Frame me if you must: PrEP framing and the impact on adherence to HIV Pre-exposure Prophylaxis
Abstract
Background
“PrEP whore” has been used both as a pejorative by PrEP opponents in the gay community and, reactively, by PrEP advocates as a method to reclaim the label from stigmatization and “slut-shaming.” The actual prevalence and impact of such PrEP-directed stigma on adherence have been insufficiently studied.
Methods
CCTG 595 was a randomized controlled PrEP demonstration project in 398 HIV-uninfected MSM and transwomen. Intracellular tenofovir-diphosphate (TFV-DP) levels at weeks 12 and 48 were used as a continuous measure of adherence. At study visits, participants were asked to describe how they perceived others’ reactions to them being on PrEP. These perceptions were categorized a priori as either “positively framed,” “negatively framed,” or both. We used Wilcoxon rank-sum to determine the association between positive and negative framing and TFV-DP levels at weeks 12 and 48.
Results
By week 4, 29% of participants reported perceiving positive reactions from members of their social groups, 5% negative, and 6% both. Reporting decreased over 48 weeks, but positive reactions were consistently reported more than negative. At week 12, no differences in mean TFV-DP levels were observed in participants with positively-framed reactions compared with those reporting no outcome or only negatively-framed (1338 [IQR, 1036-1609] vs. 1281 [946-1489] fmol/punch, P = 0.17). Additionally, no differences were observed in those with negative reactions vs. those without (1209 [977–1427] vs. 1303 [964–1545], P = 0.58). At week 48, mean TFV-DP levels trended toward being higher among those that report any reaction, regardless if positive (1335 [909–1665] vs. 1179 [841–1455], P = 0.09) or negative (1377 [1054–1603] vs. 1192 [838–1486], P = 0.10) than those reporting no reaction. At week 48, 46% of participants reported experiencing some form of PrEP-directed judgment, 23% reported being called “PrEP whore,” and 21% avoiding disclosing PrEP use.
Conclusion
Over 48 weeks, nearly half of participants reported some form of judgment or stigmatization as a consequence of PrEP use. However, individuals more frequently perceived positively framed reactions to being on PrEP than negative. Importantly, long-term PrEP adherence does not appear to suffer as a result of negative PrEP framing.

Spectrum and Bethe-Salpeter amplitudes of baryons from lattice QCD
The baryons with are studied on the lattice
in the quenched approximation. Their mass levels are ordered as
, as is expected from the
constituent quark model. The mass values are also close to those of the four
states observed in experiments, respectively. We calculate the
Bethe-Salpeter amplitudes of and and find there
is a radial node for the Bethe-Salpeter amplitude, which may
imply that is an orbital excitation of baryons as a
member of the supermultiplet in the quark model description. Our results are helpful for identifying the

Recent HIV Risk Behavior and Partnership Type Predict PrEP Adherence in Men Who Have Sex with Men
Abstract
Background
Individuals engaging in higher risk behavior are often more adherent to PrEP but it is unclear if partnership type itself affects PrEP adherence. We examined the effect of recent HIV risk behaviors and partnership type on PrEP adherence in men who have sex with men (MSM) taking PrEP.
Methods
CCTG 595 is a 48-week PrEP demonstration study of 398 HIV− at-risk MSM. At baseline and week 48, HIV risk score was estimated as the probability of seroconversion over the next year based on number of condomless anal sex acts with HIV+/unknown partners in the last month and any STI diagnosed at study visit. HIV risk score was categorized as low (<0.12), moderate (0.12−0.59) and high (>0.59) risk based on population seroconversion probabilities. Partnership type was assigned as no/single HIV− partner, single HIV+ partner, or multiple partners of any serostatus in the past 3 months. PrEP adherence was estimated by intracellular tenofovir-diphosphate (TFV-DP) levels as a continuous variable at week 48. Statistical methods included McNemar’s test, Wilcoxon rank-sum test, and linear regression model where appropriate.
Results
Of 313 MSM who completed week 48, there was no significant change in HIV risk category from baseline to week 48 (low: 44 to 42%; moderate: 27 to 24%; high: 28 to 34%; P = 0.25). There was a significant change in partnership type, with the proportion of those with no or single HIV− partnerships increasing (1 to 9%, P < 0.001). In univariate analysis, moderate and high-risk groups had higher TFV-DP levels than the low-risk group at week 48 (P = 0.018). Participants with no/single HIV− partner had significantly lower TFV-DP levels than those with one HIV+ partner or multiple partners (P = 0.007). In a multivariable linear regression model, only low-risk partnerships remained significant where no/single HIV− partnerships were associated with lower TFV-DP levels (mean difference = −344fmol/punch [−617, −71], P = 0.014).
Conclusion
Although there was a shift in partnership type towards lower risk partnerships, objective HIV risk behavior remained stable over time. Individuals with higher HIV risk behaviors and risk partnerships had higher TFV-DP levels suggesting maintained strong motivation for PrEP adherence. Thus, recent sexual risk behavior and partnership type may be important predictors of PrEP adherence in MSM.

