charge asymmetric nuclei, we have performed a complete
systematical study using isospin dependent quantum molecular dynamics model.

Diagnostic accuracy of mediastinal width measurement on posteroanterior and anteroposterior chest radiographs in the depiction of acute nontraumatic thoracic aortic dissection
We aimed to explore the diagnostic accuracy of various mediastinal measurements in determining acute nontraumatic thoracic aortic dissection with respect to posteroanterior (PA) and anteroposterior (AP) chest radiographs, which had received little attention so far. We retrospectively reviewed 100 patients (50 PA and 50 AP chest radiographs) with confirmed acute thoracic aortic dissection and 120 patients (60 PA and 60 AP chest radiographs) with confirmed normal aorta. Those who had prior history of trauma or aortic disease were excluded. The maximal mediastinal width (MW) and maximal left mediastinal width (LMW) were measured by two independent radiologists and the mediastinal width ratio (MWR) was calculated. Statistical analysis was then performed with independent sample t test. PA projection was significantly more accurate than AP projection, achieving higher sensitivity and specificity. LMW and MW were the most powerful parameters on PA and AP chest radiographs, respectively. The optimal cutoff levels were LMW = 4.95 cm (sensitivity, 90 %; specificity, 90 %) and MW = 7.45 cm (sensitivity, 90 %; specificity, 88.3 %) for PA projection and LMW = 5.45 cm (sensitivity, 76 %; specificity, 65 %) and MW = 8.65 cm (sensitivity, 72 %; specificity, 80 %) for AP projection. MWR was found less useful and less reliable. The use of LMW alone in PA film would allow more accurate prediction of aortic dissection. PA chest radiograph has a higher diagnostic accuracy when compared with AP chest radiograph, with negative PA chest radiograph showing less probability for aortic dissection. Lower threshold for proceeding to computed tomography aortogram is recommended however, especially in the elderly and patients with widened mediastinum on AP chest radiograph. © 2012 The Author(s).published_or_final_versio
Percutaneous pulmonary valve implantation in humans - Results in 59 consecutive patients
Background - Right ventricular outflow tract (RVOT) reconstruction with valved conduits in infancy and childhood leads to reintervention for pulmonary regurgitation and stenosis in later life.Methods and Results - Patients with pulmonary regurgitation with or without stenosis after repair of congenital heart disease had percutaneous pulmonary valve implantation (PPVI). Mortality, hemodynamic improvement, freedom from explantation, and subjective and objective changes in exercise tolerance were end points. PPVI was performed successfully in 58 patients, 32 male, with a median age of 16 years and median weight of 56 kg. The majority had a variant of tetralogy of Fallot (n = 36), or transposition of the great arteries, ventricular septal defect with pulmonary stenosis (n = 8). The right ventricular (RV) pressure (64.4 +/- 17.2 to 50.4 +/- 14 mm Hg, P < 0.001), RVOT gradient (33 +/- 24.6 to 19.5 +/- 15.3, P < 0.001), and pulmonary regurgitation ( PR) (grade 2 of greater before, none greater than grade 2 after, P < 0.001) decreased significantly after PPVI. MRI showed significant reduction in PR fraction (21 +/- 13% versus 3 +/- 4%, P < 0.001) and in RV end-diastolic volume (EDV) (94 +/- 28 versus 82 +/- 24 mL (.) beat(-1) (.) m(-2), P < 0.001) and a significant increase in left ventricular EDV ( 64 +/- 12 versus 71 +/- 13 mL (.) beat(-1.) m(-2), P = 0.005) and effective RV stroke volume ( 37 +/- 7 versus 42 +/- 9 mL (.) beat(-1) (.) m(-2), P = 0.006) in 28 patients (age 19 +/- 8 years). A further 16 subjects, on metabolic exercise testing, showed significant improvement in V(O2)max (26 +/- 7 versus 29 +/- 6 mL (.) kg(-1) (.) min(-1), P < 0.001). There was no mortality.Conclusions - PPVI is feasible at low risk, with quantifiable improvement in MRI-defined ventricular parameters and pulmonary regurgitation, and results in subjective and objective improvement in exercise capacity
