Management and prognosis of uterine rupture during labor in an under-medicalized country: about 513 cases collected at the Cocody University Hospital Center (Abidjan-Cote d'Ivoire)
Background: Even today, uterine rupture is necessary as an indicator of health status in developing countries, like Cote d’Ivoire. The objective of this study was to describe the factors influencing the management and the prognosis of this pathology in a level III maternity of a third world country.Methods: The study was made in Cocody University Hospital Center (Abidjan-Cote d’Ivoire). A retrospective descriptive study of all women with ruptured uterus during labor managed between January 2002 and December 2014 was conducted. It covered 513 cases of uterine ruptures collected in 13 years.Results: The overall incidence of uterine ruptures was 0.95% or 1 in 105 deliveries. Most cases occurred in women with unscarred uterus (76.8%) and 23.2% of women had a scarred uterus. Surgical treatment was radical by hysterectomy in 35.3% of all women. Treatment was more conservative by uterine suture in women from the communes of Abidjan and its suburbs (71%) versus 25% of women who came from inland towns (p=0.000). Maternal mortality rate was 5.8% and was significantly influenced by the type of surgery (p=0.000), by the time of uterine rupture (p=0.000) and by the transportation distance (p=0.000). Fetal mortality was 94.1% for all women.Conclusions: Uterine rupture still poses a major public health problem in under-developed countries. The multiplication of obstetric surgical units but also the availability of blood products and the effectiveness of free care will be a vital contribution to effectively and sustainably improve the prognosis of this serious pathology when just constituted
Microplastics, microfibers and associated microbiota biofilm analysis in seawater, a case study from the Vesuvian Coast, southern Italy
The growing concerns regarding pollution from microplastics (MPs) and microfibers (MFs) have driven the scientific community to develop new solutions for monitoring ecosystems. However, many of the proposed technologies still include protocols for treating environmental samples that may alter plastic materials, leading to inaccurate results both in observation and in counting. For this reason, we are refining a protocol, based on optical microscopy without the use of pretreatments, applicable to different environmental matrices, which allows not only counting but also a complete morphological characterization of MPs and MFs. Previously, the protocol has successfully been tested on marine sediments from the Vesuvian area of the Gulf of Naples (Italy) with good results. In the present study, we tested the protocol on MPs and MFs in seawater samples collected environments. The protocol enabled not only the morphological characterization of MPs and MFs but also the collection of information on the colonies of microorganisms present on the microparticles. Next Generation Sequencing (NGS) metagenomic technologies enabled us to characterize the microbiota composition of the sampled MPs, the so-called Plastisphere. The analytical approach allowed the characterization of several potentially pathogenic bacteria, which represent a potential threat to the environment and human health. In fact, they may exploit their ability to form biofilms on plastics to proliferate in marine ecosystems
A Victor's History: A Comparative Analysis of the Labour Historiography of Indonesia's New Order
Some observers have identified a common pattern in developing countries whereby unions are transformed from a political force valued for their contribution to the struggle for independence to a state-sponsored ‘tool of development’. A less well-explored question concerns the harnessing of labour historiography to justify such transitions. As this article shows, Suharto’s New Order (1966–98) undertook a conscious and purposeful rewriting of Indonesian labour history in support of a single vehicle of labour representation organized around a narrative of the dangers of political unionism and designed to control and harness the industrial workforce in the name of economic development
Clinical knee findings in floor layers with focus on meniscal status
<p>Abstract</p> <p>Background</p> <p>The aim of this study was to examine the prevalence of self-reported and clinical knee morbidity among floor layers compared to a group of graphic designers, with special attention to meniscal status.</p> <p>Methods</p> <p>We obtained information about knee complaints by questionnaire and conducted a bilateral clinical and radiographic knee examination in 134 male floor layers and 120 male graphic designers. After the exclusion of subjects with reports of earlier knee injuries the odds ratio (OR) with 95% confidence intervals (CI) of knee complaints and clinical findings were computed among floor layers compared to graphic designers, using logistic regression. Estimates were adjusted for effects of body mass index, age and knee straining sports. Using radiographic evaluations, we conducted side-specific sensitivity analyses regarding clinical signs of meniscal lesions after the exclusion of participants with tibiofemoral (TF) osteoarthritis (OA).</p> <p>Results</p> <p>Reports of knee pain (OR = 2.7, 95% CI = 1.5–4.6), pain during stair walking (OR = 2.2, 95% CI = 1.3–3.9) and symptoms of catching of the knee joint (OR = 2.9, 95% CI = 1.4–5.7) were more prevalent among floor layers compared to graphic designers. Additionally, significant more floor layers than graphic designers had clinical signs suggesting possible meniscal lesions: a positive McMurray test (OR = 2.4, 95% CI = 1.1–5.0) and TF joint line tenderness (OR = 5.4, 95% CI = 2.4–12.0). Excluding floor layers (n = 22) and graphic designers (n = 15) with radiographic TF OA did not alter this trend between the two study groups: a positive McMurray test (OR = 2.2, 95% CI = 1.0–4.9), TF joint line tenderness (OR = 5.0, 95% CI = 2.0–12.5).</p> <p>Conclusion</p> <p>Results indicate that floor layers have a high prevalence of both self-reported and clinical knee morbidity. Clinical knee findings suggesting possible meniscal lesions were significant more prevalent among floor layers compared to a group of low-level exposed graphic designers and an association with occupational kneeling could be possible. However, causality cannot be confirmed due to the cross-sectional study design.</p