DEFENS - Drug Exposure Feedback and Education for Nurses’ Safety: study protocol for a randomized controlled trial
Abstract
Background
Three decades of research findings have documented the health effects of handling hazardous drugs. Oncology nurses are vulnerable due to frequent administration of antineoplastics, low adherence to equipment use, reported barriers to use, and perceived low risk of health effects. No interventions have been tested in a controlled, multi-site trial to increase nurses’ use of protective equipment when handling hazardous drugs. The Drug Exposure Feedback and Education for Nurses’ Safety (DEFENS) study will compare the efficacy of education (control) versus an audit and feedback intervention (treatment) on nurses’ self-reported use of personal protective equipment when handling hazardous drugs. The treatment intervention will include tailored messages based on nurses’ reported barriers to protective equipment use.
Methods/Design
The DEFENS Study is a cluster randomized controlled trial. We are enrolling cancer centers and will recruit nurse participants in April 2015. Eligible cancer centers employ at least 20 eligible registered nurses in the chemotherapy infusion setting and have on-site phlebotomy resources. Eligible participants are nurses who work at least 0.40 full-time equivalent hours in the chemotherapy infusion setting and have not received an antineoplastic drug for a health problem in the past year. An encrypted, user-authenticated website will administer surveys and deliver control and treatment interventions. The primary endpoint is the change in score on nurses’ reports of the Revised Hazardous Drug Handling Questionnaire between baseline and approximately 18 months later. A baseline survey is completed after informed consent and is repeated 18 months later. Nurses in all sites who experience a drug spill will also report incidents as they occur; these reports inform the treatment intervention. Plasma will be obtained at baseline, approximately 18 months later (the primary endpoint), and with drug spill occurrences to measure hazardous drugs levels and to inform the treatment intervention. Potential mediators include knowledge of hazardous drug handling and perceived risk of drug exposure. We will examine whether personal factors and organizational factors moderate the intervention effects.

Toroidal modelling of plasma response and RMP field penetration
The penetration dynamics of the resonant magnetic perturbation (RMP) field is sim-
ulated in the full toroidal geometry, under realistic plasma conditions in MAST experiments.
The physics associated with several aspects of the RMP penetration - the plasma response
and rotational screening, the resonant and non-resonant torques and the toroidal momentum
balance - are highlighted. In particular, the plasma response is found to significantly amplify
the non-resonant component of the RMP field for some of the MAST plasmas. A fast rotating
plasma, in response to static external magnetic fields, experiences a more distributed electro-
magnetic torque due to the resonance with continuum waves in the plasma. At fast plasma
flow (such as for the MAST plasma), the electromagnetic torque is normally dominant over
the neoclassical toroidal viscous (NTV) torque. However, at sufficiently slow plasma flow,
the NTV torque can play a significant role in the toroidal momentum balance, thanks to the
precession drift resonance enhanced, so called superbanana plateau regime
New Open Conformation of SMYD3 Implicates Conformational Selection and Allostery
SMYD3 plays a key role in cancer cell viability, adhesion, migration and invasion. SMYD3 promotes formation of inducible regulatory T cells and is involved in reducing autoimmunity. However, the nearly “closed” substrate-binding site and poor in vitro H3K4 methyltransferase activity have obscured further understanding of this oncogenically related protein. Here we reveal that SMYD3 can adopt an “open” conformation using molecular dynamics simulation and small-angle X-ray scattering. This ligand-binding-capable open state is related to the crystal structure-like closed state by a striking clamshell-like inter-lobe dynamics. The two states are characterized by many distinct structural and dynamical differences and the conformational transition pathway is mediated by a reversible twisting motion of the C-terminal domain (CTD). The spontaneous transition from the closed to open states suggests two possible, mutually non-exclusive models for SMYD3 functional regulation and the conformational selection mechanism and allostery may regulate the catalytic or ligand binding competence of SMYD3. This study provides an immediate clue to the puzzling role of SMYD3 in epigenetic gene regulation
New result for the neutron -asymmetry parameter from UCNA
The neutron -decay asymmetry parameter defines the correlation
between the spin of the neutron and the momentum of the emitted electron, which
determines , the ratio of the axial-vector to
vector weak coupling constants. The UCNA Experiment, located at the Ultracold
Neutron facility at the Los Alamos Neutron Science Center, is the first to
measure such a correlation coefficient using ultracold neutrons (UCN).
Following improvements to the systematic uncertainties and increased

A Unified Approach to the Classical Statistical Analysis of Small Signals
We give a classical confidence belt construction which unifies the treatment
of upper confidence limits for null results and two-sided confidence intervals
for non-null results. The unified treatment solves a problem (apparently not
previously recognized) that the choice of upper limit or two-sided intervals
leads to intervals which are not confidence intervals if the choice is based on
the data. We apply the construction to two related problems which have recently
been a battle-ground between classical and Bayesian statistics: Poisson
processes with background, and Gaussian errors with a bounded physical region.
In contrast with the usual classical construction for upper limits, our
construction avoids unphysical confidence intervals. In contrast with some
popular Bayesian intervals, our intervals eliminate conservatism (frequentist
coverage greater than the stated confidence) in the Gaussian case and reduce it
to a level dictated by discreteness in the Poisson case. We generalize the
method in order to apply it to analysis of experiments searching for neutrino
oscillations. We show that this technique both gives correct coverage and is
powerful, while other classical techniques that have been used by neutrino

Limits on Light Weakly Interacting Massive Particles from the First 102.8 kg day Data of the CDEX-10 Experiment
We report the first results of a light weakly interacting massive particles
(WIMPs) search from the CDEX-10 experiment with a 10 kg germanium detector
array immersed in liquid nitrogen at the China Jinping Underground Laboratory
with a physics data size of 102.8 kg day. At an analysis threshold of 160 eVee,
improved limits of 8 and 3 cm at a
90\% confidence level on spin-independent and spin-dependent WIMP-nucleon cross
sections, respectively, at a WIMP mass () of 5 GeV/ are

\psip events accumulated at the BESIII detector at the BEPCII
