Development of a theoretical-practical script for clinical simulation
Abstract OBJECTIVE To develop a theoretical-practical script based on the opinion of experts to be used in simulated clinical activities. METHOD Qualitative study through analysis of content of interviews with experts on the theme in order to develop the proposed script. Of the 24 invited experts, 12 specialists from educational institutions in Brazil and abroad participated in the study in compliance with the ethical precepts. The experts responded to questions on the characterization of their study attributes and described the items required for the development of a simulated scenario. In view of the responses obtained, data content was analyzed and classified into units and subunits of significance. RESULTS The items mentioned for the development of the script generated seven units of significance. The units and subunits of significance were gathered in three stages of the main components of the simulated scenario: prior, preparation, and finals. CONCLUSION This study enables an innovative, stimulating teaching experience, making it easier for professors to use the simulation resource as a learning process in an effective and objective manner, as a guide to professors and researchers in the area of clinical simulation
Genetic variation in HSD17B13 reduces the risk of developing cirrhosis and hepatocellular carcinoma in alcohol misusers
BACKGROUND & AIMS
Carriage of rs738409:G in patatin-like phospholipase domain-containing 3 (PNPLA3) is associated with an increased risk for developing alcohol-related cirrhosis and hepatocellular carcinoma (HCC). Recently, rs72613567:TA in hydroxysteroid 17-beta dehydrogenase 13 (HSD17B13) was shown to be associated with a reduced risk for developing alcohol-related liver disease and to attenuate the risk associated with PNPLA3 rs738409:G. This study explores the risk-associations between these two genetic variants and the development of alcohol-related cirrhosis and HCC.
APPROACH AND RESULTS
Variants in HSD17B13 and PNPLA3 were genotyped in 6,171 participants, including: 1,031 with alcohol-related cirrhosis and HCC; 1,653 with alcohol-related cirrhosis without HCC; 2,588 alcohol misusers with no liver disease; and 899 healthy controls. Genetic associations with the risks for alcohol-related cirrhosis and HCC were determined using logistic regression analysis. Carriage of HSD17B13 rs72613567:TA was associated with a lower risk for both cirrhosis (OR 0.79 [95% CI 0.72-0.88], p=8.13×10-6) and HCC (OR 0.77 [95% CI 0.68-0.89], p=2.27×10-4), while carriage of PNPLA3 rs738409:G was associated with an increased risk for developing cirrhosis (OR 1.70 [95% CI 1.54-1.88], p=1.52x10-26) and HCC (OR 1.77 [95% CI 1.58-1.98], p=2.31×10-23). These associations remained significant after adjusting for age, sex, body mass index, type II diabetes mellitus and country. Carriage of HSD17B13 rs72613567:TA attenuated the risk for developing cirrhosis associated with PNPLA3 rs738409:G in both men and women but the protective effect against the subsequent development of HCC was only observed in men (p=1.72×10-4; ORallelic, 0.75; 95% CI, 0.64-0.87).
CONCLUSIONS
Carriage of variants in PNPLA3 and HSD17B13 differentially affect the risk for developing advanced alcohol-related liver disease. A genotypic/phenotypic risk score might facilitate earlier diagnosis of HCC in this population
Using technology to teach anatomy and physiology
The teaching of anatomy and physiology to health-care students has always raised challenges for staff. The diversity of ability of the students and the need to apply theoretical concepts to clinical practice are particularly difficult for some lecturers. These lecturers may be non-clinical physiologists or clinical specialists with less detailed understanding of the scientific rationale. This project looks at the development of a new teaching strategy using computer software to supplement existing teaching methods to support a range of pre-registration health-care students in learning anatomical and physiological principles

High-Fidelity Patient Simulation in Critical Care Area: A Methodological Overview
High-Fidelity Patient Simulation (HFPS) allows the nursing students’ immersion in a learning environment marked by complexity, dynamism, reality, and safety, connecting theory to practice. The increased use of the HFPS was also sustained by the beneficial impact that HFPS seems to produce on nursing students’ learning outcomes. However, available research is still conflicting, and several methodological issues do not allow achieving best-quality evidence. To analyse the impact of the HFPS on nursing students’ learning outcomes in critical care area, a methodological overview of the studies included in a systematic review was conducted. Heterogeneity about the geographic area, methods, simulation techniques, and control approaches was pointed out. However, studies based on cardiocirculatory scenarios showed the highest standardization of methods and assessment tools. The development and application of shared guidelines derived from adequately-sampled randomized controlled trials are expected to demonstrate the high-fidelity simulation to be a valuable adjunct to the traditional training program