Stratified Medicine Paediatrics: Cell free DNA and serial tumour sequencing identifies subtype specific cancer evolution and epigenetic states
We profiled a large heterogenous cohort of matched diagnostic-relapse tumour tissue and paired plasma-derived cell free DNA (cfDNA) from patients with relapsed and progressive solid tumours of childhood. Tissue and cfDNA sequencing results were concordant, with a wider spectrum of mutant alleles and higher degree of intra-tumour heterogeneity captured by the latter, if sufficient circulating tumour-derived DNA (ctDNA) was present. Serial tumour sequencing identified putative drivers of relapse, with alterations in epigenetic drivers being a common feature. In keeping with epigenetic alterations being a common driver of many childhood cancers, fragmentomics analysis of cfDNA identified tumour-specific epigenetic states and transcription factor binding sites accessible in chromatin. This study leverages a large and well-annotated genomic dataset of aggressive childhood malignancies, identifies genomic and epigenetic drivers of childhood cancer relapse, and highlights the power and practicality of cfDNA analysis to capture both intra-tumoural heterogeneity and the epigenetic state of cancer cells
Benefits for children with suspected cancer from routine whole-genome sequencing
Clinical whole-genome sequencing (WGS) has been shown to deliver potential benefits to children with cancer and to alter treatment in high-risk patient groups. It remains unknown whether offering WGS to every child with suspected cancer can change patient management. We collected WGS variant calls and clinical and diagnostic information from 281 children (282 tumors) across two English units (n = 152 from a hematology center, n = 130 from a solid tumor center) where WGS had become a routine test. Our key finding was that variants uniquely attributable to WGS changed the management in ~7% (20 out of 282) of cases while providing additional disease-relevant findings, beyond standard-of-care molecular tests, in 108 instances for 83 (29%) cases. Furthermore, WGS faithfully reproduced every standard-of-care molecular test (n = 738) and revealed several previously unknown genomic features of childhood tumors. We show that WGS can be delivered as part of routine clinical care to children with suspected cancer and can change clinical management by delivering unexpected genomic insights. Our experience portrays WGS as a clinically impactful assay for routine practice, providing opportunities for assay consolidation and for delivery of molecularly informed patient care.</p
Stratified Medicine Pediatrics: Cell-Free DNA and Serial Tumor Sequencing Identifies Subtype-Specific Cancer Evolution and Epigenetic States
Trabecular Reorganization in Consecutive Iliac Crest Biopsies when Switching from Bisphosphonate to Strontium Ranelate Treatment
BACKGROUND: Several agents are available to treat osteoporosis while addressing patient-specific medical needs. Individuals' residual risk to severe fracture may require changes in treatment strategy. Data at osseous cellular and microstructural levels due to a therapy switch between agents with different modes of action are rare. Our study on a series of five consecutively taken bone biopsies from an osteoporotic individual over a six-year period analyzes changes in cellular characteristics, bone microstructure and mineralization caused by a therapy switch from an antiresorptive (bisphosphonate) to a dual action bone agent (strontium ranelate). METHODOLOGY/PRINCIPAL FINDINGS: Biopsies were progressively taken from the iliac crest of a female patient. Four biopsies were taken during bisphosphonate therapy and one biopsy was taken after one year of strontium ranelate (SR) treatment. Furthermore, serum bone markers and dual x-ray absorptiometry measurements were acquired. Undecalcified histology was used to assess osteoid parameters and bone turnover. Structural indices and degree of mineralization were determined using microcomputed tomography, quantitative backscattered electron imaging, and combined energy dispersive x-ray/µ-x-ray-fluorescence microanalysis. CONCLUSIONS/SIGNIFICANCE: Microstructural data revealed a notable increase in bone volume fraction after one year of SR treatment compared to the bisphosphonate treatment period. Indices of connectivity density, structure model index and trabecular bone pattern factor were predominantly enhanced indicating that the architectural transformation from trabecular rods to plates was responsible for the bone volume increase and less due to changes in trabecular thickness and number. Administration of SR following bisphosphonates led to a maintained mineralization profile with an uptake of strontium on the bone surface level. Reactivated osteoclasts designed tunneling, hook-like intratrabecular resorption sites. The appearance of tunneling resorption lacunae and the formation of both mini-modeling units and osteon-like structures within increased plate-like cancellous bone mass provides additional information on the mechanisms of strontium ranelate following bisphosphonate treatment, which may deserve special attention when monitoring a treatment switch
